UCLH specialises in women’s health and the treatment of cancer, infection, neurological, gastrointestinal and oral disease. It has world class support services including critical care, imaging, nuclear medicine and pathology. The appointment An exciting opportunity has arisen for a full-time NHS appointment (10 PAs) at University College London Hospitals (UCLH) to work and to develop new pathways of care for patients living with cancer in the emergency, acute assessment, elective day care, and clinic settings. In addition to extensive clinical experience within the inpatient and outpatient settings, the postholder will be supported in leading service developments within this large tertiary cancer centre. The aim of this post is to help develop the transfer of care from the emergency department and inpatient setting to the day care environment within the new state-of-the-art UCH Macmillan Cancer Centre. In addition, the postholder will have the opportunity to review and audit the evolving service and be involved in research projects. There are currently 39 beds for oncology inpatients receiving care for a range of malignancies. Elective admissions are for delivery of chemotherapy, radiotherapy including radioisotope treatment, symptom care and management of complications of cancer treatment. The post holder will work closely with core medical trainees and oncology specialist registrars to ensure effective delivery of care from admission through to discharge and the day care/clinic environment as required. The post holders will fulfil a pivotal liaison role between other departments such as accident and emergency and the acute medical admissions and will be expected to liaise with consultant oncologists as needed. The post holders will be expected to take an active role in education, training and audit in relation to the ward and day care service of junior doctors, nurses, CNS’s and other allied health care professionals. This post would be suitable for a senior trainee who intends to pursue an academic or NHS oncology career. The postholder will be responsible to the Lead Clinicians for Chemotherapy and Acute Oncology at UCH. Eligibility for full GMC Registration is required. Possession of MRCP or equivalent is desirable. Specialist training in clinical or medical oncology with experience in delivery of chemotherapy is desirable. The clinical sessions will be based in the cancer centre day unit, clinics and the inpatient wards/emergency department, but the postholder may need to see some patients at other locations within the Trust. University College Hospital (UCH) is a new state-of-the-art hospital in central London (http://www.uclh.nhs.uk/), with access to all types of cancer therapy and imaging, including MRI-PET and functional imaging, and molecular pathological services. The newly built UCH Macmillan Cancer Centre provides a unique environment for the investigation and treatment of cancer patients. In-patient and radiotherapy facilities are located in the main UCH building close by. UCL is one of the U.K.’s premier research and teaching Universities (www.ucl.ac.uk) and has one of the largest Biomedical Faculties in Europe. UCL is both a Cancer Research UK Centre and an Experimental Cancer Medicine Centre providing key infrastructure for cancer research. Further Information & Visits to the Hospital General enquiries, about the job or for an informal discussion about this post should be directed to Dr Martin Forster ([email protected]) or Dr Kai-Keen Shiu ([email protected]) Main Duties & Responsibilities The two post holders will join the existing third post holder, working on a rotational basis to share and internally cross cover the following duties and responsibilities; Clinical Provide assessment, triage and treatment plans for unwell oncology patients providing integrated and cohesive outpatient and inpatient levels of care in conjunction with existing oncology trainees and consultants. Assess and instigate initial management of acute admissions to UCH to the Emergency Department or Cancer Centre (either patients with known malignancy or with a suspected cancer diagnosis, patients with carcinoma of uncertain primary) supporting the CMTs, Specialist Registrars, or referrers as part of the Acute Oncology (AOS) and CUP Service led by Dr Shiu. Carry out a daily ward round with the Acute Oncology Consultant of the week, Acute Oncology SHO and Acute Oncology CNS, for assessment and management of oncology patients admitted within 24 hours to the trust or referred to the AOS with a suspected cancer diagnosis. Provide medical care for patients undergoing treatment in the UCH Cancer Centre chemotherapy and day assessment unit. Undertake one out-patient tumour specific oncology clinic per week Attend the relevant tumour specific MDM each week. Work together with the SpRs in medical and clinical oncology to support the junior medical staff on the oncology wards for existing unwell patients. Maintain ALS skills Be aware of, and comply with, patient confidentiality and freedom of information act Be aware of ethical issues which arise in end-of-life care Any other duties which may be reasonably requested by the Clinical Lead for Chemotherapy or the Divisional Clinical Director Service Development Work with the Clinical Leads and Divisional Clinical Director and management to develop an oncology