1.1 Provide Online Directory of Services
Please note: Provide will launch a fully revised interactive online version of their Directory of Service in
September 2014.
In addition to the currently available service information on their corporate website they will introduce a version specifically for and only
accessible to those who commission and refer to their Services: Commissioners, GPs etc. (Customers).
This Customer Directory contains not only detailed information of the Services but also gives information on waiting times and performance
(where reported), referral forms and other documentation such as patient information leaflets.
This content will only be available to registered users, registration requests and further details can be found on the Provide website after 1st
September 2014:

1.2 Central Point of Access (CPA) to Integrated Care Teams
Central Point of Access Service Description & Contacts
CPA Service Description
 Integrated Care Services can be accessed through one Central Point of Access (CPA) who will manage ICT referrals to Provide for
patients registered with GPs in your geographical area.
 The CPA will be manned by Telephonists and supported by a named registered nurse contact, virtually.
 The named registered nurse will be available to resolve clinical queries to enable appropriate and safer allocation of response times.
 Depending on urgency service users can be seen within 24 hours.
 The Central Point of Access also acts as a central contact point for patients and their family.

Nursing care and advice for patients who are suitable for home care and treatment.
Complex case management for patients with long-term conditions and for frail elderly patients who require extended support to
keep them safe at home.
District Nursing and Admission Avoidance Nursing
District Nurse Liaison – Helping patients transfer back home from hospital or hospice.
District Nurse Palliative Care – Provide support and advice for those with a life-limiting illness.

Referral Criteria:
 For service users aged over 18 years old who live in Mid Essex who are registered with a GP in that area.
 The Central Point of Access will handle referrals from primary care, secondary care and other health and social care professionals.
 These referrals are for patients presenting with a healthcare need that can be appropriately and safely managed by the Integrated Care
Service.
 All referrals should be made to the Central Point of Access using the attached generic Referral Form.

Operating Times: Any referrals sent to the CPA out of operational hours will not be picked up until the next working day.

Community Nursing & Complex Case Management
Area Covered: Adults registered with a GP in the NHS Mid Essex area.
Service Description
The service provides a nursing service for those who for reasons of acute or chronic illness are unable to leave their home by domestic vehicle.
The service can offer the following treatments:
 Rehabilitation – to restore independence or enable the carer to care independently. Time limit: three months
 Wound management – for trauma and chronic wounds
 Lower-leg care – venous, mixed, diabetic or arterial ulceration requiring assessment, dressing or monitoring
 Post-operative care – includes care of drains or wounds, constipation and pressure relief. Time limit: first six weeks post-surgery
 Administration of medication
 Treatment and monitoring of health problems – includes IV therapy. Time limit: six weeks
 Long-term support of chronic degenerative conditions (such as MS, paraplegia, dementia, CCF) – includes continence assessment,
catheter care and bowel management
 Preventing health complications – includes flu vaccinations
 Terminal or palliative care – includes IV therapy
 Teaching self-care to patients and patients carers – includes administering injections, PIC lines, eye drops and dressings
 Avoiding unnecessary hospital admission through patient and carer education, symptom management, enrolment on to various health
care programmes, adoption of Telehealth programmes to empower patients to self-manage their chronic conditions.
Team Description: Our team includes qualified specialist practitioners, nurses and healthcare assistants.
Exclusion criteria:
 People who live in Mid Essex but are not registered with a Mid Essex GP
 People under 18 years of age
 Provision of nursing tasks within nursing homes or other support which should be provided by the home
 Core primary care provision which is contained within the GP contract and remains the responsibility of Primary care.
Referral Process
 Self-referral of referral by a healthcare professional.
 Referral form should be faxed to the contact details below.
 We will call the patient within 24 hours of referral to arrange a visit. If the referral is urgent, we’ll call the patient within four hours

District Nurse Palliative Care
Area Covered: Adults registered with a GP in the NHS Mid Essex area or patients in a Mid Essex community hospital
Service Description:
The service helps patients, and their families, who have a life-limiting illness and are in the last stages of life.
The service can provide:
 Training
 Ongoing support
 Advice
 Guidance
 This includes:
 Visiting the patient
 Liaising with other agencies to achieve the patient wishes
 Acting as a resource to the district nurse teams in aspects of palliative care
 Assistance with application of the End of Life Tools, such as the Preferred Priorities of Care documentation and the use of the Liverpool
Care Pathway

Team Description: Our team includes two qualified district nurses with specialist palliative care skills

 The patient can refer themselves or a relative, or be referred by any healthcare professional.
 Referrals can be made by phone or fax, using the contact details below

Occupational Therapy Access Criteria:
 The patient requires rehabilitation with an Occupational Therapist to regain or improve independence with activities of daily living
 The patient requires therapy to manage or reduce the impact of impairment e.g. hand splinting, fatigue management
 The patient has a terminal illness and is not under the care of Farleigh hospice or eligible for their services (Farleigh hospice tel.

