Clients may contact us directly to make appointments but we will request that a physical exam and laboratory testing be performed before the consultation (see scheduling an appointment). These procedures can be completed during the behavior consultation if you as the primary veterinarian request this. If you have questions regarding a specific case, please fill out this UT Behavior History (SHORT CONSULT) form and e-mail or fax and then contact Nelle Wyatt, UTVMC Animal Behavior Technician. Phone consultations with the client and you will be considered on an individual basis. The fee is $75/hour and this can be credited to a future behavior consultation at UTVMC. Animal Owners Scheduling an Appointment Physical or medical problems often contribute to a behavioral problem or can affect a treatment plan. Please contact your regular veterinarian to schedule a physical exam if your pet has not been examined for this behavior problem in the last three months. Routine blood work and possibly other tests like urinalysis and endocrine function are important in assessing your animal’s health and will be required before any medications will be prescribed. Please see your regular veterinarian for these laboratory tests as well. Lab testing can be performed during the consultation if requested by your veterinarian. Please fill out the appropriate form and RETURN TO US 3 DAYS BEFORE APPOINTMENT at the UTVMC Behavior Clinic. The dog and cat forms are interactive PDFs so you may type on the form, then save the file to your computer desktop. You may then send the form back to us as an e-mail attachment. If you prefer to hand-write your answers, simply print off the blank form and fax or mail the completed form to us. Animal Behavior Appointment and Fee Information (11-2012) pg. 1 Veterinary Medical Center 2407 River Drive, Knoxville, TN 37996 (865)974-VETS or (865)974-8387 Animal Behavior Consultation Fee Schedule LEVEL 1 [FEES cannot be refunded if either follow-up appointments or phone/e-mail follow-up are not utilized.] CANINE ($335): Two (2 hrs.) consultations with six months of unlimited phone/ e-mail follow ups. FELINE ($125): One consultation (approx. 2 hrs.) with six months of unlimited phone/ e-mail follow-ups. LEVEL 2 ($125): Two (1 hr.) consultations only, cases are often Court mandated. NEW PET ($62): One (1 hr.) consultation. Training classes from a reward-based trainer are always recommended (see AVSAB recommendations on choosing a trainer). Prevention consultations are not intended to address serious behavior problems like aggression or severe anxiety. We may require a full consultation to address certain problems; however, the fee can be credited to a full consultation. LEVEL 3 ($62): One (1 hr.) consultation only. This is for the person who would like to talk to our team without committing to a Level 1 plan. We can evaluate your pet, and provide a basic treatment plan. The fee can be credited to a Level 1 consult within the three months of the initial consultation. EQUINE, SMALL RUMINANTS, AVIAN or EXOTIC ANIMALS ($62): One consultation with six months of phone/e-mail follow-ups. Phone consultations (if applicable) - $75/hour Recheck Appointments - $30 + $1/minute for training with Ms. Nelle Wyatt Training only appointments - $1/minute Travel fee - $1.50/miles with $20 minimum NOTE: MEDICATIONS CANNOT BE PRESCRIBED at LEVEL 2 or 3 CONSULTATIONS due to additional patient follow-up are required after psychoactive medications are prescribed.
