An Important Privacy Notice from
Coast National Insurance Company
In the course of doing business with you, we collect personal information about you to provide you with
our products and services. We treat it as confidential. We value your business. We also value your
confidence in us. This notice describes our privacy practices. It also gives you a privacy choice. We ask
you to review it with care.
This notice describes our privacy practices for personal information about our California customers that
obtain products or services from us for personal, family, or household purposes. It is required by law. This
notice also applies to information about our former customers.
Personal information we collect.
We collect information about you and the members of your household (“you”). We may collect it from
more than one source. We do so to provide you with the coverage, product, or service you request. We
also may collect it for other purposes, such as to process claims, investigate potential fraud, or to service
your account.
• We collect information you give us on applications or other forms. Examples include your social
security number, prior insurance, and driving safety record;
• We collect information about your transactions with us, our affiliates, or others. Examples include your
policy coverage, premiums, and payment history; and
• We also may collect information we receive from a consumer reporting agency or insurance support
organization (“ISO”) or other third party. Examples include investigative reports, motor vehicle records
or claims history. Reports that we receive from a consumer reporting agency or an ISO also may be
kept by that organization and disclosed to other parties.
How we protect your personal information.
At Coast National, our customers are our most valued assets. Your privacy is important to us. We limit the
access of our agents and employees to information about you to those who need it to do their jobs. We
require those to whom we permit access to your information to protect it. We maintain safeguards that
comply with applicable law to guard your information. These include physical, electronic, and procedural
safeguards.
We do not disclose any information about you, as our customer or former customer, except as described
in this notice.
Access and correct your personal information.
You may request access to certain information about you that we have in our records. To request access,
please send us a written request. Be sure to reasonably describe the information you want. If you believe
that your information is incomplete or inaccurate, you may request that we make changes. Please send
any of the requests listed above in writing to: Policy Services Manager, PO Box 31029, Independence,
OH 44131-0029.
If you request corrections, additions, or deletions, we will either make the changes that you request or
notify you why we will not do so. If we decline your request, in some states you may have the right to file
a concise statement with us about the dispute.
The rights in this section do not apply in certain cases, such as information related to litigation.
Personal information we disclose.
We may disclose any information we collect about you as permitted by law and without your prior
authorization. Examples include:
• To those who need this information to perform normal business, professional, or insurance, functions
for us;
• To another insurance company, agent, self-insurer, or ISO, to detect or prevent criminal activity or
fraud in connection with an insurance transaction, or to perform an insurance transaction involving
you;
• To a medical professional or institution, to verify insurance coverage or benefits or to inform an
individual of a medical problem of which the individual may not be aware;
• To law enforcement or another government authority;
• To an insurance regulatory authority;
• To persons conducting actuarial or research studies;
• To a party to a proposed or consummated sale, transfer, merger, or consolidation of all or part of our
company;
• To persons that perform marketing services on our behalf, as permitted by law;
• To a group policyholder to report claims experience or conduct an audit of our operations, but only as
necessary to conduct the review or audit;
• To affiliates, as permitted by law (discussed in more detail in the next section);
• To other non-affiliated third parties, as permitted or required by law or legal process;
• To professional peer review organizations to review the conduct of a medical-care institution or
professional;
• To your agent or broker, who may need the information to service your policy;
• To a government authority so that it may determine eligibility for health benefits for which the authority
is liable;
• To a certificate or policyholder for the purpose of providing information regarding the status of an
insurance transaction;
• To a lien holder, lessor, or other person identified in our records as having a legal or beneficial
interest in a policy; and
• To an insured when the information disclosed is from certain accident reports, supplemental reports,
and investigative reports.
We also may disclose information with your written consent.
We are permitted to disclose health information to process your transaction with us. This may include, for
instance, to process claims or to prevent fraud. We also may make disclosures with your written
authorization and others allowed by law.
Personal information we share with our affiliates.
We have a number of affiliates, including members of the Bristol West Insurance Group and the Farmers
Insurance Group of Companies®. Sharing your information with them helps us to offer you a more
complete range of products and services.
We may share any of the information we collect with our affiliates to the extent allowed by law. We have
several types of affiliates. Some are financial service providers. These include insurance companies,
investment firms, underwriters and brokers/dealers, for example. Some are other types of service
providers, such as management companies and billing firms.
We are allowed by law to share with our affiliates our transaction and experience information with you
without your authorization. To the extent allowed by law, we also may share information about you with
our affiliates for marketing purposes, without your authorization.
We also may share consumer report information that we have received from you and from third parties,
such as consumer reporting agencies and insurance support organizations, with our affiliates. This may
include certain application information you provide. This also may include information we obtain in
consumer reports or other third party information. You have a choice about this type of sharing. Please
see the next section.
You have a choice.
If you prefer that we not share consumer report information with our affiliates, except as otherwise
permitted by law, you may use the Opt-out link below. We will implement your request within a reasonable
time after we receive the form. Any policyholder may opt-out on behalf of other joint policyholders. An optout by any joint policyholder will be deemed to be an opt-out by all policyholders of the policy.
If it is your decision not to opt-out and to allow sharing of your information with our affiliates, you do not
need to request an Opt-Out or respond to us in any way.
If two or more consumers jointly obtain an insurance product from us, one consumer can opt-out for
everyone on that policy.
We may change this privacy notice.
We may change our privacy practices in the future. If we make changes, we will update our privacy
notice, if necessary. We also will provide you with a revised notice. If we want to disclose your personal
information for a purpose not already permitted by law, we will give you a choice before we disclose your
information.
Other Privacy Policies.
Any rights you may have as described in this notice that you may have as a consumer, claimant, or
beneficiary, are not limited by other privacy notices that we may use.

Restrict Information Sharing With Our Affiliated Companies
As discussed in the “You have a choice” section above, unless you say “no” (opt-out), we may share
consumer report information about you with our affiliated companies. To Opt- out you may use the Optout link located on the CA Customer Privacy Policy on www.bristolwest.com.
