Notice of Privacy Practices (Effective June, 6 2006)
To our consumers: This notice describes how health information about you, as a consumer of our
services, may be used and disclosed, and how you can get access to hour health information.
This is required by the Privacy Regulations created as a result of the Health Insurance Portability
and Accountability Act of 1996 (HIPAA).
Our commitment to your privacy:
Share & Care for Special People, Inc. is dedicated to maintaining the privacy of your health
information. We are required by law to maintain the confidentiality of your health information.
This notice applies to the medical/health records that are generated in or by Share & Care We are
required by law to follow the notice that is most current.
We may obtain, but are not required to, your consent for the use or disclosure of your protected
health information for treatment, payment or health care operations. We are required to obtain
your authorization for the use or discloser of your information for other specific purpose or
reasons. We have listed some of the types of uses or disclosures below. Not every possible use or
disclosure is covered, but all of the ways that we are allowed to use and disclose information will
fall in to one of the categories.
Department of Mental Health Services: We realized these laws are complicated, but we must
provide you with this important information. You may be receiving services as part of an
Organized Health Care Arrangement with the Department of Mental Health. In that case, Share
& Care is required to follow the terms of their Notice of Privacy Practices. In other situations, we
may use other individuals or providers to provide services to you and shall follow the terms of
this Notice of Privacy Practices.
In these cases, we may share medication information with each other that we serve in common
for the purpose of treatment, payment health care operations as those terms are described later in
this Notice of Privacy Practices.

Who is required to follow this notice:
All of our employees, staff, contracted employees, volunteers and certain others whose work is
controlled by us are required to follow this Notices of Privacy Practices.
Use and Disclosure of your health information in certain special circumstances:
We will make a good faith effort to have you acknowledge your copy of the Notice of Privacy
Practices. We can use or disclosure medical information about your treatment, payment for
services, or for facility operations. All of the ways we are permitted to use and disclose
information will fall into one of the following categories, though not every reason or example of
use or disclosure can be listed.
