Abstract Compared the efficacy of individual cognitive behavior therapy (CBT), systemic behavior family therapy (SBFT), and individual nondirective supportive treatment (NST) among 107 adolescents with Diagnostic and Statistical Manual of Mental Disorders-III-Revised (DSM-III-R) major depressive disorder. Ss were randomized to 1 of the 3 treatments; 37 Ss received CBT, 35 Ss received SBFT, and 35 Ss received NST. Outcome assessment included depression symptoms as measured by the Beck Depression Inventory, suicidality, functional impairment, and parent-rated treatment expectancy and credibility. CBT was more effective than SBFT or NST with respect to clinical recovery, rate of symptomatic improvement, and parent-rated treatment credibility. Patientens dysfunktionella beteenden och negativa självuppfattning minskas genom att terapeuten med s.k. Abstract A meta-analysis of child and adolescent psychotherapy outcome research tested previous findings using a new sample of 150 outcome studies and weighted least squares methods. The overall mean effect of therapy was positive and highly significant. Effects were more positive for behavioral than for nonbehavioral treatments, and samples of adolescent girls showed better outcomes than other Age * Gender groups. Paraprofessionals produced larger overall treatment effects than professional therapists or students, but professionals produced larger effects than paraprofessionals in treating overcontrolled problems (e.g., anxiety and depression). Results supported the specificity of treatment effects: Outcomes were stronger for the particular problems targeted in treatment than for problems not targeted. The findings shed new light on previous results and raise significant issues for future study. (2004). Fluoxetine, cognitive-behavioral therapy, and their combination for adolescents with depression: Treatment for Adolescents With Depression Study (TADS) randomized controlled trial. JAMA, 292, 807-820. Abstract CONTEXT: Initial treatment of major depressive disorder in adolescents may include cognitive-behavioral therapy (CBT) or a selective serotonin reuptake inhibitor (SSRI). However, little is known about their relative or combined effectiveness. OBJECTIVE: To evaluate the effectiveness of 4 treatments among adolescents with major depressive disorder. DESIGN, SETTING, AND PARTICIPANTS: Randomized controlled trial of a volunteer sample of 439 patients between the ages of 12 to 17 years with a primary Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, diagnosis of major depressive disorder. The trial was conducted at 13 US academic and community clinics between spring 2000 and summer 2003. INTERVENTIONS: Twelve weeks of (1) fluoxetine alone (10 to 40 mg/d), (2) CBT alone, (3) CBT with fluoxetine (10 to 40 mg/d), or (4) placebo (equivalent to 10 to 40 mg/d). Placebo and fluoxetine alone were administered double-blind; CBT alone and CBT with fluoxetine were administered unblinded. MAIN OUTCOME MEASURES: Children's Depression Rating Scale-Revised total score and, for responder analysis, a (dichotomized) Clinical Global Impressions improvement score. RESULTS: Compared with placebo, the combination of fluoxetine with CBT was statistically significant (P =.001) on the Children's Depression Rating Scale-Revised. Compared with fluoxetine alone (P =.02) and CBT alone (P =.01), treatment of fluoxetine with CBT was superior. Fluoxetine alone is a superior treatment to CBT alone (P =.01). Rates of response for fluoxetine with CBT were 71.0% (95% confidence interval [CI], 62%-80%); fluoxetine alone, 60.6% (95% CI, 51%-70%); CBT alone, 43.2% (95% CI, 34%-52%); and placebo, 34.8% (95% CI, 26%-44%). On the Clinical Global Impressions improvement responder analysis, the 2 fluoxetine-containing conditions were statistically superior to CBT and to placebo. Clinically significant suicidal thinking, which was present in 29% of the sample at baseline, improved significantly in all 4 treatment groups. Fluoxetine with CBT showed the greatest reduction (P =.02). Seven (1.6%) of 439 patients attempted suicide; there were no completed suicides. Abstract BACKGROUND: Psychotherapy is widely used for depressed adolescents, but evidence supporting its efficacy is sparse. METHODS: In a controlled, 12-week, clinical trial of Interpersonal Psychotherapy for Depressed Adolescents (IPT-A), 48 clinic-referred adolescents (aged 12-18 years) who met the criteria for DSM-III-R major depressive disorder were randomly assigned to either weekly IPT-A or clinical monitoring. Patients were seen biweekly by a "blind" independent evaluator to assess their symptoms, social functioning, and social problem-solving skills. Thirty-two of the 48 patients completed the protocol (21 IPT-A- assigned patients and 11 patients in the control group). RESULTS: Patients who received IPT-A reported a notably greater decrease in depressive symptoms and greater improvement in overall social functioning, functioning with friends, and specific problem-solving skills. In the intent-to-treat sample, 18 (75%) of 24 patients who received IPT-A compared with 11 patients (46%) in the control condition met recovery criterion (Hamilton Rating Scale for