emergency assessment and treatment service on the Day Care Unit in the UCH Macmillan Cancer Centre Take responsibility for researching and analysing best practice Lead on the development of agreed conditions and procedures to be serviced in the triage and treatment service. Develop and implement protocols and pathways division wide Liaise closely with Emergency Services Division to develop a seamless service for oncology inpatients and outpatients. Work with junior medical staff to ensure doctors’ skills are enhanced by the triage and treatment service. Clinical Governance & Audit Attend and participate in educational and governance activities – including day unit inpatient MDTs and departmental meetings Undertake Clinical Audit Projects related to Cancer Services Maintain high standards of clinical note keeping Contribute towards developing, monitoring and reviewing the service including preparing reports as required Audit triage and treatment service Post holders will be fully supported in presenting practice changing audits to the Cancer Division, Trust and external meetings by their Clinical Supervisors Dr Forster and Dr Shiu. Coordinate the SHO rotas and leave. Research & Teaching Maintain GCP updates and skills. Develop and initiate research projects (with the guidance of other faculty) Participate in the late phase clinical trials programme taking responsibility as appropriate in the clinic Undertake continued professional development and attend national and international conferences to keep abreast of field Critically analyze clinical research literature relevant to position and have a basic knowledge on clinical databases and relevant statistical approaches Coordinate the SHO local departmental induction programme. Oversee the SHO weekly SpR-led teaching programme. The Surgery and Cancer Board provides services based in the UCH Tower, UCH at Westmoreland Street and the UCH Macmillan Cancer Centre, with 5 divisions, each with a Divisional Clinical Director and Divisional Manager: Cancer, Surgical specialties, Gastrointestinal services, Imaging and Theatres and Anesthesia. The division is responsible for: Developing and implementing the Cancer Strategy for UCLH, as well as implementing cancer standards and cancer waiting times across the Trust Clinical haematology Oncology Chemotherapy Radiotherapy UCH Macmillan Cancer Centre Working closely with the UCL Cancer Institute, the division is implementing a programme of change and investment to make UCL and UCLH one of the top cancer centres in Europe. The UCLH Board of Directors supported this strategy with agreement to invest £100million in a new cancer centre, which has been built directly opposite the Cancer Institute in Huntley Street. This strategic programme of work will ensure that clinical research, clinical outcomes, pathways and waiting times, partnerships, productivity and profitability and the patient environment and experience will all be developed to match the aspirations of the Trust and UCL for cancer services. UCLH has been designated as one of the two proton therapy sites to be built in the UK, this is due to open to patients in 2019. The development of effective clinical partnerships and cancer patient pathways with other Trusts within the new provider network, London Cancer (www.londoncancer.org), is a crucial component of this Cancer strategy. Clinical haematology (Lead clinician Dr Kirsty Thomson) has 71 in-patient beds, all single rooms with ensuite facilities on levels 13 14 and 16 of the new University College Hospital. Outpatient and daycare facilities are shared with the oncology directorate. There are active clinical and research programmes in leukaemia, lymphoma, myeloma, transplant, clotting and TTP, red cell and anti-coagulation. The directorate works closely with laboratory haematology services, which are managed as part of the clinical support division. There is a cellular therapy unit provides accredited facilities to support the department’s ambitious programme in stem cell harvests, gene therapy and immunotherapy. In December 2015 the malignant haematology service from the Royal Free Hospital was integrated with and transferred to UCH. This is now one of the largest in Europe Haematology services in Europe. Oncology (Lead clinician Dr Naomi Fersht) has 39 beds on levels 10, 13 and 14 of University College Hospital, shares outpatient and daycases with clinical haematology, and employs medical and clinical oncologists working in cancer MDTs across the whole of UCLH. The team is supported by 12 clinical oncology registrars, 7 medical oncology registrars and 3 oncology speciality doctors. The radiotherapy directorate (Lead Clinician Dr Yen Ching Chang) is located on level 2 of University College Hospital, undertaking 2000 new courses of treatment per year. Equipment includes 5 Varian linear accelerators, all with multileaf collimators and portal vision and some with respiratory gating and onboard imaging. There is a high dose rate microselectron unit, and planning equipment (CT simulator as well as access to CT PET equipped to match the facilities on the linear accelerators.) The department offers a variety of specialised treatment, including CT-based TBI, ultrasound-guided and CT-planned HDR brachytherapy, high precision conformal radiotherapy utilising MR/PET/CT fusion, IMRT and stereotactic radiotherapy.
