Peake Awards Ballot Submission Instructions
Categories
There are seven categories on the ballot. (Four Supplier Awards categories and three Distributor
Awards categories) Choose one winner in as many categories as you wish.
Eligibility to Vote
One ballot per CPPA Member Company will be accepted. Votes may be submitted via this ballot
or using the online ballot.
Deadline
All ballots must be received by 5:00pm (EST) on Friday, January 16, 2015.
Completing The Ballot
If Using Adobe Acrobat: Click on the boxes to make your selection. The completed form can be
saved to your computer and emailed only if you have Adobe Acrobat. Use the ‘File’ drop-down
menu to locate the ‘Save As” command, RENAME the file, and then save to your computer.
If you are not able to save changes to the form, then you may print and fax the ballot or complete
the online ballot here.
