To activate Standard Referral in Healthone. Click Tools, Options Select Healthlink- Referrals. Ensure Standard Referral is ticked and Work Offline is not ticked as below Using the electronic referral form Record your consultation notes as usual in a new transaction in the patient’s file. Using SOAP works well with this form. A number of fields in the form are mandatory, i.e. they must contain information. This can apply to both administrative fields (such as “address”) and clinical fields (such as “reason for Page 1 referral”). The referral form won’t open if the mandatory administrative information is not already present in the patient file. A message window will open advising you which administrative data is missing. The mandatory clinical field headings will appear in red, you must complete these if they do not already contain the relevant information you wish to send. Prior to opening the referral form, it is recommended that you confirm that the patient’s contact details (address and phone number) are up to date. The electronic referral form can be opened by clicking on the icon as indicated and choosing “standard referral” Page 2 The electronic referral form will open in a new window, and establish a connection with Healthlink. For convenience it may be useful to maximise this window by clicking on the icon as indicated. Page 3 Patient and referral destination details: For female patients, please type “yes” or “no” in the pregnant field. This field will not appear for male patients. If the patient already has a Healthlink record available, you may choose to use this record instead. However if you have confirmed the patient details are up to date, then it is best to use your patient details, (included by default). This icon will allow you to update the GPs details in the user database (this can also be done by accessing the user database in the usual way). The GP details must be in the database for the referral to work, if they are not present, a window will open which will allow you to update the database Typing the referral priority, either”urgent” on “routine” From the drop down menu select the hospital and the appropriate specialty. Only the hospitals and specialties which are accepting online referrals will be available for selection. Enter “yes or “no” for previous hospital attendance if known. Type the name of your preferred consultant if you wish in the fields provided (optional) Page 4 Clinical information: Clinical information recorded in the patient file can be included in the form. Some of this is included automatically, but you can configure the form to include information from specific health one items in your referral. You can type free text and edit the content in these boxes as you wish. The red icon next to some of the fields on the right hand side allows you to extract information from specific Health One items in your active transaction (clicking on the red icon will open a window, and allow you to select a health one item you want to use, and import it to the right hand column, so that this item will be used as default). For example, if you are using SOAP to record your clinical transaction, you may wish to configure the form to include information as follows: SOAP item Subjective symptoms Field in form History of presenting complaint Objective findings Assessment Clinical exam Reason for referral Other fields can also be configured by clicking on their associated red icon: e.g. “History of past illness” can be configured to include the health one item “medical history” or “problem”. Page 5 Ticking the vital signs box will include the last value of pulse, BP, BMI and their date recorded. Adding lab and radiology results: Lab results can be added by clicking on the “add” button next to Lab investigations. This will open a search window, which will allow you to type in the health one item you are looking for, e.g. cholesterol. By highlighting the item you wish to include and clicking “ok”, the item is added. Multiple values can be selected by highlighting several items at once and clicking ok. This process can be repeated to include other items, for example haemoglobin or creatinine. Page 6 The content of radiology reports can be similarly added by clicking the “add” button next to rad investigations, and searching for the appropriate health one item you use to store radiology reports. Click “OK” and the result is added. At any time, the content of the patient’s file can be searched if you need to check something, by clicking on the “All HCR Data” button. Page 7 Alcohol and smoking status can be entered here, or these fields can be configured to import data from the patient’s file from specific health one items, by clicking on the red icons next to the fields in the same way as previous outlined for other fields. If you wish to include a parent’s or relative’s name in the referral, these should be entered here. Again, you can configure this field to import data from a single Health One item, e.g. ”next of kin” in the basic administrative page of the patient’s file. Page 8 Adding a list of medication: The current medication field can be used to add your repeat medications list. You must click on the “Add” button next to the current medication field. This will display a selection window of the patient’s repeats medications. You can un-tick particular medications if you do not wish to include them in the referral, for example catheters or test strips. Please type yes or no in the “patient on anticoagulants” field Adding other comments: You can add free text in the in the additional relevant information field if you wish. This field can also be configured (see previous comments about configuring items). Page 9 Sending and saving the referral: When you have completed entering information in the referral, the referral can be sent electronically by clicking the OK button at the end of the form. The form will be saved in the transaction as highlighted below. In the comments area, “accepted” will indicate that the form was sent successfully. This is an acknowledgement message generated by Healthlink, to confirm transmission of your electronic referral. If you do not see “accepted”, the form has not been received by Healthlink and you should either try to re-send the form later, or print and post, or fax, the referral form. Page 10 Following consultant triage, within 5 working days, the hospital will send an electronic message You will receive this Response Message via Healthlink from the hospital in the same way as a lab result will be received, and it will advise about the outcome of consultant triage as urgent or routine, information of the patient’s appointment if available, or placement on a waiting list. If you do not receive this response message within seven working days, a problem may have occurred at the hospital in processing your referral, and you may wish to follow up with the hospital. Page 11
