Dear Colleagues,
At Inova, we have a goal not just to provide today’s healthcare, but
to be the future of health. This means many things to us—genomics,
personalized medicine, disease prevention, research, educating the
physicians and clinicians of tomorrow, and constantly challenging
ourselves to better the current practice of medicine.
The future of health is an even more poignant concept when Inova
Children’s Hospital is concerned. Our patients quite literally, are the
future of the Washington Metropolitan Region. They are the sons
and daughters of our community, and the care that we provide must
be nothing less than extraordinary.
Over the past year, the Inova Children’s Heart Program has made
many advancements. As you will read in this report*, we have added
a single ventricle clinic, launched a specialized critical care pediatric
ambulance, provided more comprehensive care through the
addition of a n inpatient Nurse Practitioner to coordinate complex
care, and mended the hearts of hundreds of children so that they
might get back to doing what they do best--being kids.
2012
In September, work began on a new home for the Inova Children’s
Hospital. A state-of-the-art 650,000 square-foot space dedicated to
caring for the women and children of our community.

Recent Achievements
■ The Melody Transcatheter Pulmonary Valves are now an option for children and
adults with congenital heart defects. Children and adults now have a minimally
invasive treatment option for pulmonary valve replacement, saving them from
facing difficult open heart surgery.
Pediatric Cardiothoracic CME
■ Inova Children’s Hospital joined the National Pediatric Cardiology Quality
Improvement Collaborative—a group that is currently focused on improving care for
patients with hypoplastic left heart syndrome.
■ Neonatology and Pediatric Cardiology worked together to develop a specific
algorithm of care for premature infants with patent ductus arteriosus (PDA).
This algorithm guides physicians in determining which children are best candidates
for surgical ligation.
Melody Valve
■ The 3nd Annual Pediatric Cardiothoracic CME Conference on the topic of
Congenital Heart Disease: Maternal Fetal Medicine was held in September 2011.
This event is made possible by the Shapiro Heart Education Fund.
■ The Inova Children’s Hospital Heart Program was joined by Meghan Edwards,
FNP-BC, who coordinates patient care between cardiology, cardiac surgery,
and other disciplines involved in the highly-complex care that pediatric heart
patients receive. Shelly Fleischer, PA, has also joined the team. Shelly leads the
program’s Single Ventricle Clinic, which provides continuing care for children with
hypoplastic left heart syndrome or other forms of single ventricle physiology.

■ Tom Hougen, MD, led a team including Lucas Collazo, MD, James Konigsberg, MD,
Ruben Munoz, CPA, Russell D’Sa, RCIS/RCVT and clinical nurse specialist Beth
Suddaby on a medical mission to Ecuador. The team was able to perform
life-saving cardiac surgery and interventional cardiac catheterizations on children
with little or no resources or access to care.
■ The Neonatal Intensive Care Unit was the first in the nation to receive the Joint
Commission’s Gold Seal of Approval for the specific care of newborns needing
specialized care.
■ The ECMO Program held its first on-sight training program for ECMO specialists to
prepare nurses and respiratory therapists in this role.

Cardiac Imaging
The basis of diagnosis and
treatment of congenital heart
disease is the use of imaging
techniques to visualize anatomy
and understand the unique
physiology of each congenital
patient. Inova Children’s Hospital
uses all possible modalities
to obtain the most information
about the patient’s problems,
in order to make the best
decisions for their care,
including:
■ MRI
■ Low-radiation cardiac CT
■ Echocardiography, including
transesophageal and 3D imaging
■ Cardiac catheterization.
Improvements in non-invasive imaging
such as echo, TEE, 3D echo, MRI, and CT
have significantly reduced the need for
invasive diagnostic catheterizations.
At the same time, newer techniques
have evolved allowing transcatheter
interventions to treat certain heart
abnormalities, sparing many children
from the need for heart surgery.

