Helicobacter pylori Urea Breath Test Collection Procedure
Materials:
Materials provided in single-patient BreathTek Collection Kit contains:
• One plastic drinking cup
• One tri-fold wallet containing:
— 1 pouch of Pranactin-Citric powder
— 1 plastic straw
— 2 breath collection bags: 1 blue bag for BASELINE specimen and 1 pink bag for POST-DOSE specimen
— 1 specimen transport bag
— Test instructions
— A set of 4 self-adhesive bar code stickers. All bar codes should bear the same number
Other materials needed:
• Mechanical timer
• Scissors for opening Pranactin-Citric pouch
Step-By-Step Procedure:
1. Verify the following information with patient:
A. Remind patient that Pranactin-Citric contains phenylalanine. Phenylketonurics restrict dietary phenylalanine.
Note: Pranactin-Citric contains a 75 mg per dosage unit of phenylalanine that is approximately equivalent to a diet cola
soft drink.
B. Nothing by mouth for 1 hour prior to testing.
C. Abstain from antibiotics, proton pump inhibitors, and bismuth for 14 days prior to initial testing.
Note: H2 antagonists may be substituted, as these medications do not affect test results.
2. Remove and open the tri-fold wallet in BreathTek Collection Kit. Label each breath collection bag with a bar code sticker
provided and a patient specimen label with date, time, and phlebotomist’s initials.
3. Collect BASELINE breath specimen:
A. Remove pull-off cap from mouthpiece of blue breath collection bag.
B. Instruct patient to:
1)Breathe normally.
2)Take a deep breath then pause momentarily.
3)Exhale into mouthpiece of bag.
C. Replace cap on mouthpiece of bag.
4. Prepare Pranactin-Citric solution no more than 60 minutes before giving to patient. Urea slowly decomposes in water.
A. Remove pouch and tap to settle contents into bottom of packet.
B. With scissors, cut off top of packet and carefully empty contents into drinking cup provided. Make sure to transfer all
contents by tapping.
C. Add drinking water to fill line indicated on outside of container by a raised plastic ridge.
D. Replace lid securely and swirl up to 2 minutes to dissolve packet contents; typically, only 1 minute is required for
complete dissolution. The resulting solution should be clear with no particulate matter. If particulate matter is
present after thorough mixing, the solution should not be used.
5. The patient drinks solution using straw provided, without stopping. Advise patient NOT to ‘rinse’ the inside of his/her mouth
with solution before swallowing. Discard straw and cup.
6. Set timer for 15 minutes. The patient should sit quietly and should not eat, drink, or smoke during the 15-minute interval.
Timing is critical.
7. After 15 minutes have elapsed, remove pink breath collection bag and collect POST-DOSE breath specimen as described in
step 3 above.
8. Store specimen at 15° C to 30° C until analysis is performed. Use the plastic specimen transport bag for transport of breath
specimen.
