In today's environment, good cli nical skills
alone do not ensure success, whelhe r sta rti ng a
practice. adding a new doctor to an existing
practice. or pu rchas ing an estab li shed practice.
More practice management skills are needed to
ensure professional success and to avo id making
costly mistakes . One major goal of the practice
management· cu rriculum is to educate docto rs in
how to think through management problems and
strategies , as we ll as providing some basic tools
that will be useful. The objective of a practice
management program is to prov ide the residents
with the concepts , tools and skills needed to help
them get their private li fe off to a good start and
he lp them see how the balance of professional
and personal life goals is possible with fewer
mistakes and more confidence in the residents
ea rly years . A structu red practice management
curriculum has been outlined in this article.

In t he ora l environment. orthodontic
atlachmenis are exposed to potentially damaging
physical and chemical agents . These conditions
may affect the amou nt of metallic co rrosion .
Recycli ng of meta l orthodontic brackets is of
current interest to the profession. In this study,
the authors analyzed the metal ions released from
new and recycled metal brackets aller imme rsion
in artificia l sa liva or buller solutions of varying pH
lor t ho ur and 12 weeks. Four dille rent brands of
metal brackets were analyzed. Unitek Dynalock
twin-torque brackets, Tony melal base brackets,
O r mco standard edgewise brackets and
Dentauram Rickel! brackets. New brackets were
used as supplied without heat treatment. Recycling
of the brackets was mimicked by app lying
adhesive to the bracket base and once set. heating
the bracket al 350°C for 30 minutes to burn out
the ad hesive. Brackets were immersed in the
relevant solutions and incubated at 37"C for ' 2
weeks. Afte r incubation , the solution was tested
by atomic absorption spectrophotometry for nickel ,
chromium, iron and manganese ions . Surface
analysis of the brackets was ca rri ed out by
Scanning electron microscopy. Resu lts showed
that the relea5e of ionic nickel was highe r from
the recyc led brackets than from the new. Overall .
the recyc led brackets re leased greate r amounts
of metal ions than did the new brackets, especially

in the ar tificia l sa liva and pH solution. and the
total amount of ion re leased increased with time .
Afte r recycling. th e composition of the brackets
did not change. If one is to use a recycled bracket,
the bracket should be as close to its original
condition as possible .

Developments in light-curing units have led
to the int roduction of a plasma-arc lig ht-curing unit
that del ivers high-intensity output and exhibi ts
accelerated lig ht-curing times :of 3 seconds per
site. This rapid curi ng feat ure saves considerable
chairs ide time when compared with conventional
light-curing un its. In this study the shear bond
strengths of LCG IC cured with a plasma arc lightcuring unit was determined and the amo unt of
residual adhesives on the corresponding ename l
surfaces was assessed . The du rability of the bond
formed between the bracket and th e enamel was
also evaluated with thermal cycling. Two bonding
adhesives were used in this study. Fuji ORTHOLC
(GC , Tokyo , Japan) and Light-Bond (Relia nce

bond strengths of the lCGIC, wi th the use of
conventional light-curing units after 24 hours, and
24 hours with thermal cycli ng were 25.8 ± 3.8 MPa
and 24.6 ± 4.9 MPa, respectively. Th ere were no

statistically significant differences between the
shear bond strengths of LCR and LCGIC cored
with a conventional light curing unit. There was no
statistical difference in bond strengths between
teeth that were thermal cycled and those that were
not. There is also reduction in chair time with
plasma-arc light curing unit wi thout actually
affecting the shear bond strength.
Effects of combined application of antimicrobial
and fluoride varnishes in orthodontic patients

Formation of white spot lesions on the labial
surfaces is a serious side effect of treatment with
fixed orthodontic appliances. This study was done
to investigate the effect of a varnish containing
Chlorhexidine and Thymol (Cervitec , 1 %,
Chlorhexidine , 1% thymol) in combination with a
lIuoride varnish (Fluor Protector, 5% difluorosilane)
on the development of white spot lesions during
fixed orthodontic treatment. The effects of the
varnish on the occurrence of gingivitis and plaque
formation were also studied . It was also
investigated whether white spot lesions cou ld be
predicted early during treatment. The antimicrobial
varnish Cervitec significantly reduced the number
of mutans streptococci in plaque during the first
48 weeks of treatment with fixed orthodontic
appliances. This effect did not result in significantty
less increments of white spot lesions on the labial
surfaces in the dentition compared with the group
receiving only fluoride varnish application . There
was a clear trend that the combination of the
varnishes more effectively reduced the increments
of new lesions of the maxillary inciso rs that is , in
the area where white spot lesions represent the
largest esthetic problem. Lesion development was
difficult to predict early after bonding , the best
predictors being visible plaque and mutans
streptococci around the appliance shortly after
bonding. No significant differences were found
between the groups with respect to gingivitis and
plaque.
