Diabetic Shoe Program
Use this program to provide your customers with access to diabetic
footwear, improving their wellbeing as well as your business.
What benefits will this program provide to you?
 You will gain a competitive advantage by providing a service that many
pharmacies do not provide.
 Providing this service to patients in need will also give you an additional
relationship to retain them as a customer.
 Diabetic patients also have a higher incidence of prescriptions. Retaining
them will increase your prescription fills and your income.
How does the program work?
 Pharmacies will be compensated a fee by Sports and Wellness for service
to evaluate, fit and follow up with patients in their facility.
 The pharmacy will provide the in person evaluation, fitting and follow up
with the patient, and will complete the required forms provided by Sports
and Wellness.
 Customer Service will be provided for billing, documentation requirements,
vetting qualifying conditions, education resources, and marketing.
Sports and Wellness will be responsible for:
 Patient Identification
 Insurance Verification & Authorization
 Billing and Collection
 Compensating Pharmacies within 30 days
Pharmacies will be responsible for:
 Assisting in patient identification and marketing
 Scheduling patient upon approval
 Completing in person evaluation, fitting and follow up including support
documentation
 Proving additional follow up or warranty appointments as necessary