 The patient requires short term loan of equipment* (6 months or less) to improve independence with activities of daily living
*People who ONLY require either major adaptations to their home or long term provision of equipment should be referred to the social care Occupational Therapy team on

Physiotherapy Access Criteria:
 The patient is housebound and unable to access outpatient therapy services, (but not due to transport issues)
 The patient has specific needs that necessitate that therapy would
a) Be more appropriately provided at home e.g. mobility within the home environment, or
b) The treatment outcomes would be compromised by travel into outpatient therapy (e.g. some stroke patients)

Unscheduled Therapy Domiciliary Service
Area Covered: Adults who are registered with a GP in the NHS Mid Essex area
Service Description:
 The service therapists diagnose and treat health-related conditions which limit the patients ability to move and perform everyday
activities
 The service offers specific purposeful activities, exercise and advice to help the patient with independence, mobility, pain management
and living a healthy lifestyle
 The service offers occupational therapy if the patient:
 Need rehabilitation to regain or improve independence with everyday activities
 Need therapy to manage or reduce the impact of impairment, such as hand splinting or fatigue management
 Have been diagnosed with a terminal illness and aren’t already under the care of a hospice
 Need short-term loan of equipment (six months or less) to improve independence with everyday activities
 The service offers physiotherapy if the patient:

Are housebound and unable to access outpatient physiotherapy services, but not because of transport issues
Have specific needs which mean therapy would be more appropriately provided at home, such as some stroke patients
Our team includes occupational therapists, physiotherapists and therapy assistants
Exclusion Criteria: Patients requiring either major adaptation to their home or long-term provision of equipment who should be referred to the
social-care occupational therapy team
Referral process:
 The patient can be referred by any health or social-care professional by fax or post.
 Copies of our referral forms can be obtained by calling us on the number below
 The patient will be seen within 2 days to 18 weeks, depending on urgency

Assessment and Rehabilitation Unit (ARU) - Maldon
Service Description:
 Assessments and diagnostic tests to either discharge patient with a care package or admit them to an inpatient bed.
 Day-treatment service for patients receiving blood transfusions and Intravenous medications.
 One-stop anticoagulation clinic.
Team Description: Nurse led service and supported by agency doctor three times a week.
Referral Criteria/Process:
 Adults (18 years or above) who are registered with a GP in the NHS Mid Essex area.
 The service user can be referred by any healthcare professional.
 Referral to be faxed.

Rapid Assessment Unit (Admissions Avoidance)
Area Covered: Patients Registered with a GP in Mid Essex
Service Description
The service assesses any symptoms or problems caused by falls, infections, possible below knee DVT and mobility. If necessary they can:
 Do blood tests, x-rays, ultrasounds and ECGs

 Provide transport in our non-emergency ambulance
 Do follow-up assessments
 Provide aids, such as raised toilet seats and walking frames
The service is not a walk-in clinic. Appointments are arranged once a referral is received.
All assessments are carried out on either the same or next day of referral.
Investigations for blood tests, x-rays and ECGs can be performed on the day within the department.
Team Description: The team includes registered nurses, physiotherapists, occupational therapists and healthcare support workers, and is
supported by a team of GPs and Care of the Elderly Consultant.
Referral Criteria
Adults (over 18 years) within NHS Mid Essex with a known health condition
Inclusion Criteria:
 Immobility
 Confusional states
 Falls
 Dehydration
 Incontinence
 Heart/Respiratory failure (not acute SOB)
 Asthma
 Infections-Chest, urine or cellulitis (ERON stage 1& 2)
 Joint Pains
 Diabetic conditions – unstable blood sugars
 Suspected below the knee DVTs (see exclusion criteria)
 Haematology and methyl-prednisalone infusion patient
Exclusion Criteria
 The service are unable to assess acutely ill patients, such as those with a suspected heart attack, stroke, acute abdominal pain, broken
bones or fractures. Or anyone requiring mental health assessment.
 Myocardial Infraction
 CVA including TIAs
 Trauma including suspected fractured neck of femur
 Patients who are medically unstable
 Patients with low bp, increased pulse, low blood gases, or decreased levels consciousness
 Patients who are fitting
 Severe unexplained abdominal pain
 Persistent back pain- not associated with movement

