READY TO QUIT
My Stop Smoking Checklist
I am ready to stop smoking
I’ve written down my reasons to quit
I’ve thought about when I might want to
smoke and how I can avoid giving in
I’ve asked my family and friends for help
I’ve picked a quit date
I know that quit medications can help
I’m going to ask my healthcare provider
for help
Quit smoking medications can help you quit and stay a non-smoker*
Your health care provider might suggest:
• Nicotine replacement therapy: Products like patches or gum that provide nicotine in a less dangerous
way than commercial tobacco to reduce craving and withdrawal symptoms
• Zyban (bupropion): Tablets that reduce withdrawal symptoms
• Champix (varenicline): Tablets that reduce cravings and the pleasure of smoking
