Post IV tPA Assessment Tracking:
INSTRUCTIONS:
 Write the tPA bolus administration time below.
 Begin post-intervention assessments 15 minutes after the bolus was administered.
 NOTE: Document actual vital signs and neuro checks in the Electronic Health Record (Cerner).

NOTE: Use this standard when transferring the care of an acute stroke patient to another RN. This includes handover
to ICU, interventional radiology, PACU, the floor or shift change.
1. Complete a neuro check exam together at handover.
2. Provide the following information to the receiving RN:
☐Symptom onset time if known or the date and time that the patient was last known to be well.
☐Neurological deficits.
☐Blood Pressure management including medications administered and high / low BP target range.
☐IV tPA or IA tPA administration. Time that the 50mL saline bag is due post IV tPA.
☐Endovascular interventions including diagnostic angiogram, clot retrieval, stenting, closure device.
☐Level of sedation or anesthesia including current airway management.
☐Complications or concerning co-morbidities.
☐Family location/contact information.
