Strabismus as related to the GMFCS level. Primary esotropia: convergent heterotropia or cover test before any
treatment; vertical strabismus/DVD: hypertropia with corresponding hypotropia and/or dissociated vertical deviations;
primary exotropia: divergent heterotropia before any treatment; dyskinetic strabismus: heterotropia manifested as both
esotropia and exotropia; sensory strabismus: unilateral heterotropia of eye with substantially lower acuity due to afferent
pathway anatomic lesion or anisometropic amblyopia. See the Appendix for details.
