JOB PURPOSE The post holder has professional responsibility, professional and operational accountability to assist in the development and delivery of a specialist Arthroplasty out patient review service for Golden Jubilee National Hospital (GJNH), having responsibility for clinical governance issues relating to the Arthroplasty Practitioner service. Ensuring robust and effective communication with the consultants involved with the service to ensure optimum care for the patient. The individual provides direct clinical care encompassing assessment, investigation, diagnosis, advice and management of Arthroplasty patients in outpatient clinics to enhance patient care. The Arthroplasty Outcomes Practitioner’s role is to provide long-term review of patients with Arthroplasty (joint replacement). Education of Arthroplasty patients on the aims and objectives of the Arthroplasty Outcomes Service and the benefits to patients. 3. ORGANISATIONAL POSITION See Attached Diagram 4. SCOPE AND RANGE The post holder will work autonomously with work being managed and not supervised. The postholder will be responsible to the Senior Charge Nurse for Outpatients. Involved in the ongoing assessment and management of a specific cohort of patients namely those who have joint replacement surgery. Required to supervise qualified and unqualified staff as required. Autonomous Practitioner running up to 10 out patient clinic sessions per week Assessment initially by telephone follow-up 10-14 days after surgery, thereafter clinical assessment at 6week/3 months, 1 year, 2 years, 5 years then dictated by the needs of the patients and demands of the service. MAIN DUTIES/RESPONSIBILITIES Clinical Ability to organise Arthroplasty service workload with regards to assessing patient needs, planning, implementation and evaluation of a programme of care which is evidence based utilising all available resources taking into consideration the lifestyle, gender and cultural background and ensure involvement with the patient, family, carers and significant others. Act as a specialist practitioner resource in Arthroplasty outcomes, helping colleagues by sharing expert knowledge and skills. Increasing awareness of the importance of long term follow-up of Arthroplasty patients and the necessity for early identification of failure/complications in hospital and primary care setting to enhance standards of care. Provide highly specialised extended clinical skills beyond the scope of routine nursing practice working across professional boundaries to maximise patient care and education. Ensure the quality of patient care is continually reviewed and assessed, with any necessary changes implemented and monitored to maintain and improve standards of care given to patients and their families. Delivery of expert service to maximise patient potential following Arthroplasty surgery and to facilitate the optimal treatment of early/late complications/problems encountered. Provide accurate and timely correspondence to General Practitioner and other relevant health professionals regarding the outcome of consultations and any necessary follow-up required. Manage and co-ordinate the long term Arthroplasty patient review, which includes: Assessing and examining patients (lower limb joint replacement and foot surgery) Ordering (through arranged local protocols) radiographs and interpretation of same. Initiating investigations including blood tests and referral for invasive X-ray procedures Interpreting clinical findings and investigation results – X-rays, blood tests, in order that the consultant / medical staff are fully appraised of findings to allow relevant intervention if required. Implementing the appropriate patient management plan to include: Providing expert support, counselling advice and education Identify appropriate patients for revision Arthroplasty, surgery of failing joints or for other lower limb primary surgery following confirmation by the consultant and relaying this information back to the patients General Practitioner or referring Health Board. Referring for further investigations to internal / external services e.g. an A&E department or General Practitioner for investigation of suspected DVT or white cell scans. Recommendation for admission of patient to consultant to elective Orthopaedic Unit e.g. suspected wound infection requiring urgent surgical treatment. This occurs occasionally Recommendation for admission following discussion with consultant to admit patient to elective Orthopaedic Unit for manipulation of joint. Planning further management of a patient i.e. timing of next clinical review / investigations/ referral. Obtaining appropriate patient consent for Bone Banking according to hospital and blood transfusion services guidelines. Ensure good communication and documentation in patient records: dictate clinical notes / letters to General Practitioner / referral letters. Develop policies, procedures and competencies for Arthroplasty service. Provides specialist Arthroplasty advice through a dedicated help-line, which facilitates early review of patients with complications/problems. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB It is likely that 20 per cent of the job function will be spent on the statistical aspect of the role as outlined above, with the remaining eighty per cent of the job function spent on the clinical, leadership, managerial aspects whilst providing a supportive clinical and educational role. However the demands of the service can be unpredictable and this percentage of time allocation will change from time to time.
