EQUIPE D'ACCUEIL 4393 Laboratoire d’Investigation Clinique (LIC): Epidémiologie Clinique-Evaluation Médico-économique Année 2009-2010 Titre ou thème de recherche : Les thématiques de l’équipe sont centrées sur les cohortes de maladies rares dans le cadre des centres de référence et sur l’évaluation de l’innovation au plan médical mais également économique. Les principaux domaines concernent les dermatoses rares, les spondylarthropathies, le cancer colorectal, l’oncogériatrie et la décision médicale. Dermatoses bulleuses. Après avoir travaillé sur l’identification des facteurs de risque des maladies bulleuses, nous avons créé et validé un score pronostique pour le Syndrome de Lyell (SCORTEN). Ce score étant couramment utilisé par différentes équipes, notre projet est d’améliorer les performances de ce score et de le tester dans différentes populations (collaborations internationales). Neurofibromatose, NF1. Nous avons identifié les facteurs de risque de morbi-mortalité sur des cohortes française et nord-américaine, évalué le retentissement de la NF1 sur la qualité de vie de l’adulte et de l’enfant. Notre objectif est à présent i) de développer et valider un score pronostique pour identifier les patients nécessitant une imagerie régulière, ii) d’estimer la mortalité de la NF1, iii) et de tester de nouvelles biothérapies chez les patients ayant des neurofibromes inopérables menaçant le pronostic vital (phase II). Hidradénite suppurée (Maladie de Verneuil). Nous avons constitué une cohorte de plus de 500 malades caractérisés phénotypiquement. Notre premier objectif a été d’estimer la prévalence de la maladie en population générale en collaboration avec la Sofres, puis de rechercher les facteurs de risque (J Am Acad Dermatol. 2008) et les facteurs associés à la gravité de la maladie (J Am Acad Dermatol 2009). Nos projets consistent i) à mieux décrire les caractéristiques de la maladie, ii) évaluer différentes thérapeutiques, et iii) évaluer la prévalence des manifestations articulaires, décrire leur phénotype et profil évolutif, ainsi que leur retentissement sur la qualité de vie. Par ailleurs l’équipe participe à un projet européen (FP7) sur la qualité de vie dans les maladies rares. 2 –Pronostic du cancer colorectal Parallèlement aux travaux physiopathologiques réalisés pour mieux comprendre le pronostic de ce cancer (Gut. 2008 ; Cancer Res. 2008), nos objectifs sont de montrer que la pratique systématique d’une TEP dans le suivi des malades de stades III et IV traités chirurgicalement permet de détecter plus précocement les récidives et d’améliorer le pronostic (PHRC 2007). Dans le cadre de la campagne de dépistage systématique, nos projets concernent i) l’évaluation de tests biologiques alternatifs pour le dépistage dans quatre départements franciliens (PHRC 2009), ii) la recherche de facteurs limitant la participation des médecins généralistes au dépistage dans le département du Val de Marne, iii) et l’évaluation de stratégies permettant d’améliorer le dépistage (Financement CNAM). Nos travaux concernent également l’évaluation de la prise en charge carcinologique chez les personnes âgées afin de l’optimiser, nos objectifs spécifiques sont i) de tester des échelles d’évaluation standardisées et ii) d’identifier les facteurs prédictifs de tolérance des traitements (financement INCA sur appel à projet UPCOG). 3- Spondylarthropathies Dans ce domaine nos projets concernent d’une part la transformation de la file active de patients suivis sur le site en une véritable cohorte (en cours) afin d’évaluer les effets des nouvelles biothérapies et d’autre part l’identification des scores radiologiques les plus appropriés (SASSS, mSASSS et BASRI) pour l’évaluation des fomres récentes. A partir de la cohorte nationale de 700 patients évalués de manière standardisée (DESIR), les objectifs spécifiques sont i) d’analyser la validité du mSASSS en routine clinique, ii) d’évaluer les performances des scores en termes de validité et de sensibilité au changement dans les formes débutantes et iii) d’évaluer l’impact des lésions radiologiques sur la fonction.. 4 – Evaluation de l’innovation dans d’autres domaines Ces projets concernent l’évaluation des performances de nouveaux tests diagnostiques dans les infections sévères (HEMATOCPLUS, CIRC 2006 et, SEPTIFAST, STIC 2007) et la coqueluche (DIAMOCOQ 2005) ; l’évaluation de l’innovation en chirurgie, assistance robotique et coelioscopie (ARTE, CIRC 2005 et Propenlap 2006 )….