The cardiac surgery program provides highquality care with low surgical mortality. In
2011, our cardiac surgery team performed 276
pediatric and 43 adult congenital surgeries with
a 98.6% survival. We participate in the Society
of Thoracic Surgeons Congenital Heart Surgery
National Database in order to compare our
results to those across the nation. It is difficult
to benchmark pediatric outcomes because of
the variety of types of congenital surgery.
Coarctation of Aorta
Inova Children’s Hospital takes on the most
difficult cases and is advancing the field using new techniques.
Our surgical team includes dedicated pediatric cardiac anesthesiologists with
the assistance of cardiac nurse anesthetists, cardiac perfusionists and cardiac
operating room nurses, in addition to pediatric cardiac surgeons and surgical
physician assistants. Once our patients leave the operating room, they are cared
for in the pediatric intensive care unit. This multidisciplinary collaboration results
in the best care possible for children with congenital defects. That collaboration
continues as the patient advances toward home with the surgical team, cardiology
and our inpatient nurse practitioner guiding care.
Pediatric Cardiac Anesthesia
At the Inova Children’s Heart Program, pediatric sub-specialists read cardiac
imaging, provide intensive care in the NICU or PICU, perform cardiac surgery
and provide cardiovascular care. This same sub-specialization occurs in the
area of pediatric cardiac anesthesia. Inova Children’s Hospital’s Department of
Anesthesiology includes physicians specially trained in caring for the tiniest of
patients, particularly those with heart conditions.
The team provides comprehensive anesthesia services for children in the
operating rooms, catheterization and electrophysiology labs, and other settings,
including providing sedation for imaging.

Cardiac Catheterization Volume 2007-2011
Interventional Cardiology
Our pediatric interventional cardiology team performed
234 cardiac catheterizations in 2011. Interventional
catheterization cases totaled 130 and diagnostic cases
totaled 104. In order to benchmark our catheterization outcomes,
we participate in Improving Pediatric and Adult Congenital
Treatments (IMPACT), an arm of the National Cardiovascular Data
Registry of the American College of Cardiology.
Inova Children’s Hospital and the Inova Heart and Vascular Institute
were early adopters of the Melody Transcatheter Pulmonary Valve.
In many cases, this pulmonary valve replacement spares children
and adults the need for open heart surgery.
We support children undergoing catheterization with just-in-time
preparation on the day of the procedure by our cardiac child
life specialist, making our program child-friendly as well as
technically expert.

to identifying and treating these problems.

Adult Congenital Heart Clinic
One of the unique aspects of our program is the ability to provide congenital cardiac care
from before birth through adulthood. Statistically, more adults than children are in need
of congenital heart treatment, the adult congenital heart disease program has become
a major focus of the Inova Heart and Vascular Institute. We believe it is critical that the
care of these patients transition to adult care while continuing to be treated by experts in
congenital heart disease. At the Inova Heart and Vascular Institute they can receive this
expertise within an environment that specializes in adult cardiac care.
Our Adult Congenital Heart Clinic consists of congenital surgeons, pediatric cardiologists
and adult cardiologists who manage the care of these patients throughout their lives.
Because this is a service shared by experts of the Inova Children’s Hospital and the Inova
Heart and Vascular Institute, we are able to provide care through every stage of a child’s
life, from before their birth and into their seventies, eighties and beyond.

Adult Congenital Catheterization
In 2011, the cardiac catheterization team performed 88 catheterizations for adult congenital
heart disease. We continue to close atrial septal defects for adults at risk for strokes or
transient ischemic attacks. Other unique procedures include closing perivalvar leaks,
particularly of the mitral valve, and mitral valve dilations.

Caring for the Caregiver
As a pediatric medical-surgical nurse at Inova Children’s Hospital, Catie
Ensz spends her days taking care of very sick children. But when her baby
Josiah faced a series of life-threatening medical challenges, it was her
coworkers’ turn to take care of mother and son.
While Catie was pregnant, Josiah was diagnosed with transposition of the
great arteries (TGA), a rare and complex congenital heart defect in which
the aorta and pulmonary artery are reversed. He would need open heart
surgery at birth.
Catie and her husband put themselves in the capable hands of her
colleagues in the Pediatric Cardiac Program. Patient navigator Kelly Gallo
guided them through the medical maze of subspecialists and treatment
options, providing critical education and support. “Kelly was our
advocate and lifesaver,” Catie says. “It was such a relief to have
someone else to answer the medical questions and help make
decisions so I could just be mom.”