Service Description:
Service treats patients with a diagnosis of heart failure.
The service can offer the patient:
• Clinic and home visits to assess and educate, and optimise use medication
• Inpatient hospital visits to assess before discharge
• SOS response - If patient becomes unwell, call us and we’ll come out to assess to help prevent patient from having to go to hospital
• Cardiac rehabilitation: a ten-week course of exercise based on patient’s ability. Service will give information about patient’s condition
and how best to manage it

• Clinical assessment which includes blood pressure and pulse oxygen saturation

Continence Advisory Service
Area covered: Adults registered with a GP in the NHS Mid Essex area.
Service Description:
The service treats or supports:
 Urinary and faecal problems/incontinence
 Pelvic floor problems, including electrical stimulation treatment
 Teaching and management of intermittent self-catheterisation
 Selection and supply of continence products, in cases where specific criteria is met
 Long-term management of incontinence
 Education for carers and professionals
 Service is not able to treat people with indwelling urinary catheters.
Team Description: Includes a clinical nurse specialist, continence adviser, continence nurses, associate practitioner and healthcare support
workers.

Coronary Heart Disease Specialist Nurses
Area Covered: Patients registered with a GP in the NHS Mid Essex area
Service Description:
The coronary heart disease nurses can help patient’s manage their angina if they have been prescribed medical treatment only.
The service is not able to see patient’s if they have been earmarked for further intervention.
The service will visit patients at home (within 2 weeks of referral) and show them what they can do to help themselves, such as:
 Regular-paced exercise, such as walking, swimming and cycling
 Relaxation and breathing exercises
For the following 12 weeks our nurses will encourage and support the patient’s by phone.
Referral Criteria:
Patient’s must have a confirmed diagnosis of angina.
Patient must be willing and able to do some exercise.
Referrals from any healthcare professional by fax, letter or telephone.

Diabetes – Adult Community Service
Area Covered: Adults (18 and over) who are registered with a GP in the NHS Mid Essex area
Service Description
The Service provides step-up care for people with Type 2 diabetes and some patients with Type 1 diabetes. The service offers a One-StopShop model providing:
 Glycaemic intensification

Insulin conversion either in a group or one-to-one depending on patient choice
Structured group education via the CREDIT education programme, or “one- to-one” if preferred by the patient
Telephone support and advice to support clinical activity
Telephone support and advice for other primary care colleagues, district nurses and other community-based healthcare professionals
as required
 Training for a variety of healthcare workers and residential-home staff
 Smooth transition and speedier discharge for those who have been in Mid Essex Hospital Trust (MEHT)
 hospitals and the Essex Cardiac Centre within agreed clinical pathways.
The service are unable to undertake routine surveillance of diabetes. This remains the responsibility of your GP.
For the first appointment, please ask the patient to bring:
 A urine sample
 A list of current medications
 Any records relating to your diabetes, such as home monitoring
The service will see patient’s within 18 weeks of referral. The service aims to discharge you back to the GP once the patient’s care package is
complete or when the patient has achieved optimal control within the above ratios
Exclusion Criteria:
The service are not able to treat you if you are:
 Pregnant or need pre-conceptual counselling
 A newly diagnosed Type 1 patient
 Have two or more diabetes-related complications, such as renal disease or acute foot problems
 Need an insulin pump
Team Description: The service are supported by the wider integrated team at our diabetes centre in Broomfield Hospital, Chelmsford. Our
team includes GPs with special interest (GPWSI), diabetes specialist nurses, a dietitian and podiatry support.
Referral process: Service users can self-refer or be referred by any healthcare professional.

