AFFIDAVIT
I do hereby swear or affirm that the following statements are true and correct:
1.
I am seeking to serve as an unpaid volunteer for the Second Presbyterian Church of
Carlisle without any compensation or remuneration whatsoever.
2.
I have been a resident of Pennsylvania for the entirety of the past 10 years.
3.
I have not violated any law or committed any act in the Commonwealth of Pennsylvania
or any other jurisdiction that would be subject to criminal prosecution or investigation by
a child protective services agency or that would cause disqualification from my serving
as a volunteer responsible for children.
4.
I agree that if any of the above statements should no longer be true or correct, I will
promptly notify Second Presbyterian Church in writing of the change.
5.
I understand that Second Presbyterian Churach has agreed to waive the requirement for
the submission of an FBI Federal Criminal History Record in reliance on these
statements.