Inova Fetal Care Program
When Josiah was born at 37 weeks,
pediatric cardiac surgeons Irving Shen,
MD, and Lucas Collazo, MD, performed
an arterial switch, moving the aorta and
pulmonary artery to their normal positions.
The fragile newborn was placed on a
bypass machine while the surgeons
repaired his tiny heart. Afterwards, he spent
eight days in the PICU with his chest open
because he was too small to close. All told,
Josiah spent 37 days at Inova Children’s
Hospital.
Catie continued to rely on her colleagues
in the months following Josiah’s surgery.
PICU nurses pulled him through some
serious post-operative complications, the
Cardiac Feeding Team provided nutritional
guidance, and occupational and speech
therapists helped with developmental
issues. Josiah is now a thriving 17-monthold, fully caught up to his peers in all areas.
“In the end, probably more than 100 of my
coworkers touched my son, and we are so
grateful to every one of them,” says Catie.
“This is an amazing program and we feel
lucky to have this kind of care.”
Using a multidisciplinary approach of specialty physicians
and nurses, the Inova Fetal Care Program provides prenatal
consultation and pediatric care planning for pregnant women
identified as carrying babies with fetal heart abnormalities, birth
defects or medical problems. The Program provides the advantage
of high-risk pregnancy expertise available at Inova Women’s
Hospital with our pediatric surgical and medical specialties.
When problems are identified, all of our surgical and medical
specialties are available for prenatal consultation, coordinated by
our patient navigator, Kelly Gallo, RN, who helps the expectant
mother throughout the entire process of her hospital visit, delivery
and surgery. Because the fetal care, birth and pediatric care
can all occur in the same setting, the family does not have to be
separated from their baby at this critical time.
In 2011, Inova Fetal Care Program diagnosed fetal heart defects in
more than 36 expectant moms and facilitated their delivery and
ongoing care of their child.

Single Ventricle Clinic
In response to our growing population of infants with hypoplastic
left heart syndrome or other forms of single ventricle physiology,
the Inova Children’s Hospital has created a specialty clinic
that focuses on the care of these fragile infants. Using a
multidisciplinary approach, children receive cardiology care in
concert with nutrition consultation and speech therapy so vital
to their growth and development. The Single Ventricle Clinic is
our newest endeavor to promote the best outcomes in the most
challenging of cases.

Experts in Children’s Care
At Inova Children’s Hospital, our commitment is to provide
world-class care in a child-friendly environment, close to
home. This includes the Northern Virginia’s only Pediatric
Intensive Care Unit (PICU) and the region’s most advanced
Neonatal Intensive Care Unit (NICU).
Inova Children’s Hospital’s 75-bed, level III C neonatal
intensive care unit (NICU) provides specialized care and
observation for the most vulnerable, high-risk infants. Inova
Children’s Hospital is home to Northern Virginia’s largest
subspecialty NICU, with a highly trained, multi-disciplinary
team of board-certified neonatologists, neonatal nurse practitioners, respiratory therapists
and lactation consultants on staff around the clock.
In 2011, the NICU was the first in the nation to receive The Joint Commission’s Gold Seal
of Approval™ for Prematurity. This prestigious certification represents the highest level of
care for premature infants. It recognizes the team’s commitment to continuously improve
performance around treatment protocols that reduce complications and improve longterm quality of life.
As the only Pediatric Intensive Care Unit in the region, the unit is a vital asset to Inova
Children’s Hospital and the families of Northern Virginia. The PICU provides critical care
to children with life-threatening illness, pediatric trauma, and of course the children of the
Inova Children’s Heart Program. Pediatric intensivists, pediatric critical care nurses, and
clinicians work with pediatric cardiac surgeons, and pediatric cardiologists to ensure that
children and adolescents with cardiovascular concerns receive outstanding care. In 2012,
the unit was recognized by the American Association of Critical Care Nurses with the Gold
Beacon Award for the outstanding care that they provide.
At Inova Children’s Hospital we offer certified Child Life specialists, experts in child
development, who promote effective coping through play, procedural preparation,
education and self-expression activities. They provide emotional support for families,
and encourage optimum development of children facing a broad range of challenging
experiences, particularly those related to healthcare and hospitalization.

