Dr. Donald S. Clem received his certificate in periodontics from the University of Texas at San Antonio in 1984. He is a Diplomate of the American
Board of Periodontology and the recipient of numerous awards including
recognition for Outstanding Contribution to the Field of Periodontics by the
California Society of Periodontists and co-recipient of the Clinical Research
Award from the American Academy of Periodontology. Dr. Clem is currently
in full-time practice in the fields of periodontics and dental implants in
Fullerton, California. In 2011, he served as president of the American
Academy of Periodontology.

In healthcare, the interdisciplinary approach relies on the synergy of the care
givers to achieve optimal results. This
approach is clearly the standard of care
in medicine. Gone are the days of the
“family doc” who treated all one’s ills.
We are living longer and better than ever
before in history with advances in healthcare occurring at a rapid
pace. Some believe that in dentistry the age of the “superdentist”
is upon us and this would seem counterintuitive to the rest of
healthcare. This presentation will explore the recent advancements
in regeneration, tissue engineering and periodontal medicine that
can assist the orthodontist and the periodontist to manage high
patient demands as well as the complexities of their conditions.
Who are we as specialists and how can we not only work better
together, but also engage our general dentist colleagues, to insure
that patients receive the benefits of our advancing technologies?

Orthodontists are treating an ever increasing number of adult patients. These
patients come to the orthodontist with
a varied set of presenting problems,
including a risk for periodontal disease.
The orthodontist is responsible for evaluating both the periodontal disease state
for patients and potential risk for periodontal problems prior to starting treatment. This presentation will report the recommendations
of a joint AAO-AAP taskforce, providing a periodontal screening
protocol as one of the screening options open to orthodontists.

• Calculate the patient’s risk for developing periodontal problems during orthodontics
• Recognize the risk of unmanaged periodontal disease to orthodontic – patients
and potentially to your practice
Dr. Lee Graber received his dental and Ph.D. degrees from the University
of Michigan and his orthodontic M.S. degree at Northwestern University. He
has served on the editorial boards of multiple journals, and has contributed
to the clinical and research literature, most notably as co-editor of the 5th
edition of Orthodontics: Current Principles and Techniques. A member
of the joint AAO-AAP taskforce and past AAO president, Dr. Graber has
worked to increase inter-specialty communications to enhance patient care.

The Use of CBCT in Periodontal and Orthodontic Diagnosis

This presentation will provide an overview
on biotherapeutic approaches that affect
bone remodeling and turnover during situations germane to both periodontology
and orthontics. The application of host
modulation therapies that can increase
alveolar bone mass locally (e.g., teriparatide and anti-sclerostin) as well as
regenerative agents (e.g., BMPs and PDGF) will be highlighted in
the oral, dental and craniofacial arena. Both preclinical and early
human clinical trials will be showcased with application to the clinical practice setting.

• Examine the role of locally and systemically delivered host modulation agents
that can affect tooth movement, retention and stability
• Review clinical trials that use biotherapeutic agents to optimize regenerative
therapies in patients at risk for low bone mass or destruction due to periodontitis

With regard to the inconveniences
related to 2D technology, a request for
CBCT is justified for detailed diagnosis
of intrabony defects, measurements of
buccal and lingual defects in dehiscence
and fenestration defects, and furcation
lesions. Such assessments aid clinical
decisions when defects can be improved
with orthodontic treatment and when
defects require pre-orthodontic periodontal surgical intervention.
CBCT is also useful in selected cases for assessment of periodontal
bone healing and changes over time in adult orthodontic patients
with underlying periodontal problems. Recent work with limited
field of view CBCT systems has demonstrated the detailed characterization of the periodontal structure by high-resolution systems.

Dr. William Giannobile is the Najjar endowed professor of dentistry and biomedical Engineering and the chairman of the Department of Periodontics
and Oral Medicine at the University of Michigan School of Dentistry. He received his DDS from the University of Missouri. He later received his Certificate in Periodontology and Doctor of Medical Sciences in Oral Biology
degree from Harvard University. He subsequently completed postdoctoral
training in molecular biology at the Dana-Farber Cancer Institute and Harvard Medical School. Dr. Giannobile previously served as a faculty member
at Harvard and at the Forsyth Institute in Boston. In 2006 he completed his
sabbatical as a visiting professor at the Biotechnology Institute of Regenerative Medicine at the University of Genova Medical School in Genova, Italy.
Dr. Giannobile currently serves as the editor-in-chief for the Journal of Dental Research. He is also a consultant to the U.S. Food and Drug Administration for dental devices. Last year Dr. Giannobile received the Distinguished
Scientist Award from the American Academy of Periodontology for his research contributions to the field. He is currently the president of the American Academy of Periodontology Foundation.

