Primary Purpose:
To undertake individual work with households who are at risk of tenancy
breakdown and homelessness due to their many complex and overlapping
needs, utilising the Care and Case Management Model. The Prevention
Case Management service is a project designed with regard to Dublin
Regional Homeless Executives Pathway to Home Model 2010, across the
Dublin Region. Specifically to assess their needs, to advocate on their
behalf, to provide support and make appropriate referrals on their behalf;
thus ensuring a planned and effective stabilization of the household within
the context of the household, tenancy and community. The Prevention
Case Management Service is a Partnership between Fingal County
Council, Dublin South County Council, Dun Laoghaire-Rathdown County
Council and Focus Ireland.
Key Responsibilities
1.
To work within the framework of the overall objectives of Focus Ireland.
2.
To work in accordance to the Service Level Agreement between Dun Laoghaire-Rathdown County
Council and Focus Ireland. To ensure adherence to Dublin South County Council Generic Housing and
Health and Safety Policies.
3.
To work within the agreed caseloads with Dun Laoghaire-Rathdown County Council.
4.
To ensure to engage customers effectively in keeping with the covenants and responsibilities set out in
Dun Laoghaire-Rathdown County Council Tenancy Agreements, Policies and Systems for rent
collection, maintenance and estate management.
5.
To contribute to the formulation of customer’s needs assessment and support plan utilizing the Dublin
Homeless Executive Holistic Needs Assessment, PASS system and keep up to date case work
records.
6.
To provide one to one engagement to ensure that the Customer reflects on their situation and is
supported in making informed decisions relevant and in compliance with the challenges and
responsibilities they face as a household and under tenancy to the Local Authority.

To contribute and participate in conjunction with the project leader in ensuring a package of support is

designed and implemented by the project worker ensuring and promoting the welfare of the household.
8.
To work as a part of a multi disciplinary / multi agency service
9.
To use a flexible approach in engaging households with their individual needs, taking into consideration
dynamics relevant to their needs and behaviours.
10.
To work with the Customer in identifying the significant needs/behaviours in their life, to address the
issues of difficulty that has contributed to the household being in breach of tenancy and at risk of
homelessness.
11.
In conjunction with the Project Leader develop and maintain links with the Garda, Probation, Health
Service Executive, Medical Services, Community based services and Local Authority services to ensure
effective communication, referrals, case work, Case reviews and discharge planning where appropriate
to ensure that as staff they work co-operatively and effectively in line with the processes of the service
agreed with the Local Authority.
12.
To contribute to a case conferencing model ensuring effective liaison with external professionals and
service and the Customer where appropriate.
13.
To ensure appropriate liaison with all stakeholders in the household support circle including as
appropriate Social Workers, Schools, G.P.’s and other clinical service practitioners.
14.
To accompany and provide support to Customers on appointments to other services e.g. doctors,
counsellors, social workers if required.
15.
To work within a rota for a five day service. Flexibility may be required from time to time including
evening cover.
16.
To attend and contribute to meetings and reviews as required and follow up on decisions made
between the Project Leader and Local Authority representatives.
17.
To implement the policies and procedures of the both the Project and Focus Ireland, inclusive of Health
and Safety policy now and in any amendments in the future. To include Professional Conduct, Child
Protection and Working Alone policies. Where based in a Local Authority Office to ensure to remain
complaint to the Health and Safety Policy and Fire evacuation procedures of the location.
18.
To attend team, sectional, divisional and any other organisation meetings as required
19.
To engage in supervision sessions on a regular basis with the Project Leader
20.
To participate in relevant training and development courses as agreed with your Project Leader
21.
To undertake such other duties as might be reasonably assigned from time to time in consultation with
the Project Leader.
22.
To be vigilant to any Health, Safety and Welfare risks in the workplace and bring any concerns to the
attention of your line manager or Health & Safety Representative

Criteria: Project Worker
Essential Criteria
 Proven, extensive experience of working with Customer at risk
 Third Level qualification at degree level in Social Care or Equivalent.
 Experience of working with customers who have co-existing conditions
 Experience of team working
 Report writing abilities
 Experience of working and liaising with other agencies
 Understanding of issues pertinent to anti-social behaviour and homelessness
 A full clean driving license and a car owner with appropriate insurance for using their car for work.
Person needs to demonstrate

- Knowledge and understanding of adolescents and issues affecting adolescents
