Sample Employee Agreement
I understand that the Ottawa County Smoke-Free Indoor Air Regulation prohibits
smoking in public and private workplaces and public buildings within the county. The
regulation also requires that outdoor smoking be further than 25 feet or to property
edge, whichever is closer, from all entrances, ventilation intakes and operable windows.
I have reviewed our company’s policy in compliance with this regulation and understand
that failure to adhere to this policy could result in disciplinary action, consistent with
company policy, and/or the filing of a complaint with the Ottawa County Health
Department.
Smoking is strictly prohibited in all enclosed areas within this worksite
without exception. This includes common work areas, auditoriums,
classrooms, conference and meeting rooms, private offices, elevators,
hallways, medical facilities, cafeterias, employee lounges, stairs,
restrooms, employer owned or leased business vehicles occupied by more
than one employee or the public, all other enclosed facilities, and outdoors
within 25 feet of all entrances, ventilation systems and operable windows.
This policy applies to all employees, clients, contractors, and visitors.
I agree to the following rules and company policy about smoking in enclosed work
spaces and within 25 feet of entrances, windows and ventilation systems:
 I will not smoke in any indoor area, company vehicle or within 25 feet of
entrances, windows and ventilation intakes.
 If I observe any person in violation of this policy, I will bring it to the attention
of the offending party and seek voluntary cooperation.
 If the offender is noncompliant, I will notify a supervisor, manager or owner of
the violation.
 I may contact the Ottawa County Smoke-Free Regulation Coordinator at
616.393.5793 if all internal steps fail to result in compliance.
In turn, management agrees to support my efforts to insure compliance with this
regulation and protection from the known hazards of secondhand smoke.