Depression score < or =6) at week 12. CONCLUSIONS: These preliminary findings support the feasibility, acceptability, and efficacy of 12 weeks of IPT-A in acutely depressed adolescents in reducing depressive symptoms and improving social functioning and interpersonal problem-solving skills. Because it is a small sample consisting largely of Latino, low socioeconomic status adolescents, further studies must be conducted with other adolescent populations to confirm the generalizability of the findings Rossello J., & G. Bernal, G. (1999). The efficacy of cognitive-behavioral and interpersonal treatments for depression in Puerto Rican adolescents. J.Consult.Clin.Psychol, 67, 734745. Abstract This study evaluated the efficacy of cognitive-behavioral therapy (CBT) and interpersonal psychotherapy (IPT) with depressed adolescents in Puerto Rico. Seventy-one adolescents meeting Diagnostic and Statistical Manual of Mental Disorders (3rd ed., rev.; American Psychiatric Association, 1987) criteria for a diagnosis of depression were randomly assigned to 1 of 3 conditions: CBT, IPT, or wait list (WL). Pretreatment, posttreatment, and 3-month follow-up measures of depression symptoms, self-esteem, social adjustment, family emotional involvement and criticism, and behavioral problems were completed. Results suggest that IPT and CBT significantly reduced depressive symptoms when compared with the WL condition. IPT was superior to the WL condition in increasing self-esteem and social adaptation. Clinical significance tests suggested that 82% of adolescents in IPT and 59% of those in CBT were functional after treatment. The results suggest that both IPT and CBT are efficacious treatments for depressed Puerto Rican adolescents. (2004). A randomized effectiveness trial of interpersonal psychotherapy for depressed adolescents. Arch Gen Psychiatry 61 (6):577-584, 2004. Abstract CONTEXT: Adolescent depression is highly prevalent and has substantial morbidity, including suicide attempts, school dropout, and substance abuse, but many depressed adolescents are untreated. The school-based health clinic offers the potential for accessible and efficient treatment, although it is unknown whether school-based clinicians can be trained to implement evidence-based psychotherapies for depression in routine care. OBJECTIVE: To assess the effectiveness of interpersonal psychotherapy modified for depressed adolescents (IPT-A) compared with treatment as usual (TAU) in school-based mental health clinics. DESIGN: A 16-week randomized clinical trial was conducted from April 1, 1999, through July 31, 2002. SETTING: Five school-based mental health clinics in New York City, NY.Patients Sixty-three adolescents referred for a mental health intake visit who met eligibility criteria. Eligible patients had a mean Hamilton Depression Rating Scale score of 18.6 (SD, 5.5) and a mean Children's Global Assessment Scale score of 52.6 (SD, 5.5) and met DSM-IV criteria for major depressive disorder, dysthymia, depression disorder not otherwise specified, or adjustment disorder with depressed mood. Mean age was 15.1 years (SD, 1.9 years). The sample was predominantly female (n = 53 [84%]), Hispanic (n = 45 [71%]), and of low socioeconomic status.Intervention Patients were randomly assigned to receive IPT-A (n = 34) or TAU (n = 29) from school-based health clinic clinicians. MAIN OUTCOME MEASURES: The Hamilton Depression Rating Scale, Beck Depression Inventory, Children's Global Assessment Scale, Clinical Global Impressions scale, and the Social Adjustment Scale-Self-Report. RESULTS: Adolescents treated with IPT-A compared with TAU showed greater symptom reduction and improvement in overall functioning. Analysis of covariance showed that compared with the TAU group, the IPT-A group showed significantly fewer clinician-reported depression symptoms on the Hamilton Depression Rating Scale (P =.04), significantly better functioning on the Children's Global Assessment Scale (P =.04), significantly better overall social functioning on the Social Adjustment Scale-Self-Report (P =.01), significantly greater clinical improvement (P =.03), and significantly greater decrease in clinical severity (P =.03) on the Clinical Global Impressions scale. CONCLUSIONS: Interpersonal psychotherapy delivered in school-based health clinics is an effective therapy for adolescent depression. Under 2003 framkom att paroxetin ökade suicidalt beteende jämfört med placebo. I strävandena att få en bättre inblick i sidoeffekterna vid (S)SRI begärde Amerikanska FDA fram "råmaterialet" från alla RCT som gjorts, både publicerade och opublicerade. När dessa sammanställdes framkom att enbart fluoxetin hade effekt vid depression hos barn och unga. Fluoxetine behandling påbörjas vanligen med 10 mg som sedan ökas var 3-7 dag beroende på patientens tålighet för biverkningar, stöd i familjen etc. Effekten sätter in efter ca 2-3 veckor på fulldos (vanligen 20-50 mg) per dag och kan, p.g.a. fluoxetinets långa halveringstid tas som endos. Behandlingstiden skall normalt överstiga den "längd" en egentlig depression brukar vara (ca 9 månader) och utsättning bör ej göras under höst- och vinterhalvåret men kan reduceras till en