1. Terms and Conditions of Service
The terms and conditions applicable to this post are those of all NHS Employees.
2. Superannuation
You have the option to join the NHS Superannuation Scheme, to participate in the State
Earnings Related Pension Scheme or to take out a Personal Pension.
Employee’s contributions to the NHS Scheme amount to 5.2% to 14.7% of salary
(depending on rate of Pensionable Pay) and the employers’ contribution equates to
13.5% of salary. Employees in the NHS Scheme are “Contracted-out” of the State
Earnings Related Pension Scheme and pay a lower rate of National Insurance
contributions. Employees who choose to participate in the State Earnings Related
Pension Scheme pay the higher rate of National Insurance contribution. A Stakeholder

2. JOB PURPOSE
The post holder has professional responsibility, professional and operational accountability to
assist in the development and delivery of a specialist Arthroplasty out patient review service for
Golden Jubilee National Hospital (GJNH), having responsibility for clinical governance issues
relating to the Arthroplasty Practitioner service.
Ensuring robust and effective communication with the consultants involved with the service to
ensure optimum care for the patient.
The individual provides direct clinical care encompassing assessment, investigation, diagnosis,
advice and management of Arthroplasty patients in outpatient clinics to enhance patient care.
The Arthroplasty Outcomes Practitioner’s role is to provide long-term review of patients with
Arthroplasty (joint replacement).
Education of Arthroplasty patients on the aims and objectives of the Arthroplasty Outcomes
Service and the benefits to patients.
3. ORGANISATIONAL POSITION

4. SCOPE AND RANGE
The post holder will work autonomously with work being managed and not supervised. The
postholder will be responsible to the Senior Charge Nurse for Outpatients.
Involved in the ongoing assessment and management of a specific cohort of patients namely
those who have joint replacement surgery.
Required to supervise qualified and unqualified staff as required.
Autonomous Practitioner running up to 10 out patient clinic sessions per week
Assessment initially by telephone follow-up 10-14 days after surgery, thereafter clinical
assessment at 6week/3 months, 1 year, 2 years, 5 years then dictated by the needs of the
patients and demands of the service.
Number of Out Patient appointment / sessions 3000+ appointment per year
Offer patient accessibility to the Arthroplasty outcomes service at all times via a telephone
advisory helpline providing specialised advice concerning care, treatment and support to both
patients and relatives, also fast track patients with potentially acute problems.
Resource consciousness and awareness of the department budget – no direct budgetary
control.
Provides accurate information for audit and clinical effectiveness contributing to research.
Responsible for ensuring the delivery of evidence based care and contributing to the
development of the evidence based practice.
Provide clinical leadership/expert advice to nursing staff and other members of the
multidisciplinary team.
MAIN DUTIES/RESPONSIBILITIES
Clinical
Ability to organise Arthroplasty service workload with regards to assessing patient needs,
planning, implementation and evaluation of a programme of care which is evidence based
utilising all available resources taking into consideration the lifestyle, gender and cultural
background and ensure involvement with the patient, family, carers and significant others.
Act as a specialist practitioner resource in Arthroplasty outcomes, helping colleagues by
sharing expert knowledge and skills.
Increasing awareness of the importance of long term follow-up of Arthroplasty patients and the
necessity for early identification of failure/complications in hospital and primary care setting to
enhance standards of care.
Provide highly specialised extended clinical skills beyond the scope of routine nursing practice
working across professional boundaries to maximise patient care and education.
Ensure the quality of patient care is continually reviewed and assessed, with any necessary
changes implemented and monitored to maintain and improve standards of care given to
patients and their families.
Delivery of expert service to maximise patient potential following Arthroplasty surgery and to
facilitate the optimal treatment of early/late complications/problems encountered.
Provide accurate and timely correspondence to General Practitioner and other relevant health
professionals regarding the outcome of consultations and any necessary follow-up required.
Manage and co-ordinate the long term Arthroplasty patient review, which includes:
Assessing and examining patients (lower limb joint replacement and foot surgery)
Ordering (through arranged local protocols) radiographs and interpretation of same.
Initiating investigations including blood tests and referral for invasive X-ray procedures
Interpreting clinical findings and investigation results – X-rays, blood tests, in order that the
consultant / medical staff are fully appraised of findings to allow relevant intervention if required.
Implementing the appropriate patient management plan to include:
Providing expert support, counselling advice and education
Identify appropriate patients for revision Arthroplasty, surgery of failing joints or for other lower
limb primary surgery following confirmation by the consultant and relaying this information back
to the patients General Practitioner or referring Health Board.
Referring for further investigations to internal / external services e.g. an A&E department or
General Practitioner for investigation of suspected DVT or white cell scans.
Recommendation for admission of patient to consultant to elective Orthopaedic Unit e.g.
suspected wound infection requiring urgent surgical treatment. This occurs occasionally
Recommendation for admission following discussion with consultant to admit patient to elective
Orthopaedic Unit for manipulation of joint.
Planning further management of a patient i.e. timing of next clinical review / investigations/
referral.
Obtaining appropriate patient consent for Bone Banking according to hospital and blood
transfusion services guidelines.
Ensure good communication and documentation in patient records: dictate clinical notes /
letters to General Practitioner / referral letters.
Develop policies, procedures and competencies for Arthroplasty service.
Provides specialist Arthroplasty advice through a dedicated help-line, which facilitates early
review of patients with complications/problems. E.g. fast tracking of patients with suspected
wound infection / failing joint replacement
Participate in the monitoring of standards of care within the defined policies, procedures and
protocols of the service, ward / department to ensure adherence to, and delivery of a high
quality service
Contribute to the development of highly specialist programmes for care packages for patient
within the Arthroplasty service
Liase with other healthcare professionals regarding patient treatment and progress
Organises / participates in organisational wide committee meetings as designated by Clinical
Nurse Manager and Senior Charge Nurse
Organises / participate in external committee meetings to promote GJNH Arthroplasty Outcome
Service
Leadership and Management
Responsible for managing the daily duties of the department, including work allocation,
deployment and supervision of staff, to ensure smooth running of the area
Lead members of the team with forward planning, priority setting and decision-making. This
includes establishing clear, concise achievable and measurable objectives to meet the patient’s
needs
Maintain and process accurate / up-to-date electronic or Integrated Care Pathway (ICP) clinical
records in accordance with both legal and departmental requirements
Ability to instigate specific initiatives within the multi-professional team (MPT), in the
development, implementation and maintenance of policies, procedures, standards and
protocols of the Arthroplasty service

