They will also provide information and support while you are waiting for your appointment. Things to bring to your appointment: • Your Ontario Health Card (or other provincial health card). • Your Ottawa Hospital green card (if you have one). • Your completed Self-Reporting Health History form. If you have not already received a copy, you will be given one when you register at the Cancer Centre. • The name, address and phone number of your family doctor. • A list of medications you are taking printed from your pharmacy. • A list of vitamins, minerals and/or herbal supplements you are taking regularly. We encourage you to bring someone with you to provide support during your visit. You may also want to bring: • A notebook and pen to record information. • Your drug benefits plan information or card (if you have one). • A list of questions you may have for the doctor or nurse. • A drink and/or snack from home (we do not provide meals at the Cancer Centre, but you can buy them at the cafeteria or at one of our coffee shops). 4 The Ottawa Hospital has a ‘scent free’ policy. Please do not use perfumes, colognes or other scented products on the day of your visit. Checking In All patients must register at the reception desk which is located at the main entrance of the Cancer Centre. You will be given a green hospital card if you do not already have one. Please carry your green hospital card AND your health insurance card (Ontario Health Card or other provincial health card) with you at all times. You will need to show both cards each time you come to the Cancer Centre for any appointment. Let the receptionist at the front desk know if there has been any change in your name, address, phone number, family doctor or emergency contact person. How Are You Feeling Today? (Symptom Screening) Each time you come to the Cancer Centre, you will be asked to complete a short survey on common cancer related symptoms that you may be having. Your answers will help your health care team better understand how you are feeling that day and provide the support you need. 5 Meeting your Health Care Team After checking-in for your appointment and completing your symptom screening survey, you will meet your cancer nurse (Patient Designated Nurse) and cancer doctor (Oncologist). Your team will review your medical history and reports, and talk about your cancer. They will talk with you about what treatment options are available. In most cases, this first visit will focus on planning for your care and will not include any treatment. Be sure to ask questions if there is something you do not understand. Please note, your first visit may be long in order to allow enough time for your team to be as thorough as possible. Parking at the Cancer Centre While we do our best to set aside parking spots for patients and families, finding a parking space at times can be a challenge. We recommend that you allow enough time to ensure you arrive on time for your appointment. There are patient pick-up and drop-off areas at all entrances to the Cancer Centre. However, these are no parking zones which are strictly enforced. 6 Where do I park at The Ottawa Hospital Cancer Centre – General Campus? Parking for patients coming to The Ottawa Hospital Cancer Centre will be in the Critical Care Tower parking lot (right beside the Cancer Centre). Parking for patients coming to appointments at the IGFCC (Queensway Carleton Hospital) is located in front of the main entrance to the Cancer Centre. Unfortunately, there is no free parking on the hospital site. Please note that all money received from parking fees is used to buy medical equipment and build new patient-care centres. There are several paid parking options. To choose the best option for you, please ask a member of your health care team. Please note—these prices are subject to change. Daily Pass TOH $13 IGFCC $14 Weekly Pass: $42.25 $50 Monthly Pass: $92 $105 Frequent User Pass: $105 The Frequent User Pass will remain active until you have made 15 visits in our parking system. Note —daily and frequent user passes allow for in and out privileges at both QCH and TOH sites. Weekly and monthly passes do not include access to QCH. If using a daily pass, it is important that you obtain and keep your receipt which proves you have paid the daily maximum in case you need to access the TOH or QCH that same day. How do I pay for my parking? All parking operations are automated. Take the parking ticket with you inside. 9 When you leave after your appointment you may pay at one of our Automated Pay Stations. You may use cash or credit card (machines do not accept debit cards — but ATMs are on site). You can also pay at any of the exits in the lots with your Credit Card. Where can I get a parking pass? You can buy the parking pass at the following locations: General Campus: Cashier (Main Level) or the Security and Parking office on the 2nd level. Queensway Carleton Hospital: Security office. All Security and Parking offices operate 24 hours a day. If I have a parking question who can I contact? Volunteers may be able to provide you with a free drive to the Cancer Centre. Call the Canadian Cancer Society at 1-888-939-3333 or visit www.cancer.ca for more information and to find the local office to best meet your needs. Home Support Programs may be able to provide a ride. There is a fee. Check with the program in your community. Connect with the Community Care Access Centre for more information on home support programs. 10 Ontario Works or Ontario Disability Support Program (ODSP) If you are connected with Ontario Works or ODSP, contact your Case Manager—they may be able to provide financial help for transportation. For Ottawa only: Para Transpo provides rides for people with disabilities who cannot get on and off the city bus. For more information, contact Para Transpo at 613-244-1289. A doctor’s signature is required to access this service. If transportation to and from the Cancer Centre remains a problem, more information is available through the Navigating Cancer Care information session offered by the Psychosocial Oncology Program. Helpful Information Navigating Cancer Care Information Sessions You will be given an appointment time to attend this session where you will receive important details on what to expect at the Cancer Centre. Your primary care provider helps take care of your health care needs that are not related to cancer. If you do not have a primary care provider, you can call Health Care Connect at 1-800-445-1822. This program connects people with family doctors and nurse practitioners who are accepting new patients in their community.