underhållsdos (ofta ca 20-30 mg). Behandlingen bör kombineras med KBT dels för att detta reducerar den ökade suicidaliteten under de första behandlingsveckorna och dels för att den enda effektstudien som gjorts (TADS) indikerar en förstärkt effekt av fluoxetine behandling vid samtidig KBT. SSRI kan genom aktivering/disinhibition hos ungdomar leda till ökad suicidalitet och hos barn till utagerande och riskabla beteenden. I enstaka fall kan man också få allmänt försämrad motivation. Uppföljning för bedömning av suicidrisk bör göras varje vecka den första behandlingsmånaden. Vissa belägg finns för minskad tillväxt vid SSRI behandling. Vid långtidsbehandling bör därför längd- och viktutveckling följas. Det finns risk för utveckling av mani vid SSRI behandling, i synnerhet om hereditet föreligger. Vid all SSRI behandling måste familjen informeras om biverkningar och vid återbesök måste man kontrollera för dessa. Barn under 8 år bör ej behandlas med SSRI. Graham J. Emslie, John Rush, Warren A. Abstract Evaluated the efficacy, safety, and tolerability of fluoxetine treatment for children with nonpsychotic major depressive disorders (NMDD). 96 outpatients (aged 7-17 yrs) meeting Diagnostic and Statistical Manual of Mental Disorders-III-Revised (DSM-III-R) criteria for NMDD participated in a randomized, double-blind, placebo-controlled study. 48 of the patients were randomized to fluoxetine treatment and the other 48 received placebo. The 2 major outcome measures were the Clinical Global Impressions (CGI) scale and the Children's Depression Rating Scale--Revised (CDRS--R) and were administered weekly throughout the 8-wk study. 27 Ss receiving fluoxetine and 16 receiving placebo were rated as "much" or "very much" improved according to the CGI. The CDRS--R scores were significantly lower for the fluoxetine group at week 5. Overall, results suggest that fluoxetine was significantly more effective than placebo in the acute phase treatment of NMDD, but complete remission was rare. Fluoxetine for acute treatment of depression in children and adolescents: a placebo-controlled, randomized clinical trial. J. Am.Acad.Child Adolesc.Psychiatry, 41,1205-1215. Abstract BACKGROUND: This report presents results from the acute treatment phase of a clinical trial designed to confirm efficacy of a fixed dose of 20 mg of fluoxetine in children and adolescents with major depressive disorder (MDD). METHOD: After a 3-week screening period, 122 children and 97 adolescents with MDD ( ) were randomly assigned to placebo or fluoxetine. After a 1-week placebo lead-in, fluoxetine-treated patients received fluoxetine 10 mg/day for 1 week, then fluoxetine 20 mg/day for 8 weeks. RESULTS: Fluoxetine was associated with greater mean improvement in Children's Depression Rating Scale-Revised (CDRS-R) score than placebo after 1 week ( <.05) and throughout the study period. Significantly more fluoxetine-treated patients (41%) met the prospectively defined criteria for remission than did placebo-treated patients (20%) ( <.01). More fluoxetine- (65%) than placebo-treated (53%) patients met the prospectively defined response criterion of > or =30% decrease in CDRS-R score, but this difference was not significant ( =.093). Significantly more fluoxetine-than placebo-treated patients completed acute treatment ( =.001). There were no significant differences between treatment groups in discontinuations due to adverse events ( =.408). CONCLUSION: Fluoxetine 20 mg daily appears to be well tolerated and effective for acute treatment of MDD in child and adolescent outpatients. ECT har inte prövats i RCT men det finns många fallbeskrivningar och öppna studier. Dessa har granskats i en metaanalys som indikerar att ECT är en både efffektiv och säker behandlingsmetod. Effektstorleken låg väl i linje med den man sett i vuxenstudier. Effekten sätter in efter att flera ECT givits och har stabila resultat om en full serie genomförs. 75 % av patienterna uppges bli förbättrade. Effekten är snabbare och kraftigare än vid läkemedelsbehandling. 90 % av patienter med djupa depressioner svarar. Biverkningar fr.a. minnesproblem har varit milda. Ghaziuddin, N., Kutcher, S. Abstract Electroconvulsive therapy (ECT) may be an effective treatment for adolescents with severe mood disorders and other Axis I psychiatric disorders when more conservative treatments have been unsuccessful. ECT may be considered when there is a lack of response to two or more trials of pharmacotherapy or when the severity of symptoms precludes waiting for a response to pharmacological treatment. The literature on ECT in adolescents, including studies and case reports, was reviewed and then integrated into clinically relevant guidelines for practitioners. Mood disorders have a high rate of response to ECT (75-100%), whereas psychotic disorders have a lower response rate (50-60%). Consent of the adolescent's legal guardian is mandatory, and the patient's consent or assent should be obtained. State legal guidelines and institutional guidelines must be followed. ECT techniques associated with the fewest adverse effects and greatest efficacy should be used. The presence of comorbid psychiatric disorder is not a contraindication.
