ANTIBIOTIC PROPHYLAXIS
 cefazolin (Ancef)
Dose and frequency of therapy
 1 gram intravenously every 8 hours times 2 doses, then discontinue
 2 grams intravenously every 8 hours times 2 doses, then discontinue
Alternative antibiotic prophylaxis (use if Cefazolin (Ancef) is contraindicated)
 vancomycin 1 gram intravenously every 12 hours times 2 doses, then discontinue

SEDATION
 dexmedetomidine (Precedex) 1 mcg/kg bolus over 10 minutes, the continuous infusion 0.2 – 1.5 mcg/kg/hr AND fentanyl infusion at 1 - 5
mcg/kg/hr to maintain RASS of ________ or BIS ________ if using BIS monitor.
 propofol (Diprivan) 10 mg/mL IV at 20 mcg/kg/min to maintain RASS of ________ or BIS ________ if using BIS monitor.
