Coverage of Expenses for Diagnosis and
Treatment of the Disease
Reimbursement of 75%, up to NIS 10,000, for expenses associated with
diagnosis and treatment of the disease, such as medical consultation,
psychotherapy, complementary medicine, private nutritional
consultation, patient supervision, travel expenses and supportive aids.
Professional Patient and Family Guidance and
Consultation in Treating the Disease
A team that includes a nurse and social worker in collaboration with an
oncology consultant who assists with information and making specific
decisions at critical points of interface throughout all treatment stages at
NIS 500 at institutes in the arrangement plan.
* Within two years from diagnosis of the disease.
* The binding terms and qualification periods are according
to Mushlam Platinum Regulations.
New Coverage in the Platinum Plan
Didactic or Psychodidactic Evaluation
For children aged 6 to 18 at institutes in the arrangement plan.
At NIS 550 per didactic evaluation and NIS 950 per psychodidactic
evaluation. The benefit is for one test.
ADHD Evaluation
MOXO, BRC or TOVA tests at NIS 50 per test at institutes in the
arrangement plan.Two tests may be performed during the membership
period (including eligibility in the Mushlam Zahav plan).
Vaccinations for Overseas Travelers
Free consultation at clinics in the arrangement plan and a vaccination at a
cost of up to NIS 20.
Expanded Pregnancy Basket of up to NIS 5,000
The Platinum plan allows pregnant women to receive 75% reimbursement
of actual expenses, up to NIS 5,000, for consultations, tests and treatments
of the member's choice associated with pregnancy or birth, such as genetic
tests, second level ultrasound, private Caesarean section, labor coach
(doula), Verifi test, etc. The reimbursement is granted for all pregnancies
during the membership period.

Additional Coverage in the Platinum Plan
Dental Care
Preventive dental care for ages 12 to 18 free of charge and over 18 at 70% discount.
Orthodontic care under 18 at 60% discount and over 18 at 50% discount. Complex
treatments such as crowns, implants, bridges and dentures at 50% discount.
The dental care is provided at the Clalit Smile clinics.
Optics
Free optical glasses or contact lenses (non-cosmetic) for children under 18 at a value
of up to NIS 600 annually. The optical care is provided at the Clalit Optic stores.
Pregnancy Tests
The tests are provided at low prices at institutions in the arrangement plan. Second
level ultrasound at NIS 57 only. Amniocentesis / chorion villus sampling at NIS 285 only.
Convalescence for New Mothers
Contribution to convalesce expenses for new mothers at a convalescence institute/
facility for the first, second or third child. 75% of the cost and maximum NIS 309 per
day for up to three days for one child only.
Egg (Ova) Donation Overseas
75% reimbursement of the cost of the IVF process for ova donation overseas, up to
NIS 12,000, for the first and second child, for all the attempts for each child.
Private Surgery in Israel
Medical surgery at a private hospital by a private surgeon who has an agreement with
Clalit, at an especially low price of up to NIS 1,500 maximum for each surgery in the
agreement included in the list. Implants - up to 90% reimbursement of the actual cost
up to a maximum of US$ 6,000 per calendar year.
Private Surgery Overseas
Contribution to expenses for medical surgery overseas and related expenses (medical
airlifting, transplanted organs, travel expenses of an escort, etc.), by prior approval.
Cosmetic Surgery and Therapy
Exceptionally low prices at institutes in the arrangement plan.
The therapies offered include: hair removal, breast surgery, rhinoplasty, eyelid
surgery, hyperhidrosis therapy, etc. The treatments are provided at Clalit Estetika and
Proporzia clinics
Ambulance Transportation to ER
Eligibility for 50% reimbursement of the cost, up to NIS 301, twice per calendar year.
