- TROCHANTERIC BURSITIS o Symptoms – pain over the greater trochanter (lateral hip), radiating to the buttock; pain increased when the patient climbs stairs o Causes – (Acute) – direct blow to the greater trochanter (Chronic) – irritation from the IT band passing over the bursa o Predispositions – increased Q angles (see knee, this also causes overuse forces to be placed on the greater trochanter), leg length discrepancy, increased bike riding and running may also contribute to this injury. (1) - ISCHIAL BURSITIS o Symptoms – pain over the ischial tuberosity, in the vicinity of the gluteal fold o Causes – (Acute) – direct blow to the ischial tuberosity such as falling on it (Chronic) – repeated shifting and moving while weight bearing in the seated position (rowing) o Predispositions – tightness of the hamstring muscle group, prolonged sitting and rocking, especially on a hard surface (1) - Piriformis Syndrome o Symptoms – pain in the deep posterior aspect of the hip, radiating into the buttock and possibly down into the posterior aspect of the leg; increases upon standing and often decreases with the patient lying supine and knees flexed o Causes – blow to the buttocks, hyperinternal rotation of the femur, other trauma that causes spasm of the piriformis muscle o Predispositions – anatomical deviation in which the sciatic nerve passes through the piriformis muscle; females have an increases incidence of acquiring piriformis syndrome (1) - Iliac Crest Contusion (Hip Pointer) o Symptoms – pain at the iliac crest, possibly radiating into the internal and external oblique muscles o Causes – blunt trauma o Predispositions – unprotected ilium (1)
