INSTRUCTION SHEET for SWPPP REVIEW FORMS
Instructions:
These steps below demonstrate when each of the specific SWPPP Review Forms are to be submitted to the
Town for a project that is required to provide post-construction Stormwater Management Practices (SMPs).
Those projects required to prepare only an Erosion and Sediment Control (E&SC) Plan must complete the
SWPPP Application Review Checklist when a Site Plan or Subdivision Application is submitted to the Town.

Required steps for the submission of a Sketch Plan Application to the DPC
Form 1:
SMP Selection Matrices Form
(Contains matrices for the selection of appropriate Stormwater Management Practices)
Step 1: Submit completed form to the Development Planning Committee (DPC)
Step 2: Engineering Division staff provide direct feedback on proposed SMP(s)
Proceed to the next step if continuing with the Town project approval process

Required steps for the submission of a Site Plan or Subdivision Application
Form 2:
SWPPP Application Review Checklist
(Contains the minimum required components for a SWPPP based on the type of project)
Form 3:
SMP Design Review Worksheet
(Contains required elements for the specific types of Stormwater Management Practices)
Step 3: Submit forms, the SWPPP, and the Site Plans to Economic Development &
Planning (ED&P) with the Stormwater Management Certification Form - Part A
Step 4: Engineering Staff reviews documents and provides ED&P with comments
Step 5: ED&P coordinates Planning Board approval of the SWPPP and Site Plans
