Eligibility Requirements
The agency that applies for the Incentive Award (lead agency) must meet the following
eligibility requirements:
 is a 501(c )(3) that is located in or has headquarters in Cuyahoga County;
 provides mental health and/or drug and alcohol addiction treatment services;
 is financially stable as demonstrated by an unqualified audit and management
letter and;
 is an ADAMHSCC Board contract agency or has accreditation from a national
body such as the Joint Commission, Council on Accreditation (COA), or CARF
International, and;
 has received Woodruff Foundation funding in the past.
Lead agencies that meet these criteria may collaborate with each other and/or with any
other Cuyahoga County nonprofit organization from health, human service, education or
other sectors.
