Job Title: Locum Appointment for Service (LAS) Departments: Colorectal Surgery/Pelvic Surgical Oncology Division: Cancer Centre Services Base: The Christie NHS Foundation Trust ORGANISATIONAL ARRANGEMENTS Accountable to: Miss S T O’Dwyer, Divisional Director Mr M S Wilson, Clinical Director Joint Lead Clinicians - Peritoneal Tumour Service Job Description This job description outlines the principal clinical duties of the appointee and the support that will be available, including the current facilities and staff at The Christie. Duties and Responsibilities This will be a post for covering an existing North Western Deanery training slot, that will be vacant from 1st April 2010 to 3rd August 2010 participating in the activities of the Peritoneal Tumour Service and the Pelvic Surgical Oncology Team. This post provides training opportunities and exposure to peritoneal surgery and advanced pelvic surgery. The successful applicant would be in a position to gain clinical experience in the management of peritoneal surface tumours including pseudomyxoma peritoneii and peritoneal carcinomatosis, complex abdomino-pelvic malignancy and anal cancer. In addition there are facilities for engaging in clinical research. It is possible that the successful applicant could take part in the unit’s programme and submit presentations and publications to the appropriate specialty associations and journals. This post is applicable to surgical trainees who have a minimum of 2 years of general surgical experience. The post holder will be provided with suitable office accommodation and will receive IT support including a computer with internet access and e-mail facilities. The hospital has excellent library facilities offering a full range of services including books, journals and facilities for personal and commissioned computer on line searches. The medical illustration department is equipped with modern facilities for computer generated graphics and a professional medical artist to assist in the preparation of material for teaching and publication. Responsibilities will include: Pre and post-op ward rounds Clinical administration of MDT meetings Co-ordination and administration of (HIPEC) Hyperthermic Intraperitoneal Chemotherapy (training will be given) Liaison with the Service Manager and Clinical Nurse Specialist Assist with and perform surgical procedures Peritoneal Tumour Service (PTS) and Colorectal out patient clinics Clinical Research: co-ordinate specimen collection and processing, supervise data collection and audit. Prepare papers for publication Partake in on-call surgical rota with non resident night duties at a 1b, EWTD compliant level. (The Trust does not have an accident and emergency department and hence the out of hours service is limited to the surgical support for oncology patients undergoing elective in patient treatment). THE Christie NHS Foundation Trust The Christie Hospital and Holt Radium Institute were united in 1932 and together have grown to become the largest cancer hospital in the United Kingdom with registration of more than 12,000 new patients annually. The Christie is a designated teaching hospital in the School of Cancer Studies of the University of Manchester. The Hospital gained Foundation Trust status in April 2007. The Foundation Trust is committed to taking a leadership role in service provision, research and teaching as the Cancer Centre for the Greater Manchester and North Cheshire Cancer Network (GMNCCN). The Trust is the first UK institute to gain membership of the Organisation of European Cancer Institutes (OECI). The operational divisions reflect the integration of service, research and teaching: Clinical Divisions include Cancer Centre Services, (incorporating surgery, anaesthetics and critical care, Director Miss S T O’Dwyer); Networked Services (Director Jason Dawson); and Support Services (Director Dr Jeremy Lawrance). In addition there is a Division of Research and Development (Director Professor J Radford) and a Division of Education (Director Dr R Cowan). All work in close collaboration providing comprehensive services for patients in an environment encouraging training and innovation. Clinical Departments at The Christie The approach to treatment at The Christie is through the multi-disciplinary team within tumour-specific and disease-orientated groups. All the key clinical specialties are available including surgical, medical and clinical oncology, diagnostic and interventional radiological services (Director – Dr Hans-Ulrich Laasch) including ultrasound, nuclear medicine, angiography, CT, MR and PET scanning. Additional clinical groups include Paediatric and Adolescent Oncology, Palliative Care, Cancer Genetics Endocrinology, Psychological Medicine and Complimentary therapies. The Oncologists provide outreach clinics to the majority of teaching and district hospitals in Greater Manchester that in turn generates the need for specialist tertiary referral to the cancer centre. Out patient, ambulatory and in patient services are provided at The Christie site, with a dedicated day case surgical facility and short stay facilities for chemo-radiotherapy. Of the 350 in patient beds at the hospital 32 are assigned to