Kindergarten Registration 2012 - 2013 Term ______ _______ _
Kindergarten registration in the Hazleton Area School District, for the 2012-2013 school
year will be held at the Hazleton Area Career Center from 3:00 P.M. to 7:00 P.M. on the
following days:
March 5, 2012 – Monday *(Snow Date, March 13 – Tuesday)

West Hazleton Elementary/Middle School
March 6, 2012 –Tuesday *(Snow Date, March 14 – Wednesday)


Freeland Elementary/Middle School
Drums Elementary/Middle School
March 7, 2012 – Wednesday *(Snow Date March 15 – Thursday)

Heights-Terrace Elementary/Middle School
March 8, 2012 – Thursday *(Snow Date March 19 – Monday)


Arthur Street Elementary School

Application packets, which include all required and requested forms, are available at all Hazleton
Area School District elementary/middle schools or at the district’s website (www.hasdk12.org)
Children to be registered for kindergarten must be five (5) years of age on or
before September 1, 2012.
At the time of registration, the following documents must be presented:

Documentation of the child’s age. Said documentation may be in the form of a Birth
Certificate, Baptismal Certificate, notarized or duly certified copy of the Record of Baptism
showing the date of birth, notarized statement from the parents or another relative indicating the
date of birth, a valid passport, or a prior school record indicating the date of birth.

Proof of immunization. This proof can be in the form of the child’s immunization record,
written statement from the former school district, or from a medical office that required
immunizations have been administered, or that a required series is in progress, or verbal
assurances from the former school district or a medical office that the required immunizations
have been completed, with records to follow. No child five years old after September 1, 2011
can be enrolled in kindergarten.

Three (3) proofs of residency. Examples of proof of current residency are: current PA
Driver’s License, Internal Revenue Statement, W2 Form, Voter Registration Card, Property
Deed, Property Tax Bill, State ID Card, Insurance Statement, Vehicle Registration, Current
Pay Stub, Bank Statement, Billing Statement, Multiple Occupancy Form (if applicable) or a
Utility Statement.

Parent Registration Statement (Act 26). A sworn statement attesting to whether the
student has been or is currently suspended or expelled for offenses involving drugs, alcohol,
weapons, infliction of injury or violence on school property.

Home Language Survey. All students seeking first time enrollment in a school shall be
given a Home Language Survey in accordance with requirements of the U.S. Department of
Education’s Office for Civil Rights. A copy of the Home Language Survey is provided in the
registration packet or at (www.hasdk12.org)
Dr. Francis X. Antonelli, Acting Superintendent of Schools, urgently request that all parents
who plan to enroll children in the Hazleton Area School District kindergarten class for the
2012-2013 school year do so on the designated days for registration.
Parents of registered kindergarten children will be notified concerning date and time of
kindergarten orientation.
HASD FIRST GRADE REGISTRATION - Children not now enrolled in kindergarten, but who
will enter first grade in the district in September, 2012 and will be six (6) years old on or before
September 1, 2012 should be registered at the Hazleton Area Career Center on the days designated for
kindergarten registration for the school within whose boundaries the student resides.

IMMUNIZATION LAW: Parents are reminded that the state law requires that children be fully
immunized before admission to school for the first time. Medical and religious objections, in writing,
will be the only objection accepted. Immunizations required by law are: MMR (Measles, Mumps,
Rubella) Two (2) doses are required: First dose must have been administered on or after 12 months of
age. TD (Diphtheria Tetanus) -Three (3) or more properly spaced doses of diphtheria toxoid. These
immunizations may have been accomplished by administering tetanus toxoid; diphtheria and tetanus
toxoids; or diphtheria and tetanus toxoids and pertussis (whooping cough) vaccine. A fourth dose of
Tetanus and Diphtheria (TD) must be administered after the fourth birthday. POLIOMYELITIS Three (3) or more properly spaced doses of trivalent oral polio vaccine. HEPATITIS B - Three (3) doses
of hepatitis B vaccine will be required for students entering school for the first time (i.e., kindergarten or
first grade) and students entering seventh grade. VARICELLA (Chicken Pox) - Two (2) doses of the
vaccine or history of disease. A TB test must be completed for any student entering the Hazleton Area
School District for the first time.
Written verification of the immunizations by your family physician must be presented to the
school nurse upon registration for kindergarten or first grade and, thereafter, as each dose is
administered.
