Final choices form for AS courses for September 2014 Form.......................... Student’s name........................................................................ To be returned to Mr N H Denman via your son’s Form Tutor by Thursday 6 February. If parents wish their sons to leave Trinity at the end of the 5th Year, then, as usual, a term’s notice needs to be given. If a parent chooses to give ‘provisional’ notice while they are considering other options then if your son does leave we will ensure that a term’s fees are not payable. However, if he later changes his mind and wishes to stay, we cannot guarantee that a place will be available in September. If a boy does not achieve the necessary points to qualify for the 6th Form and so is unable to continue with us, then no fees will be charged for the Autumn Term. Please tick one box below to indicate your plans. My son will: definitely be staying provided he qualifies for the 6th form probably be staying and we will clarify our position before the end of this term probably be leaving Trinity and we will give provisional notice before the end of this term definitely be leaving Trinity. This will be taken as giving formal notice if ticked. 1. 2. 3. 4. 5. 6. Please note that courses can only run if there is sufficient demand. It is not possible to choose both Art and Photography. Those considering the accelerated maths course leading to further mathematics should discuss their options with their maths teacher or with Mr Wickes. In most cases those choosing the accelerated course should choose 3 other subjects below to study at AS-level, but it may be possible for some students to take accelerated mathematics with only 2 other subjects. We will try to offer as many combinations as we can, but it is not possible to offer every combination. If this applies to your choices then we will contact you after half-term to discuss this. The minimum GCSE grade criteria for each course is shown overleaf. In order to reserve a possible place in the 6th Form you must choose 4 subjects below by 6 February. It may be possible to change your mind later, and you should contact Mr Denman as soon as possible if this applies, but after submitting this form we cannot guarantee that changes will be possible. Unless your son is definitely leaving please indicate the four requested subjects by ticking four boxes below. Upper Sixth Progress to A2 will normally require at least a grade C in the subject at AS-level. Upper Sixth Form students will take at least three A-level subjects and, for many, three will be sufficient. Decisions about which subjects to continue with to A-level will be made during the Summer Term of the Lower Sixth or, where necessary, after the AS results are known in August. Most A level courses will require a further two modules to be taken at the end of the Upper Sixth year.