they arise or any changes to be considered for implementation to improve the overall service
Maintain good working relationship with Arthroplasty Practitioners in Health Boards nationally
and keep up to date on developments in clinical practice through regular course attendance

To continually promote the professional role of the Arthroplasty outcomes practitioner to
maximise the effectiveness of the multi-disciplinary team in patient treatment
To contribute to maintaining the GJNH as a centre of excellence in Orthopaedic surgery and
promote the activities of the Arthroplasty Outcomes and Out Patient Services
Organise and actively participate in the In-Service Training for the Department
Prepare and deliver formal and informal teaching sessions and support others in providing
education for patients, relatives and colleagues
Organise courses to be held at GJNH and regularly attend courses to further personal and
professional development
Ensure that on-going personal development needs and professional education and research
are identified and met
Audit and Research
Participate in clinical audit of specialist nursing services for the Arthroplasty outcomes service
to ensure evidence based practice
Contribute to / or take an active role in key clinical research projects for the Arthroplasty
specialty service
Support and apply clinical research to extend professional practice, looking at new clinical
benchmarks, outcome measures or clinical indicators
Produce independent data and ensure results are highlighted and acted upon by relevant
Orthopaedic Consultants
Development and review of Arthroplasty patient-derived outcomes audit as part of
demographic data collection for the Scottish Arthroplasty Project and the Information and
Statistic Division. Surveillance of infection and complication rates also.
Responsible for evaluating outcomes by standard setting and audit to achieve maximum
recovery and expeditious discharge in a cost effective manner, implementing changes as
necessary
SYSTEMS AND EQUIPMENT
Computer skills for recording patient notes i.e. Excelicare clinical system
Microsoft Office package for the creation of policies and competencies, creating spreadsheets
for calculating statistics; patient / doctor / MPT d/c or referral letters; PowerPoint for

7. DECISIONS AND JUDGEMENTS
Post holder organises own caseload and is responsible for clinical management of own
workload
Arthroplasty outcomes practitioners work autonomously with clinics running independently of
the consultant clinics
The Arthroplasty outcomes practitioner is responsible for planning all review appointments,
based on clinical judgement and guidelines
Has the ability to anticipate, by using expert knowledge, changes in patient’s conditions and
implement the appropriate actions required
Problem solving in the department environment – patient related problems or staff issues
Management of current caseload volume and capacity planning of clinic templates.
In conjunction with the Senior Charge Nurse and Lead Orthopaedic Consultant assists and
recommends plans for the development of the service, taking account the increasing
demographic and geographical demand within Arthroplasty
Collate statistical information for ongoing audit purposes, which will impact on service
development and quality.
Professional judgement regarding a patient’s treatment – frequency, continuation or withdrawal
of treatment, applicable to the specialty area. These decisions and judgements being made
daily, discussions involving patient / carers and the MPT
Challenges decisions made by others if they are against policy or not in the best interest of the
patient
Responds to complex / demanding patient situations, both clinically and administratively

8. COMMUNICATIONS AND RELATIONSHIPS
Communicating with
Patients – who are in pain, afraid and reluctant requiring education, reassurance, motivation
and encouragement to comply with treatment
Carers / Visitors – education, reassurance and instruct carers / visitors in the techniques to
motivate patients to comply with treatment.
Orthopaedic Consultants – ensuring regular communication with the consultants with regards
to patient requirement / investigations beyond the limitations of the practitioners scope,
experience, knowledge and skills
Orthopaedic and other Consultants – it is essential to maintain good communication and
ensure a close working relationship due to the nature of the service provided
Occupational Therapy colleagues – regarding patients needs in the community and advice to
both nursing staff and patients on same
Liaison – With Acute Pain Service within the hospital for management of patient pain prior to
discharge and other associated services
Referring Trust and local healthcare colleagues – advising them of treatments and
regarding the level of Arthroplasty input and service
Medical Secretaries / Administrative staff / Booking Office / Unit Co-ordinators – booking
patients and administration
Multidisciplinary staff – education and training of treatment for Arthroplasty patients
MPT / Physiotherapists – internally and externally regarding patient care, at times submitting
or receiving sensitive and confidential information
Interacting with other department in the hospital – Porters, administration, infection control,
radiology and laboratories amongst others
9. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB
It is likely that 20 per cent of the job function will be spent on the statistical aspect of the role as
outlined above, with the remaining eighty per cent of the job function spent on the clinical,
leadership, managerial aspects whilst providing a supportive clinical and educational role.
However the demands of the service can be unpredictable and this percentage of time
allocation will change from time to time.
Handling some difficult patients – occasional significant physical effort / demand; working in
confined space in patients’ or treatment rooms; manoeuvring of patient handling equipment
Prioritisation of advice line calls – numerous times daily
Clear understanding of the effects of surgical intervention on the normal anatomy and
physiology of the patient
Keyboard skills and interpretation of data required to be applied daily
Frequent and long duration of standing, sitting, kneeling and walking
Dealing with frequent interruptions during periods requiring intense concentration
Venepuncture – several times a week
The post holder is exposed to unpleasant working conditions during clinical duties. This may

10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB
Working within a developing infrastructure in which processes are continually evolving.
Deciding and knowing whether or when to seek advice / consultation with the Consultant in
charge of the patient
Applying clinical reasoning to diagnose complication / problem and develop management plan
e.g. potential joint replacement infection: arrange appropriate investigation include: blood test,
X-rays. Interpretation of the results and plan for further review / add to waiting list for revision
surgery as appropriate following discussion with the consultant
Large volume of calls to advice line – requires prioritisation of calls – patients reporting pain /
difficulty weight bearing / falls / wound problems addressed first
Time management skills to balance the administrative and clinical duties
Dealing with lack of communication between the medical disciplines with regards patient care
To be able to communicate complex and sensitive information in an understandable form to
patients and carers, which may include giving unwelcome or difficult to accept information
regarding diagnosis, prognosis and treatment. Also to ensure understanding of relevant
precautions and suitable activities after joint replacement and the potential consequences of
inappropriate activities, whilst recognising patient autonomy
Motivating and encouraging patients who are unwilling to co-operate with treatment
Defusing situations that can arise where patients, relatives or carers can be verbally or
potentially physically abusive.
