Cost: $15.00 – Please pay by cash or check. Cash should be submitted in an envelope with the
completed form directly to the UTB Music Office. Checks may be mailed to the UTB Department
of Music with ATTN: Dr. Joe Moore. Checks should be made payable to the “UTB Percussion
Club.” Please submit the completed form and payment together.