Surgery. Beds are allocated as needed but a dedicated critical care bed is assigned to support patients in the PTS. An eight bed Critical Care Unit was opened in December 2006 with an additional 4 bed surgical step-down unit supported by the Critical Care outreach team. In addition the acute and chronic pain team assist with a perioperative pain service. Department of Surgery There are currently 16 Consultant Surgeons with variable sessional commitments. A second generation DaVinci robot has been installed in Jan 2008 as the first phase of developing a new dedicated minimally invasive operating facility. Consultant Surgeons and Anaesthetists are members of the Directorate of Surgery (Director Mr Malcolm Wilson) within the division of Cancer Centre Services. The Peritoneal Tumour Service (PTS) The PTS was set up in November 2002 as one of two centres in the UK with funding from the National Commissioning Group (NCG). Over 300 patients have been evaluated to date from which 145 have undergone surgery. An expansion of the service over the next three years has been agreed to undertake 60 procedures per year by 2011. Initially the service concentrated on pseudomyxoma peritonei and appendiceal carcinomatosis but recently the treatment has been extended to include patients with colorectal cancer and other primary peritoneal tumours (eg mesothelioma). Extending the service to meet the NICE guidelines for peritoneal carcinomatosis (IPG 116 2004) is an objective for the Department. Mr Andrew Renehan supervises an active research programme with clinical and laboratory based projects, including a tissue bank of peritoneal tumours. The team have recently completed the first ever trial of adjuvant chemotherapy with good response rates (submitted JCO). PTS Team members The unit believes strongly in the multi-professional approach dealing with these difficult problems by offering multi-specialist services supporting patients through their entire treatment pathway. The specialist team provides a coherent, multidisciplinary approach for patients with advanced colorectal, gynaecological, urological and other rare pelvic and retroperitoneal tumours. Patients are referred from the Northwest of England, North Wales, North Midlands and on occasion from other areas of the United Kingdom. Management is streamlined through a weekly MDT meeting incorporating surgical and non-surgical oncologists, palliative care specialists, radiologists and pathologists. The Histopathology department provides a tertiary review service for the evaluation of rare tumours and difficult diagnostic problems. The unit hosts the Anal Cancer MDT for GMNCCN and coordinates the Supra regional audit for anal cancers. The team has the largest experience of salvage surgery for anal tumours in the UK with five year cancer related survival of 40%. Non Consultant Staff The team is supported by two second year Foundation posts, a junior Clinical Fellow and three Specialist Registrars on Colorectal, Gynaecology, Urology and plastic surgical sub specialty training programmes. There is a Senior Colorectal Nurse Practitioner, a Nurse Endoscopist, 4 Stoma Therapists a clinical nurse specialist for PTS and an Advanced Nurse Practitioner in Colorectal surgery. Research Research at The Christie is encouraged with a Divisional Director for Research (Prof. J Radford) supported by a Research & Development Manager. Cancer research is a strong feature in the University of Manchester with Cancer Studies achieving 5 stars at the last Research Assessment Exercise. The Trust has been supported by Cancer Research UK and the University of Manchester to develop a new Manchester Comprehensive Cancer Centre. The Paterson Institute for Cancer Research, adjacent to The Christie, is core funded by Cancer Research UK (Director Professor Nic Jones) and houses a wide range of laboratory/translational research. Within the Department of colorectal and pelvic surgical oncology clinical and laboratory research is directed towards prevention, diagnosis and treatment of colorectal, anal and peritoneal malignancies. There is an established programme supervising research fellows to degrees by thesis. Six fellows have achieved one PhD, three MDs, with a further two in preparation over the last 5 years. The Research Lead is Mr Andrew Renehan who holds monthly meetings and co-ordinates and designates projects for all staff in the PTS and Colorectal Surgical Service. It is expected that the appointee will contribute to the programme. Teaching The Trust is a designated teaching hospital and the surgical department plays an active role in undergraduate and postgraduate teaching supporting the University of Manchester Medical School. It is expected that the appointee would engage with teaching within the Trust as deemed appropriate and would be encouraged to participate in a wide range of educational activities. The Area Close to the trust there are many attractive residential areas and within easy reach of the beautiful countryside of Cheshire and North Derbyshire the Lake District, Snowdonia, and the Yorkshire Dales. There are many good schools in the area and there are excellent road, rail and air links (Manchester International destinations.
