Hospice at Home Team Manager
Services: To work directly in the patient’s home; either by shift work or roving visits. Occasional
coordination responsibilities (as required), working with partners from other organisations.
Job Summary/Main Purpose – Under the leadership of the team manager
1. To assess and care for patients who wish to receive end of life care in their homes.
2. To support Health Care Assistants (HCA) lone working staff with more complex patient care.
3. To work a variety of shift patterns and length of shift dependent on patient and staff
requirement.
4. To develop effective working relationships with all internal and external allied Health Care
Professionals; providing care and advice in relation to end of life and palliative care in the home.
5. Work collaboratively with the Hospice at Home Manager in liaison with the Community Nurse
Specialist (CNS) team in working hours, and with the advice line/ doctor on call out of hours.
6. To provide holistic assessments to patients, their families and carers and to plan for a package
of care via Hospice at Home (HAH), by liaising with the co-ordinator.
7. Clearly document all care given and plan of care.
8. Coordinate the service when necessary and organising the Hospice at Home services to meet
the needs of the patient
9. To provide leadership, mentoring and support to HCAs and Band 5 staff
10. To support lone working staff and be part of the on call for Hospice at Home

Principal Duties and Responsibilities
Leadership and Management.
1 To develop effective working relationships with external allied health care professionals, other
domiciliary care providers and external partners to ensure the patient and family receive well
coordinated care of a high quality.
2 To develop effective working relationships with other members of the Hospice multi- disciplinary
team.
3 When required - to plan and coordinate the Hospice at Home Service with the support of the
Hospice at Home Manager; developing rota’s of care for families in need and in collaboration with
other service providers.
4 To participate in the team out of hours rota in conjunction with other St Peters Community Team
members.
5 To regularly attend and participate in team and organisational meetings to improve
communication, representation and service provision/development.
6 To be included in decisions regarding development or review of organisational policies, standards
and procedures.
7 Under the guidance of the HAH Team Manager, take part in the recruitment of new staff by
contributing to the shortlisting, interviewing and selection, using agreed methods and based on
objective assessments against agreed selection criteria.
8 To contribute to data collection that feeds contractual obligations, standards and KPI’s.
9 Able to communicate effectively with other professionals including GP’s and District Nurses to
ensure that high quality palliative care is available to patients
10 To provide leadership, mentoring and support to health care assistants and band 5 staff
Clinical
1. To assess, plan, implement, document and evaluate palliative care/end of life care provision for
patients who want to be cared for in their own homes.
2. To provide high quality direct nursing care.
3. To verify the expected death of a patient
4. To support the HCAs in the patient’s home to ensure they are providing a high level of nursing care.
5. To liaise with district nurses and general practitioners to ensure the correct equipment, support and
medication is provided for the patient to meet their end of life needs whilst avoiding an out of
hours crisis.
6. To work 3 planned shifts a year on the In Patient Unit to maintain/ refresh clinical skills and
knowledge.
7. To provide immediate bereavement support for families and carers following the death of the
patient and referring to the PSS team as appropriate.
8. Support the Primary Health Care Teams at times of high workload e.g. assist with specific
medications and review need
9. To act as a conduit of communication with the CNS team.
10. To participate in effective multi-disciplinary working in the community
11. To adhere to the NMC Code for Professional Standards
12. To mentor Band 3 and 5 staff, including induction.
13. To liaise with staff on the IPU to support discharged back to the patients home
14. To be physically able to support the manual handling and physical care needs of patients in their
home environment
Quality
1. To respond to all referrals effectively and within agreed standards, in conjunction with care
providers and St Peters Hospice Access Team.
2. To contribute to established Quality Assurance Programmes including clinical audit, implementing
recommended changes from feedback and audit so ensuring evidence based, quality specialist
palliative care is being delivered to patients, carers and their families.
3. To work within SPH policy and procedures and ensure Hospice at Home standards are met and
adhered to.
4. To ensure all statutory and mandatory training is undertaken and adhered to.
5. To participate and contribute to personal Individual Performance Review, and deliver the same for
the band 3 HCA staff and Band 5 RN staff
6. To identify areas of clinical risk and with the line manager- develop plans to eliminate or reduce to a
safe level.
7. To support the Hospice at Home Manager with departmental risk and COSHH assessments
8. To attend clinical supervision as provided for under hospice policy
Professional Development
1. Contribute to the training and learning needs of the HCA and professional placements as required
by SPH.
2. To be a mentor/ assessor for the Health Care Assistants and support the education department in
the QCF programme
3. To maintain clinical competencies via internal and external training and development
4. To participate and contribute to personal Individual Performance Review
5. To take responsibility for your own NMC Revalidation process and support band 5 staff in meeting
their requirements.
General – The post holder will
 To provide cover for the Hospice at Home Team Manager as required and appropriate.
 Have access to confidential data on patients and services within St Peters Hospice. Failure to
maintain confidentiality will lead to disciplinary action, which could ultimately lead to dismissal.
 Be required to participate in the assessment of risk and thereby contribute towards clinical and
corporate governance agenda as appropriate.
 Be expected to produce work of a high standard and to promote quality at all times.
 Uphold all the St Peters Hospice policies and uphold the commitment to Equal Opportunities for all
present and potential members of staff and patients.
 Ensure that all employees, within the HAH team, understand, support and apply the Equal
Opportunities policy through their working practice; this requires all individuals to be treated with
respect, dignity, courtesy, fairness and consideration.
 Be expected to keep updated on all matters relating to Hospice procedure and policy.
 Familiarise themselves with matters relating to health and safety management as they affect them
personally and/or the Hospice, reporting any potential risks to life or property immediately in
accordance with the Hospice’s Health and Safety policy and procedures. They must use all
equipment provided to undertake their role safely.
 Refrain from smoking in any area of the Hospice premises not designated a smoking area, to
include a patient’s home.
 Conform to the professional standards set by their relevant professional body. Including ensuring
registration is current and continuous professional development is practised.
 Uphold the philosophy of the Hospice; this is based on an interdisciplinary approach and employees
and volunteers are required to participate in this concept.
