Activities To achieve the objectives of this rotation, you should complete the following activities, and other additional activities suggested by your preceptor: Activities—Drug Distribution Tour the pharmacy department and all other areas in the hospital where pharmacists are located Tour the main areas of the hospital, noting where any drug distribution system areas are located If possible, tour other regional hospitals to observe different drug distribution systems Complete the general staff orientation activities Observe the main steps in the drug distribution system, from the time a medication order is written, to the time when the medication is administered to the patient and recorded by the nurse Review the references available in the pharmacy, and note how they are used to solve drug distribution problems Review the policies and procedures for each of the activities that you observe and/or participate in during the rotation Observe and assist technicians in their various assignments within the drug distribution system, including all record keeping: Purchasing/inventory control Dispensary Narcotics Prepackaging/bulk manufacturing Unit dose manufacture Wardstock/night cupboard Nonsterile compounding Returns/drug recalls Crash cart medications Regional services (Shipping/Receiving, prepacks) Tech-check-tech program Observe and assist pharmacists and/or technicians in all dispensary functions, including record-keeping where required: (Minimum # of tasks to be performed independently) Computer order entry and profile review (40 orders, 10 patient profiles) Verification of orders/clarification problem prescriptions (5 cases) Stat medications and individual prescriptions (5 cases) Nonformulary drugs/unavailable drugs (3 cases) Automatic substitutions (3 cases) Restricted drugs Investigational/emergency release drugs Pass medications Patient’s own medications Observe and assist pharmacists in their supervisory role in various areas of the department Activities—Intravenous Admixtures Observe the administration of all types of parenterals and discuss the advantages/limitations of each with nurses and pharmacists Discuss and compare specific parenteral therapy concerns and needs for different types of patients (eg. neonatal, pediatric, elderly, critically ill) Observe the preparation of all types of IV admixtures Large volume parenterals Small volume parenterals TPN CADD cassettes, Its, outpatient IV Syringes Chemotherapy Participate in a training program in aseptic technique Review principles and proper use of equipment used in the preparation of sterile products Prepare IV admixtures using appropriate sterile technique, including order entry, calculations, concentration calculation, pump programming, etc. Assist with supervision of IV admixture preparation Review policies and procedures for preparation and administration of admixtures, chemotherapy and TPN Review the hazards, precautions, and guidelines for preparation, administration, and disposal of antineoplastics Complete all records and workload measurement reports required by the IV admixture service Review departmental resources that provide information on parenteral drug stabilities, compatibilities, administration, policies and procedures, and use these resources to solve drug-related problems with parenteral therapy
