Destroying Schedule 4 Controlled Drugs (CDs)
It is now a legal requirement that the following Schedule 4 CDs must be
denatured in a DOOP kit before being destroyed. As with other CDs they need
to be destroyed by a pharmacist and nurse.

This destruction must be recorded in the book specially held on the ward/team
base to record the destruction or return of ‘medicines requiring additional
security’.
There is however an exception - any dose of a controlled drug that is prepared
but not administered, including odd doses spat out by the patient must be
destroyed on the ward or department immediately. Though CDs are usually
destroyed in a DOOP kit, these single doses can be disposed of in the general
medicines waste bin. Solid dosage forms should be wetted in a small amount
of soapy water before tipping everything into the general medicines waste bin.
This destruction of the controlled drug must be in the presence of a second
designated practitioner or if not available another member of staff familiar
with CDS, e.g. an authorized employee trained to witness the administration of
CDs. The appropriate entry must be made either in the Controlled Drug
Register if a full CD requiring storage in the CD cupboard or if a schedule 4 CD
in the book especially held on the ward/team base. The entry must include the
signatures of the two individuals involved in the destruction.
