The Academy continues to grow the IRIS® Registry (Intelligent Research In Sight) — more than 9,000 physicians are now participating in this groundbreaking initiative. As a certified EHR technology, the IRIS Registry automatically extracts data for clinical quality measures, which also qualifies for PQRS reporting, and submits it to the Centers for Medicare and Medicaid Services (CMS) on the ophthalmologist’s behalf. Use the IRIS Registry to examine your practice’s procedures, monitor your performance and benchmark it against peers, and meet requirements and streamline reporting for the Physician Quality Reporting System (PQRS). Improve performance and outcomes in ophthalmology Continuous measuring and reporting helps ensure that you deliver effective, consistent, high-quality care. Using electronic health records (EHRs), the IRIS Registry collects 22 www.aao.org/store With the IRIS Registry you can: • Improve patient care. Monitor patient interactions, track interventions, identify gaps in quality of care and measure outcomes. • Benchmark physician and practice performance. Run reports that provide clinician- and practice-level results, plus national results and patient-level detail for PQRS measures. • Participate in quality reporting and incentive programs. Directly submit EHR data for the CMS PQRS program to avoid the PQRS penalty. Satisfy a menu requirement for Stage 2 of the EHR Incentive Program. • Help accelerate scientific discovery. Don’t sweat it— we’re here to help you. QCDR Reporting The IRIS Registry can help you report on quality measures in accordance with the latest regulations. The Academy has updated the IRIS Registry in accordance with the most current regulations and submitted the data required to meet new criteria in 2015, with no extra work on your part. • Report nine measures from the PQRS measures list. (If fewer than nine measures apply, report IRIS Registry QCDR measures too). • Successfully map your EHR system to the IRIS Registry by Aug. 1, 2016. • Include measures from three quality domains and at least two outcomes measures. • Submit your EHR data release consent form to send your information to CMS by Sept. 1, 2016. How to Participate in the IRIS Registry in 2015 If you have an EHR system, the easiest PQRS options are the automated extraction reporting mechanism for the IRIS Registry. However, the deadlines for EHR-based reporting have passed for 2015. (But if you sign up for 2016 participation now, you’ll be ahead of schedule for next year.) You still have a little time left to report PQRS manually (no EHR required) via the IRIS Registry Web portal. There are two options: Qualified Registry reporting or Qualified Clinical Data Registry reporting. Use of the Web portal is free for U.S. Academy members and their staff. Qualified Registry Reporting • Deadline to sign up is Oct. 31, 2015. • Report nine measures from the PQRS measures list. (If you report fewer than nine measures, you may be subject to a Measurable Applicability Validation review.) • Include measures from three quality domains and at least one cross-cutting measure. • Report each measure for at least 50% of the Medicare patients who you see this calendar year for whom the measure applies. • Deadline to sign up is Oct. 31, 2015. • Report each measure for at least 50% of all patients (including non-Medicare patients) you see this calendar year for whom the measure applies. To sign up for the Web portal, go to www. aao.org/irisregistry and select “Sign up” and then “IRIS Registry Application.” Timeline for EHR system-based participation • Submit your signed agreements by June 1, 2016. There are also options for participating in the IRIS Registry in 2016 if you do not have an EHR system. Data may be entered manually via the Web portal. For more information about manual entry options, visit www.aao.org/irisregistry. For more information about PQRS measures and EHR systems, visit www.aao.org/pqrs. Gear up to participate in the IRIS Registry in 2016 To get started: 1. Visit www.aao.org/irisregistry. Go to the “Join” box, and click on the yellow “Sign up” button. 2. Click on the “IRIS Registry Application” link, and fill out the short form. Tip: Have the names and Academy member ID numbers for all of the ophthalmologists in your practice. 3. We will send you a questionnaire about your EHR system and two agreements to review and sign. 4. Once you return the questionnaire and signed agreements, you will be placed on a reservation list. 5. Your practice will be sent an email for software installation to map your EHR data to the IRIS Registry. www.aao.org/store 23 Improve patients’ recall and understanding with rewritten and redesigned materials Did you know patients forget 80 percent of what you tell them?* Adding to your patient education challenge, almost one-half of American adults can’t read beyond an eighth-grade level. Age-Related Macular Degeneration (AMD) is a problem with your retina. It happens when a part of the retina called the macula is damaged. With AMD you lose your central vision. You cannot see fine details, whether you are looking at something close or far. But your peripheral (side) vision will still be normal. For instance, imagine you are looking at a clock with hands. With AMD, you might see the clock’s numbers but not the hands. Eye Words to Know Retina: Layer of nerve cells lining the back wall inside the eye. This layer senses light and sends signals to the brain so you can see. Drusen: Tiny white or yellow particles that form under the retina. They seldom cause vision loss. But many (or very large) drusen can be a sign of AMD. Cornea Lens Retina Pupil Glaucoma is a disease that damages your eye’s optic nerve. It usually happens when fluid builds up in the front part of your eye. That extra fluid increases the pressure in your eye, damaging the optic nerve. Glaucoma is a leading cause of blindness for people over 60 years old. But blindness from glaucoma can often be prevented with early treatment. With AMD, dark areas can appear in your central vision. Eye Words to Know Iris 2014–2015 Glaucoma Macula What is glaucoma? AMD is very common. It is a leading cause of vision loss in people 50 years or older. Features: Macula: Small but important area in the center of the retina. You need the macula to clearly see details of objects in front of you. nerve OpticOptic nerve: A nerve at the back of your eye that connects to your brain. The optic nerve sends light signals to your brain so you can see. Aqueous humor (“aqueous”): Clear liquid 1 of 5 inside the front part of our eyes. It nourishes the eye and keeps it inflated. (Aqueous is different from tears, which are outside the eye.) Drainage angle: The area of the eye where the aqueous humor drains from the front of the eye. Iris: The colored part of your eye. It controls the size of your pupil to let light into your eye. Features 87 topics in English and Spanish. Drainage angle Cornea • 51 topics listed in the preprinted brochure section (see page 25), plus 36 additional topics • PDFs that you can personalize (text only) • Regular updates to ensure you have the most current version Iris View samples and see all topics at www.aao.org/patienthandouts. In a healthy eye, excess fluid leaves the eye through the drainage angle, keeping pressure stable. What causes glaucoma? Your eye constantly makes aqueous humor. As new aqueous flows into your eye, the same amount should drain out. The fluid drains out through an area called the drainage angle. This process keeps pressure in the eye (called intraocular pressure or IOP) stable. But if the drainage angle is not working properly, fluid builds up. Pressure inside the eye rises, damaging the optic nerve. 12-month subscription Downloadable PDFs #PEHANDOUTS $299 member / $399 nonmember The optic nerve is made of more than a million tiny nerve fibers. It is like an electric cable made up of many small wires. As these nerve fibers die, you will develop blind spots in your vision. You may not notice these blind spots until most of your optic nerve fibers have died. If all of the fibers die, you will become blind.
