Greater than 95% of all ATP production in the adult heart is derived from mitochondrial oxidative phosphorylation. Fatty acid beta-oxidation makes up 50-70% of this ATP production. The remainder of ATP is derived from glycolysis and GTP formation in the tricarboxylic acid (TCA) cycle. A number of enzymes involved in myocardial fatty acid metabolism are regulated largely by nuclear receptor transcription factors including peroxisome proliferator activated receptors (PPARs) and PPAR gamma coactivator 1 alpha (PGC-1α). In pathophysiological settings like obesity and diabetes, there is an increase of free fatty acid in the cardiac myocyte which enhances the ability of PPARs to increase transcription of their target genes.