Dr. Lucia Cevidanes joined the faculty as assistant professor in the Department of Orthodontics at the University of Michigan School of Dentistry in
July 2011. Dr. Cevidanes completed her specialty training in 1994, received
her Ph.D. degree in 2003, and was a faculty member in the Department of
Orthodontics at the UNC School of Dentistry from 2005-11. She is a Diplomate of the American Board of Orthodontics and is a practicing clinician in
the UNC faculty associates practice. Dr. Cevidanes has published over 60
scientific papers on 3D imaging and her work has been recognized by the
2005 American Association of Orthodontics Graber Award; the 2006 Dewel
Award for the best clinical papers in the AJO-DO; the 2011 Wuhermann
Award from the American Academy of Oral and Maxillofacial Radiology;
K23 and R03 awards from the National Institutes of Health; and Faculty
Development and Biomedical Research awards from the American Association of Orthodontists Foundation.

The era of in-office Cone Beam Computed
Tomography (CBCT) is transforming interdisciplinary dentofacial therapy (IDT).
While in-office CBCT imaging may be the
latest technological breakthrough influencing IDT, expanding our opportunities,
and demanding that our specialties become more inter-related than ever before,
it is diagnosis that has remained the
constant variable critical for predictable patient outcomes. Today’s
IDT team depends upon the periodontist to establish an accurate
diagnosis and, especially, set short and long-term prognosis- a role
that requires risk assessment exceeding conventional methods.
This presentation will discuss the critical role of the periodontist in
IDT from a diagnosis and prognosis standpoint and how emerging
imaging opportunities fit into this relationship. Treatment planning
and execution of therapy will be demonstrated based on IDT analysis and 3D CBCT diagnostic inquiry.

Dr. George Mandelaris is a 1996 graduate of the University of Michigan
School of Dentistry. He completed a three-year postgraduate residency
program in periodontology at the University of Louisville School of Dentistry
in 1999 and obtained a Master of Science degree in oral biology. Dr.
Mandelaris is a Diplomate of the American Board of Periodontology and
an examiner for Part II of the American Board of Periodontology certification
process. He is the current president of the Illinois Society of Periodontists
and is in private practice at Periodontal Medicine & Surgical Specialists,
LTD, in Park Ridge and Oakbrook Terrace, IL. He limits his practice to periodontology and dental implant surgery.

Three-dimensional (3-D) imaging rendered
by Cone Beam Computed Tomography
(CBCT) facilitates refined diagnosis and
enables fine-tuning of treatment plans in
specific types of cases. In patients with
impacted and transposed teeth, the information derived is particularly important in enhancing efficiency of treatment
andrates of success and in minimizing
collateral damage during treatment. In his lecture, Dr. Kapila will
present the indications and uses of CBCT imaging in diagnosis and
treatment planning for impacted and transposed teeth. He will address protocols for communicating with the periodontist to achieve
optimal surgical access, bond placement and soft tissue support,
and the utilization of this information to implement efficient threedimensional mechanics. He will also discuss the use of refined
spring designs to tract the teeth along the shortest path through
bone while minimizing damage to roots of neighboring teeth.

from the University of California—San Francisco. Dr. Kapila has received
several research awards including the AADR Hatton Award, the AAO Milo

Foundation. Dr. Kapila is a Diplomate of the American Board of Orthodontics.

Dr. Sebastian Baumgaertel is a clinical associate professor and director
of the subspecialty clinic for skeletal anchorage at Case Western Reserve
University in Cleveland, OH, where he also maintains an active orthodontic
practice. He has lectured nationally and internationally on orthodontic miniimplants and on Cone Beam Computed Tomography to both orthodontists
and oral surgeons. Dr. Baumgaertel is well-known as the author of multiple
scientific articles and co-editor of two orthodontic textbooks: Mini-Implants
in Orthodontics – