Associate Rehire Policy The following summary describes benefit eligibility guidelines for rehired Associates. Please refer to the Benefits Summary and appropriate plan documents for detailed information. Medical/Vision, Dental, Group Life Insurance, and the Section 529 Plans Re-hire Policy 1. If an Associate is rehired within three months from the termination date, eligibility will be the first of the month following the rehire. If the Associate has enrolled in COBRA following the termination, the individual may cancel COBRA at the end of the month in which he/she is re-employed by Resources, and becomes eligible to enroll as an active employee the first of the month following the date of rehire. 2. If an Associate is rehired after 3 months from termination date, eligibility will be the first of the month following 60 days from the rehire date. Personal Time Off Plan (PTO) If an Associate returns to the Company within any amount of time, all previously accumulated billable hours will be considered for PTO eligibility and accrual. For example, an Associate who had previously accumulated 350 billable hours, the 500 hour PTO eligibility clock does not start over. Once an Associate works 150 more hours, after being rehired, the PTO accrual will begin. After an Associate works 500 life to date hours, the Associate will begin to accrue PTO at the rate of .04 hours for every hour worked. Once an Associate works 7,000 hours, the Associate will begin to accrue PTO at the rate of .06 hours for each hour worked. Holiday Pay If an Associate returns to the Company within any amount of time, all previously accumulated billable hours will be considered for Holiday Pay eligibility. For example, an Associate who had previously accumulated 350 billable hours, the 500 hours eligibility will not start over again. 401(k) If the Associate was eligible to participate in the 401(k) plan at the termination of employment, then the Associate is eligible to enroll in the plan on the first quarterly enrollment date following date of rehire. (Quarterly Plan Entry Dates: January, April, July and October). Please note, if you do not join the Plan when first eligible, you may not apply for enrollment until the next quarterly enrollment period. Employee Stock Purchase Plan Employee Stock Purchase Plan eligibility is the same for newly hired and rehired employees. Associates are eligible to participate in the Employee Stock Purchase Plan if they have 90 days of continuous employment (working minimum ten hours per week) prior to the first day of the offering period. Professional Development Program (PDP) PDP eligibility is the same for newly hired and rehired Associates. All active Resources’ employees are allotted 64 training hours per calendar year. Associates who are terminated and rehired within the same calendar year will not receive an additional 64-hours upon rehire.
