State-of-the-art cardiovascular magnetic resonance Cardiovascular Magnetic Resonance Center The UPMC Cardiovascular Magnetic Resonance Center, located at UPMC Presbyterian, can accurately and precisely characterize almost all forms of cardiovascular disease through the use of our short and wide bore, “smart” magnetic resonance scanner. •• the shorter, wider opening is more comfortable for patients and can accommodate larger patients (up to 550 lbs) and those with claustrophobia •• has the capability for cardiovascular exams without breath holding, even if arrhythmia is present •• results can be sent electronically within 24 hours •• results are read by a multidisciplinary team of radiologists and cardiologists, who are available for consultations Indications for cardiovascular MR: •• Ischemic Heart Disease >> Optimize case selection for revascularization candidates by distinguishing viable (hibernating or stunned) myocardium vs. MI, identifying clinically unrecognized MI, and so forth. >> Assessing ischemia, especially when other tests are equivocal or when ischemia is suspected post revascularization; distinguishing ischemic from nonischemic cardiomyopathy; excluding ischemia in obese patients or young people without the need for radiation. Obesity poses little problem for image quality in contrast to other modalities. Obese patients can benefit from the entire spectrum of CMR capabilities. >> Assessing myocardial edema/stunned myocardium in the days following acute coronary syndromes to identify the culprit vessel in multi-vessel disease, the area at risk, and the extent of myocardial salvage. continued > Indications for cardiovascular MR (CONTINUED): •• Cardiomyopathy >> Quantifying LV function precisely >> Evaluating scar in nonischemic cardiomyopathy >> Assessing infiltrative/inflammatory heart disease, pericardial vs. restrictive heart disease, hypertrophic cardiomyopathy >> Evaluating valvular and congenital heart disease >> Assessing iron overload in hemachromatosis and other disorders •• Pathology underlying ventricular arrhythmia >> Evaluating for unrecognized MI, ARVD, myocarditis, hypertrophic cardiomyopathy, anomalous coronaries, occult cardiomyopathy •• Mass or thrombus assessment •• MR angiography with or without contrast •• Diastolic dysfunction: pericardial vs. myocardial disease FAQs Is there contrast involved? What type? Yes, mostly gadolinium contrast dye, which is usually very well tolerated if the GFR >30 mL/kg per 1.7 m2. No iodinated contrast is used. Some scans do not require contrast. Is the procedure covered by all major health plans? Yes, but restrictions vary. Please check with your patient’s insurance carrier to be sure that the procedure is covered by UPMC Presbyterian.