Funding for the Future
As a not-for-profit organization, our commitment to world-class pediatric care is made
possible through the generous support of donors. Each year, several events take place to
raise funds for our children’s heart program.
■ In June 2011, the Bob and Lynda Gibbs Family hosted the 8th Annual Golfing for
Grace Tournament. The event raised more than $26,000 to support the heart
program. Over the past eight years this event has raised nearly $240,000 for the
Inova Children’s Hospital Heart Program.
■ On October 7, 2011, Dell Services Federal Division employees, vendors and friends
participated in their corporate golf tournament to support the pediatric cardiology
program. This is the third year that Dell has selected the Inova Children’s Hospital
Heart Program as their “Charity of Choice”.
■ The Star Kid 8K Race and Family Fun Run took a brief hiatus in 2011, but was back
in action in 2012. A record number of walkers, runners, and volunteers braved
the rain on April 22 to support the race. The event raised more than $130,000 to
support the children’s heart program and the Akl Endowment. Pediatric heart funds
such as these were most recently used to purchase pediatric specific surgical
instruments. In addition these funds have also allowed for our medical staff to
attend national conferences and meetings regarding children’s cardiology care.
■ In September 2011, we hosted the 3rd Annual Shapiro Lecture. The title of the
2011 conference was Congenital Heart Defects: Maternal Fetal Medicine. Experts
from around the country gathered to discuss leading techniques, research and
diagnoses related to maternal fetal medicine related to congenital heart disease.
The conference was made possible by the Shapiro Heart Education Fund,
established by a grateful family to honor Dr. Shapiro’s legacy and commitment
to helping children with heart issues.

Specialized Transport for the
Region’s Sickest Children
At Inova Children’s Hospital, we understand that sick infants
and children need special care. We also understand that
our region’s sickest children need specialized transport
to our hospital. One Call transfer system coordinates
Inova Children’s Hospital’s critical care pediatric
ambulance, PHI Aircare, and the transfer and intake
process for the region’s sickest children. It takes just
One Call to 877.900.9KID (9543) to transfer a child to
Inova Children’s Hospital.
The state-of-the-art mobile intensive care unit is available
24-hours a day, year round, for the children of our
community. The ambulance is able to provide care from the
most basic to the highly advanced with its dedicated team
of critical care medics and EMTs—with nurse practitioners,
neonatal and critical care pediatric nurses, and respiratory
therapists available for patients needing a higher level
of care. In addition to its highly trained team, the unit is
outfitted with leading-edge equipment, including invasive
and non-invasive cardio-respiratory monitoring, ventilators,
emergency medications, chest drainage systems, and the
capability to use nitric oxide and heli-ox if needed to
provide safe transport of the patient.
PHI Aircare transports patients by helicopter to Inova
Children’s Hospital from within a 200 mile radius.
The helicopters, equipped as flying emergency intensive
care units, contain monitoring equipment, ventilators and
defibrillators, allowing nurses and paramedics to begin
treatment while in route to the hospital.

The Future of Children’s Care in Northern Virginia
Inova Children’s Hospital and Inova Women’s Hospital officially broke
ground in 2012 for a state-of-the-art, 12-story, and 665,000-squarefoot space dedicated to the health of women and children in Northern
Virginia and the greater Washington metropolitan region. The hospital
