The Daily Activity Sheet (DAS) is to be used by the student intern to record the dates and hours
while participating in the internship activity. The DAS must be submitted to the Internship
Coordinator BEFORE the end of the semester. Failure to do so may result in a grade of “I” or
“NC”. It is strongly recommended that the intern make a copy of the completed DAS for his/her
own records.
