BHRT Pellet Therapy Hormone Replacement - $ 400 women/$600 men
Hormone replacement therapy has been extremely beneficial to men and women to combat hormonal
imbalances that develop as we age. These hormonal imbalances can be responsible for fatigue,
irritability, dryness, loss of libido, hot flashes, and weight gain. Pellet therapy is a long-lasting form of
replacement hormone therapy in which slow-release hormone pellets are implanted beneath the skin in
a simple, 5-minute office procedure. These implantable pellets do away with the need for daily hormone
pills, frequent application of hormone “creams” to your thighs, or frequent shots and provide a smooth,
continuous release of the needed hormones. Hormone replacement therapy is individualized for each

performing weight-loss surgery in 2001 and has extensive experience treating patients who need help
losing weight in a safe and effective manner. For patients that don’t need or qualify for bariatric
surgery, we can help you find a medical weight loss solution that works for you.
Alternative Medicine Consultation - $150
Some medical concerns are difficult for patients to find help for or to discuss with their regular
physician. One such area is the use of medicinal-grade marijuana to treat certain medical conditions. In
Michigan, patients may benefit from the use of medical marijuana for glaucoma, Crohn’s disease,
hepatitis C, HIV+ or AIDS, cancer, amyotrophic lateral sclerosis, severe persistent nausea, severe and
persistent muscle spasms, severe and chronic pain, cachexia or wasting syndrome, and seizure
disorders. Dr. Free began performing certifications over 5 years ago after noticing remarkable
improvements in patients with Crohn’s disease that were no longer responding to conventional medical
treatments. Dr. Free believes that medicine should be about treating illness with compassion.

Medical Aesthetic Services
Medical Aesthetics Consultation - $ 100, included with services*
Facial and body rejuvenation can help you look and feel your best and our patients are consistently very
pleased with their results! However, these treatments are medical, and involve prescription medications
and/or procedures that should be performed by an appropriately licensed, trained, and experienced
medical provider. At Revive, Dr. Free with Harbor Shores Medical, PLLC provides the medical expertise
and skills necessary to ensure that each patient has the best possible results. Before starting a
treatment, a consultation with Dr. Free provides a plan for achieving your goals that is individualized
just for you. A consultation is required for new patients and is provided at no cost when combined with
medical aesthetic procedures during the same visit. If no procedures are performed on the day of the
consultation, the fee may be applied towards a medical aesthetic procedure (other than Botox)
performed within 30 days of the consultation.
Botox - $ 11 per unit
Botox, by Allergan, is a wonderful treatment for dynamic lines on the face, including crow’s feet lines
around the eyes, forehead wrinkles, frown lines, bunny feet lines on the nose, and vertical lines above
the lips (“smoker’s lines”). Botox can also be used to provide a non-surgical eyebrow lift. Botox works by
gently weakening a target muscle, allowing the skin affected by that muscle to relax and become soft
and smooth. Results are usually seen within 7 to 10 days and typically last approximately 4 months.
Juvederm Ultra XC - $ 550 per syringe
Juvederm Voluma XC - $ 800 per syringe
A revolutionary new product from Allergan, this filler is used below the dermis to restore lost volume
due to loss of youthful fat deposits in the mid-face. Cosmetic physicians now recognize that restoring
lost tissue volume in the cheeks is the foundation of rejuvenating the entire face. Once lost volume has
been properly restored in the mid-face, rejuvenating the rest of the face is much easier with more
natural-appearing results. Just like the other fillers from Allergan, Voluma XC contains a fast-acting local
anesthetic agent for a comfortable facial rejuvenation experience. Lasts 2 years.
Bellafill - $ 1000 per syringe / $ 4000 – 5 syringe kit
This amazing product from Suneva is a dermal filler that is used in many of the same places as other
fillers. This product, however, lasts at least 5 years and even longer in studies from Europe. Provides
long-lasting treatment of nasolabial folds, Marionette lines, sunken eyebrows, tear troughs, and even
non-surgical neck lifts and hand rejuvenation. This product is also the only dermal filler that is FDAapproved for the treatment of moderate to severe facial acne scars. This unique product is a collagenbased filler that incorporates microspheres of PMMA (used safely for years for hold joint replacement
hardware in place) that stimulate the steady, long-term production of the body’s own collagen. With
other fillers, the body’s natural production of collagen stops about one year after injection and needs to
be replaced. With Bellafill, the collagen continues to remain in place 5 years later in U.S. studies (up to
10 years in European studies). We also use a unique delivery system that allows gentle, non-traumatic
placement of this product for safe, immediately-visible results. Lasts 5+ years.

From Allergan, the makers of Botox, Juvederm Ultra XC is a dermal filler that is used to replace lost
tissue in the dermis in order to soften or erase nasolabial folds on either side of the nose, Marionette
lines at the corners of the mouth, and provide new definition to the lips for a beautiful and refreshed
smile. Juvederm Ultra XC contains a fast-acting local anesthetic for a comfortable facial rejuvenation
experience. Juvederm Ultra XC is slightly thinner than Juvederm Ultra Plus XC and can also be used for
smaller, less deep lines and for enhancing lip edge definition. Lasts 12-14 months.
Get your legs beach-ready for the summer! Spider veins are those unsightly superficial blood vessels
that often appear on the legs over time. Sclerotherapy is a well-established, safe, and effective method
of obliterating those spider veins. After treatment, the spider veins slowly begin to disappear, with the
final result by 2 months afterwards. We use Asclera, a revolutionary product that is much safer than the

Belotero BALANCE - $ 400 per syringe
This dermal filler from Merz Cosmetics is a similar to other fillers but is specially formulated for a much
thinner consistency that allows treatment of very superficial lines that are difficult to treat with other,
thicker products. A common use for this unique filler for treating left-over “11” lines above the nose
which improved but are still noticeable after treating the area with Botox. This filler is also useful to
treat “smoker’s lines” above the upper lip in combination with Botox and doesn’t cause a bloated,
unnatural thickening of the upper lip. Belotero BALANCE helps smooth moderate to severe etched in
facial lines and wrinkles with a quick, easy treatment. Lasts 12-14 months.
