North Canyon Medical Center Job Description Title: Licensed Practical Nurse Dept Number: 602 Reports to: CNO Status: Non-exempt Unit/Area: Nursing Areas Position: LPN Revision Date: 04/10 I. JOB SUMMARY The Licensed Practical Nurse (LPN) is responsible for identifying and reporting patient needs through data collection, planning, intervention and evaluation and delivery of appropriate care according to nursing standards and professional practice as outlined by the nurse practice act for LPN’s. Collection of data is via interview, observation, auscultation and palpation. Through documentation and oral communication, essential information is shared with health care team members and other customers common to the unit/area(s) of assignment. II. DUTIES AND RESPONSIBILITIES A. Clinical Practice: 1. Administer oral medications, giving injections and carrying out treatment orders, which do not require direct supervision of an RN. 2. Transcribe physician medication orders accurately onto patient’s individual medication profile. 3. If certified, may start and set up lines, hang premixed fluids, monitor and discontinue peripheral IVs as necessary. 4. Give direct care to patients. 5. Perform treatment or assists in treatments and procedures. 6. Assist medical staff in examining and treating inpatients and outpatients if RN is unavailable at the time. 7. Keep charge nurse informed regarding changing conditions of patient’s responses to interventions and other pertinent observations. 8. Contribute to the admission assessment & evaluation including vital signs, allergies, history & weights within 1 hour of admission as assigned. 9. Contribute to the ongoing patient assessment as delegated by the RN 10. Work with Discharge Planner when indicated for their services. 11. Review the discharge instructions with the patient/family if requested by charge nurse. 12. Maintain a safe clinical environment and intervene with proper technique procedure and safety precautions to meet individual needs of patients. 13. Maintain a safe environment for patients and personnel 14. Complete paperwork and patient clinical documentation within time frames specified in policies and procedures. 15. Document charges on proper form and complete charting assignment as directed by the charge RN. 16. Document and sign all events personally performed or observed. 17. Review physician orders and med profiles for missed orders and proper transcribing. 18. Contribute to efficient use of hospital supplies and equipment by maintaining sufficient supplies by reordering and restocking and avoids waste of supplies. 19. Share in responsibility for maintaining the facility in a clean, orderly manner by cleaning & maintaining equipment after use according to policies and procedures. 20. Perform other duties as assigned. B. Professional Communication 1. Maintain confidentiality in matters relating to patient/family. 2. Interact with patients/families with a variety of developmental and sociocultural backgrounds. 3. Provide information to patients and families to reduce anxiety and convey an attitude of acceptance, sensitivity, and caring. 4. Maintain professional relationships and convey relevant information to other members of the health care team within facility and any applicable referral agencies. 5. Communicate accurate and pertinent information with patient care providers and other members of the interdisciplinary care team to facilitate effective and efficient patient care during the tour of duty. 6. Relay information appropriately over telephone, pagers, computers and other communication devices. C. Teamwork 1. Accept assignments based on patient census, acuity, needs, and the qualifications and competencies of self and of other staff members. 2. Work closely with other care providers to complete the initial and ongoing assessment of patients and to provide clinical assistance as needed.. 3. Report on patient conditions to those who require information including physicians, Registered Nurses and other members of the interdisciplinary health care team. 5. Initiate problem solving and conflict resolution skills to foster effective work relationships with peers. 6. Serve as a role model and mentor to peers and students in the health professions. D. Professional Development 1. Attend staff meetings, in-services, and continuing education as required for the position and the specific unit/area(s) of assignment. 2. Contribute to the annual review of unit/area(s) based scope of care statement and determination of incorporating aspects of care for clinical review. 3. Contribute to annual reviews of peer performance as requested by the unit/area supervisor, manager, or director. 4. Assist in the development of indicators, thresholds, study methods, and data collection as assigned. 5. Respond to problems/opportunities to improve care/customer service. 6. Support involvement in the hospital's Performance Improvement initiative. 7. Participate in and maintain competencies required for the position and specific unit/area(s) of assignment. 8. Graduation from a School of Licensed Practical Nursing with current LPN licensure from Idaho. B. Minimum experience as required for the position and/or specific area(s) of assignment. C. Excellent communication skills to include oral comprehension/expression and written comprehension/expression. E. PHYSICAL REQUIREMENTS Frequent standing, stooping, kneeling, squatting, twisting/turning from waist pushing/pulling objects on rollers/wheels bending,. Frequent walking, reaching above shoulder level, moving objects horizontally (left to right) and vertically (up and down) up to 25 lbs, sometimes up to 50 lbs, employees are to ask for assistance when lifting more than 50 lbs, limited sitting. Frequent handling (holding, grasping, working with hands), fine finger manipulation (pinching, picking, working with fingers). Senses needed to perform the essential functions of the position. Frequent need to hear and talk in quiet/noisy surroundings and talk/hear with over the telephone. Vision for near/far acuity, depth perception, color vision. Touch for feeling (size, shape, temperature, texture by finger receptors). Smell to determine hot/burning equipment.
