Alaska Native Health Campus
- The Community Health Aide Program (CHAP) is a primary care model for health care
delivery with established village to tertiary care medical center referral patterns.
Gambell Village Clinic
- In response to the 1950s Tuberculosis epidemic, this unique program was implemented
to meet the needs of Alaska’s Native population located in remote, sparsely populated
areas of the state.
- Program materials including this, the fourth edition CHAM, are specifically designed to
guide every patient care encounter through the History, Exam, Assessment and Plan
(HEAP) format.
