Why do I need to go to this class?
During this class you will learn how to make healthy lifestyle choices.
The goal is to prevent future strokes. It is important that you attend this class.
What will I learn during the class?
During class, you will learn:
• How to make changes in your life to reduce your risk for stroke
• What factors put you at risk for having a stroke
• What programs and services you can use in the Greater Toronto Area
• How to make healthy food choices
• How your medicines work
• Why you need to exercise
How will I learn in the class?
We know people learn best in different ways. The presenters will use different ways to give you information such as:
• handouts
• pamphlets
• power point presentations
• models
We encourage everyone in the class to:
1. Ask questions
2. Share their thoughts
3. Use services available to you.
This includes:
• smoking cessation support
• exercise programs
• any other appointments with the health care team that you may need
4. Fill in the evaluation form

Who will teach the class?
The class will be taught by:
Advanced Practice Nurse (APN)
It is important to know what are the signs of a stroke. It is also
important to know how to prevent another stroke.
The APN will talk about:
• Ontario stroke system
• What programs and services you can use in the Greater
Toronto Area
• Signs and symptoms of stroke
• What puts people at risk for stroke
• How to reduce those risks
Physiotherapist
You will need to exercise regularly. It will help you recover
from the stroke and will prevent future strokes.
The Physiotherapist will talk about:
• How exercise can help you
• Tips on how to start exercising
• How to join a supervised exercise program at
Toronto Western Hospital
Pharmacist
It is important for you to take your medicine as scheduled.
The Pharmacist will talk about:
• How to maximize benefits of your medicine
• How to minimize side effects

Registered Dietitian (RD)
It is important to make healthy food choices. It will help
reduce your chance of getting a stroke again.
The RD will talk about:
• Ways to improve your cholesterol and blood pressure
with food choices
• How to include the four food groups for a balanced diet
• The benefits of healthy weights
Occupational Therapist (OT)
You will need to make healthy lifestyle changes after a stroke.
These changes will help to prevent future strokes.
The OT will talk about:
• How to develop meaningful goals for healthy lifestyle
changes
• How to start and maintain these changes into your
daily life
What do I need to bring to class?
You will need to bring:
 Your health card (OHIP card). If you do not have an OHIP card,
please bring another form of government-issued photo ID (such as
a driver’s license, passport, or other provincial health card).
 A list of what medications you are taking.
Please feel free to bring your family members and caregivers to the class.
They are always welcome.

What should I expect after the class?
After this class you may be referred to other programs such as:
• Quit smoking programs
• Exercise programs
• Appointments with other health care providers

If you see these signs, you may be having a stroke.
The signs of a stroke often appear quickly. They can last for a long
time or could go away quickly.
Call 911 right away if you notice these signs:
Weakness
SUDDEN weakness, numbness and/or tingling in the
face, arm or leg
Trouble seeing
SUDDEN loss of vision, especially
in 1 eye or double vision
Trouble speaking
SUDDEN troubling speaking or trouble understanding
someone talking
Headaches
SUDDEN, severe headaches that are not normal
Dizziness
SUDDEN unsteadiness especially with any of the
above signs
