Simulation activity running sheet About the simulation Title: Radiation safety and communication Date: Duration: 60 minutes Venue: Faculty Facilitator/s: Simulated patient/s: Confederate/s: Learning objectives At the conclusion of this simulation, participants will be able to: 1. Identify the key safety aspects involved with working in a radiation area. 2. Demonstrate the appropriate communication required when interacting with radiation therapists delivering patient treatment. 3. Demonstrate safe conduct in a radiation area. 4. Manage a highly distressed patient in a radiation area (Radiation Therapy students only). Scenario – Health Professionals Participants need to urgently speak with either the radiation therapist or the patient who is currently receiving treatment. The contexts will reflect their usual clinical role: Social work Needs to follow up with a patient who is currently in receiving treatment about a referral to services. Dietetics Needs to liaise with to the radiation therapist about PEG feeds for another patient who will be receiving radiation therapy. Occupational Therapy / Physiotherapy Needs to communicate with the patient receiving treatment about follow up for their lymphoedema. Medicine: Needs to communicate with the patient receiving treatment about changing their medications to better manage their treatment symptoms. Nursing Needs to speak to the radiation therapist about organising chemotherapy appointments for a patient that is attending for treatment later that day. Scenario – Radiation Therapy students Participants are assuming the role of radiation therapist. Participants will need to collect the patient from the waiting room, bring them into the treatment room, and prepare them for treatment . Treating radiation therapist: • Seated at the console, away from the treating radiation therapist. Working on the computer. • Seated at the console near the monitors, operating the Linear Accelerator • If approached by the participant, then greet them and assist them with their enquiry. • Should ignore the participant unless the participant attempts to get their attention (eg. speaks or waves) • If the participant approaches the treating radiation therapist, do not intervene until the treating radiation therapist asks them to assist. • If the participant attempts to get their attention, then respond, but indicate that the patient’s treatment has been interrupted. Ask the person to speak to the non-treating radiation therapist. If participant entered the treatment room without invitation from the radiation therapist, what could have been the potential consequences? BEAM OFF sign illuminated 1 x radiation therapist 1 x participant Observers standing near the entrance of the MAZE but not blocking access. • If the participant calls out from the entrance of the treatment room, then respond to them appropriately and you may come around to speak to them at the treatment room entrance. If the participant enters the treatment room without invitation from the radiation therapist, then indicate that it is inappropriate for them to enter as the patient may be indecently exposed and they will be invading the patient’s privacy as well as delaying the preparation for their treatment. • When the participant notices console unattended, the participant approaches the entrance of the treating room, but does not go past the door. • Participant calls out to radiation therapist from the entrance of the treatment room and states who they are and asks whether the radiation therapist would be able to speak to them about their enquiry. Scenario 3: Radiation Therapy students Setup Faculty actions Expected participant actions Waiting Area Simulated patient: Participant: Treatment room with a treatment table. • Sitting in the waiting area. May be slightly fidgety or appearing somewhat anxious. - Another private room (eg. nursing room) • The participant will come to collect the patient and bring them around to the treatment room. • Collects the patient from the waiting area and brings them to the treatment room. BEAM OFF sign illuminated • The participant will ask the patient to put on a gown and then lie down on the treatment table. • Uses appropriate and polite professional communication when interacting with the patient. 1 x simulated patient • When lying down, the patient’s anxiety increases. This might present as heavy breathing, asking lots of questions, or crying. • Gives clear instructions to the patient about preparing for treatment. • Respects patients privacy when putting on the gown. • Places the mask on the patient, but immediately removes the mask when the patient becomes very distressed. • Brings the treatment table down to a safe height for the patient, and considers patient safety at all times. • Assists patient to sit up and uses communication strategies to help the patient calm down. • If/when appropriate, take the patient around to a private room. • Call nursing staff and make a referral to the social worker/psychologist if patient agreeable. Observers standing in the treatment room in an unobtrusive position. • When the participant places the mask on, then the patient becomes extremely anxious and begin yelling and trying to take off the mask. • The patient gradually decreases anxiety as the participant uses appropriate strategies to calm them down.