Service Description:
 The service treats people with acquired voice, speech, language, communication and/or swallowing disorders and difficulties, such as:
 Stroke, traumatic brain injury, dementia, critical care, chest infection, aspiration pneumonia, dehydration and nutritional
compromise, head and neck cancer, voice pathologies and end-of-life care.
 Long-term conditions, including neurological ones such as Parkinson’s disease, motor neurone disease and multiple sclerosis
 Associated speech and language pathologies, such as aphasia, dysphagia, dysarthria, dyspraxia, cognitive language processing
difficulties and dysphonia
 The service are not able to treat people with developmental disorders which persist into adulthood.
 The service provide patient-centred, individualised packages of care according to need to inpatients, outpatients and patients in acute,
intermediate and community care.

The service can offer the following:
 Vascular assessment using waveform Doppler
 A range of therapies for the promotion of wound healing, including the Versajet debridement system and topical negative pressure.
 We are not able to offer surgery for varicose veins but depending on individual circumstances we may refer you to the vascular surgeon.
Children will be seen in collaboration with the paediatric community nursing team.

The service loans equipment, such as toileting and bathing aids, hoists and pressure area care equipment, as well as minor adaptations, such
as grab rails or chair raisers, to help with the patient’s health needs and enhance their independence.
The service can also fast-track community equipment against the Social Care budget where the appropriate procedures have been followed
and provision meets Social Care criteria, eligibility and FACS (Fair Access to Care Services) criteria.
Referral process: Equipment requests from clinical staff (occupational therapists, physiotherapists and district nurses) following assessments.

Falls Prevention
Area Covered: Adults living in the mid Essex area
Service Description
The service helps prevent patient falls in the future reducing risk of serious injury, such as a fractured hip, and enabling patients to live
independently for as long as possible.
The service assesses the patient and decides whether they need:
 Treatment and advice from our team, including physiotherapy and occupational therapy
 A place in our falls prevention class.
Falls prevention classes are run by postural stability instructors at 15 locations throughout the area including Chelmsford, Great Baddow,
Maldon, Witham, Braintree, Halstead, South Woodham Ferrers and Burnham on crouch
Classes are free, last one hour and run every week for 24 weeks
Team Description: The service team includes a falls prevention physiotherapist, occupational therapist and adviser, supported by nurses and
a doctor if required
Referral process
 The service user can refer themselves or by a member of the family.
 Any healthcare or socialcare professional can refer into this service.

Smoking Support Service
Area Covered: Anyone living in the mid Essex area

Service Description:
 This service puts the patient in touch with a stop-smoking adviser who will support the patients attempts to quit for the first six to 12
weeks, on either a one-to-one or group basis during daytime or evening.
 The patient’s adviser may be a practice nurse, local pharmacist or an adviser direct from our Stop Smoking Support team.
 The patient’s adviser will tell the patient about all the treatments and medication available to help them quit, including nicotine
replacement therapies (NRT) such as patches or tablets, such as Champix or Zyban. They can help the patient choose what’s right for
them and make sure they are using it correctly
Referral process
 The patient can refer themselves directly by calling the service direct on 0800 085 2113 (freephone).
 The patient can be referred by any healthcare professional phone or by faxing or posting a Stop Smoking Support Service referral form
to the contact details below.
 Referral pads can be delivered to healthcare professionals on request
 Whichever method of referral is used, the service will need the patients name, date of birth, NHS number (if known) and a daytime
contact telephone or mobile number to arrange an appointment.
 The service will contact the patient within two weeks of referral. The treatment can start within two to three weeks.
Locations
 The service user has a choice between face-to-face and telephone clinics.
 Face to face Clinics are run in a variety of places in mid Essex such as such as leisure centres, churches and community centres at
different times during the day and evening throughout the week

 An intensive package of, goal-based support, provided for up to 6 weeks, designed to help individuals regain and maintain
independence and avoid unnecessary hospital admissions.
 Ethos is to encourage and support customers to find a way to achieve everyday activities themselves.
 Multi-disciplinary team of support assistants, support leads, nurse, physiotherapist, occupational therapist and information and advice
officer work with customers to help restore confidence and re-learn skills needed.
 Includes the provision of appropriate equipment, required to support achievement of goals.
 It is available to individuals in a variety of circumstances, but especially those no longer coping at home and to people being discharged
from hospital following illness or injury.
 Each element of the multi-disciplinary support offered is part of the Reablement package and is not available as a standalone service
via this route.
Referral Criteria
 Customers must be 18 years or older and identified as likely to benefit from a period of reablement (regardless of diagnosis or life limiting
illness).
 Must be willing to engage and work towards achievable self-defined goals and aspirations.
 Must be medically and mentally stable (can be cared for in the community).

 An intensive health and social care intervention for up to 6 days to avoid inappropriate hospital admission until alternative services can
be put in place if required.
 For vulnerable adults during an exacerbation of their condition or experiencing either a sudden or unexpected deterioration in their
ability to live independently.
 This service is for patients at imminent risk of an emergency hospital attendance/admission if they do not receive an urgent intervention
(Note: rapid response service starts within 2 hours of referral).
 Multi-disciplinary team of support assistants, support leads, nurse, physiotherapist, occupational therapist and information and advice
officer work.
Referral Criteria:
 A Rapid Response referral can be made by any healthcare professional.Only referrals from healthcare professionals will be acceptedSocial care cannot refer into this service and the service cannot be used if the person already has a social care package.

Home from Hospital – The Dengie Project Trust
Area Covered: Adults registered with a GP in the NHS Mid Essex area
Service Description:
 A low intensity intervention, primarily for customers being discharged from hospital, but also following any other health/social care
intervention when medical needs have been met.
 Service provides practical and emotional support for up to 6 weeks, as well as basic equipment, to help make a successful transition to
returning and staying at home.
 Can be a standalone service for low level needs or a top up for those requiring health/social care support.
 Examples: shopping, housework, moving furniture, home safety assessments. Personal care is not included.
Referral Criteria:
 Customer must be over 65 year’s old

as well as walks along the promenade and shopping for lunch time meals.

What is a palliative care pharmacy?
 A palliative care pharmacy is a pharmacy contracted by NHS Mid Essex to stock specialist palliative care medication.
 The drugs available at a palliative care pharmacy do not cover all eventualities but any pharmacy can usually order supplies of a
prescribed drug to be available for same or next day delivery.
About this service
 This service aims to provide immediate and consistent access to palliative care and other specialist medication across Mid Essex.
 The following section lists the specialist palliative care community pharmacies with extended opening hours, good accessibility and
parking in Mid Essex.
 These pharmacies have been contracted by NHS Mid Essex to stock an agreed list of specialist medication to supply to your patients on
receipt of a prescription during normal business hours.
 All the palliative care pharmacies have made a commitment to ensure that anyone who needs this service has prompt access to the

Community Hospitals/Community Beds
All referrals for community beds need to be made through the RAU for a clinical discussion regarding
criteria and booking of the bed. Rapid Assessment Unit at Braintree Community Hospital - 01376 555921
Community Hospital in-patient beds are for mid Essex residents experiencing an exacerbation of an existing condition, a simple short-term
illness or those requiring support to recover from an acute hospital episode of care or following a decline in health. Community beds also
support end-of-life palliative care.
What does the service do?
We can offer the following in-patient services:
 Treatment of simple illnesses or exacerbations of chronic illness that require a hospital stay but which do not require the
intensity of treatment provided in an acute hospital such as Broomfield
 Rehabilitation for patients experiencing a decline in their health and well-being while at home, or for people requiring a
period of support and rehabilitation to recover from an episode of care in an acute hospital
 Specialist stroke rehabilitation – available at St Peter’s Hospital
 End-of-life palliative care
 Day-treatment service: for people requiring intravenous infusions, such as blood transfusions and methylprednisolone
infusions
Who is the service for? Anyone registered with a GP in the NHS Mid Essex area.
Referral process
Community hospital inpatient beds can only be accessed through referral by GP, community nurse or continuing healthcare team to the Rapid
Assessment Unit at Braintree Community Hospital.

• There are three community hospitals in Mid Essex – St Peter’s Hospital in Maldon, Braintree Community Hospital or Halstead Hospital.
• Admissions are accepted between 9am-10pm

Non Emergency Patient Transport Services (PTS)

Service Description:
The purpose of the service is to provide safe, appropriate and dignified non-emergency patient transport that enables patients to travel to and
from healthcare in primary, community, secondary and tertiary settings in a reasonable time and in reasonable comfort, without detriment to
their condition.
Access Criteria:
Patients are assessed at the time of booking by the Assessment & Booking Service (contact details below) and are then asked a series of
questions to determine if they are eligible for PTS, if not an alternative is suggested to the patient. (i.e. voluntary sector transport)
