Slamet Sumarno. Physical principles of humidification • Evaporation (vaporization) 1. Vaporization adalah pelepasanan molecules dari liquid masuk dalam atmosphere dengan kinetic energy. 2. Vapor is the gaseous state of a substance the exists as a liquid at room temperature. 3. Tingkat penguapan dipengaruhi oleh: a. Temperatur cairan. b. Agitasi peningkatan cairan, aktivitas malekul cairan c. Penguapan yg menurun • 4. Panas untuk penguapan , perubahan cairan ke gas membutuhkan energi dlm bentuk panas • 5. Effisiensi penguapan, berapa panas yg hilang untuk penguapan. Humidify 1. Penguapan molekul kedlm atmosphere, tekanan partial tergantung suhu gas dan konsentrasi dari molekul uap. 2. Jumlah penguapan air dlm % dihiitung dari volume gas sehingga ada kejenuhan penguapan (absolude humidify)terlihat dari ml gr uap/liter. 3. Perbandingan % Uap air dengan % maks temperatur (relative humidify. absulute vapor content (actual) X 100 Potensial Vapor content (maximum) Terapi inhalasi. • Pengertian • Menghirup secara baik: udara, uap air, obat, bau-bauan dll. • Jenis: humidifikasi menghirup uap air. Konvensional : uap air di pinggir laut pada sore hari, pagi hari di bawah pohon . Spasme bronkus. 3. Hyperaktif bronkus. 4. Infeksi jalan nafas dan alveoli. 5. Melembabkan jalan nafas yg kering 6. Merangsang delatasi dan estruksi paru yang kolap ( emfisema). Tujuan inhalasi. 1. Menghilangkan problem (symtom) 2. Memperabiki hygiene nafas (jalan nafas, bronkus, paru serta fungsi paru). 3. Mencegah fungsi paru lebih jelek. 4. Melembabkan udara (oksigen). 5. Sarana praemedikasi chest ft. Aerosol. Obat-obatan mouth 1. Inhalasi dengan obat. 2. Jenisnya: a. Matered dose inheler. alat aerosal paling simpel seperti seproten minyak wangi yang dikemas untuk botol 20 cc biasanya dipakai untuk anti histamin dan spame bronkus. Jet Nebulizer. • Bila dengan kanul uap yang masuk pasien sekitar 1/3 atau 20-30 %. • Bila memakai masker 30-40% • Bila masker automat 80 %. b. ultrasonic nebulezer. Ultrasonic nebulizer • Dihasilkan dari piezo Quard yang memfibrasi (menggetarkan) obat atau H20 dari arus listrik frekuensi tinggi 1,3 -1,4 Mz. • H20 dipecah menjadi partikel-partikel dengan diameter mul 0,5-5 mikron mencapai 97%. Alat ini lebih baik untuk saluran nafas bagian bawah atau lebih dalam sampai bronkhus terkhir. • Alat ini dapat dikombinasi dengan oksigen. Obat-obat yg biasa digunakan dalam terapi jalan nafas. (humidifikasi) A 1. Aquades steril. Frekuensi : tiap 4 jam atau 4 x/hari Dosis : 3-5 ml Kerja : membasahi dan mengencerkan dahak. Efek samping: Bronkhospasme karena iritasi/rangsangan. 2. Na Cl 0,9 %./ Na Cl hipotonik/ hypertonik Frekuensi : tiap 4 jam atau 4x/hari. Dosis normal: 3-5 ml. Kerja : Membasahi dan mengencerkan dahak. Efek samping: Bromkospasme karena iritasi/rangsangan. B. Bronkodilatator. 1. Aerolone ( Cyclopentamine, isoproterenol). Frekuensi : tiap 4 jam/ 4 kali /hari. Dosis normal : 0,3 dalam 3 ml N/S. Kerja : bronkodilatator dan dekongestan relaksasi pada otot polos bronkus dan mengurangi udema mukose. Efek samping : Kadang-kadang isomnia, takhikardi, palpitasi gugup dan vertgo. 2. Vanonefrin (Racemic eppineprine) Frekuensi : tiap 4 jam atau 4x/hari. Dosisi normal : 0,3ml (1:100) dalam 3ml N/S. Kerja : Bronkhodilatator kuat dan vasokontriktor efek vasopresor. Efek samping : Iritasi brokhus, gugup, takhikardi, hipertensi kurang tidur. • Penggunaan epineprine yg berulang-ulang sering menuju keterikatan sehingga terjadi penurunan yg progresif thd respon obat shg butuh dosis obat meningkat. C. Dekongestan (vasokontriktor) 1.Neo-Synephrine (phenylephrine). • Frekuensi : Tiap 4 jam atau 4x/hari. • Dosis normal: 0,3 ml(0,25%)dalam3ml N/S • Kerja : Relaksasi otot bronkhus. • Efeksamping: takhikardi, palpitasi, mual, gugup, muntah, kadang-kadang berkeringat, tremor, dan kulit kemerahmerahan. D. Bronkhodilatator. 1. Issuprel (issoproterenol) Frekuensi : tiap 4 jam / 4 kali sehari. Dosisi normal : 0,3 ml (1: 200) dalam 3 ml N/S Kerja : Relaksasi otot bronkhus. Efek samping : takhikardi, palpitasi, mual, gugup, kadang-kadang muntah, keringat dinginatau tremor dan kulit merahmerah. 2. Bronkhosol (Isoethrine) • Frekuensi : tiap 4 jam/4x/hari. • Dosis : 0,3 ml dalam 3ml N/S • Kerja : Relaksasi otot polos bronkhus. Efek samping: takhikardi, palpitasi, mual, sakit kepala, insomnia, pusing. 3. Brethine, Bricanyl (Trebutaline) • Frekuensi • Dosis • Kerja : Tiap 4 jam atau 4x/hari. : 0,3 ml dalam 3 ml N/S : relaksasi pada otot polos. 4. Alupent (Metaproterenol) • Frekueensi : Tiap 4 jam/4kali/hari. • Dosis : 0,3 ml dalam 3 mll N/S. • Kerja : Relaksasi pada otot polos bronkhus. • Efeksamping : Tachikardi, gugup, tremor dan mual. 5. Atropine (Atropine sulfate) • Frekuensi : tiap 6 jam. • Dosis : 1 ml (0,4 mg/ml) dalam 3 ml N/S • Kerja : Relaksasi otot bronkhus. • Efek samping : takikardi, gugup, tremor dan mual. E. Kortiko steroid • 1. 2. 3. 4. Jenis: Decadron (Dexamethasone). Pro decadron (dexamethasone, issuprel) Bechomethasone. Aerolized steroid baru untuk asma alergika, tidak ada absorbsi atau supresi adrenal. Kerjanya secara lokal, Berguna terutama pasien dengan terapi steroid yg kronis untuk mengurangi kebutuhan steroid sehari-hari. F. antibiotik • Gentaminicin. • Mycostatin. • Untuk menghindarkan bronkhospasme dan udema bronkhus, sebaiknya pada penderita yg diberikan antibiotik ditambah dengan bronkhodilatator. • Efektif bila ada infeksi yang dapat dicapai dari permukaan jalan nafas.
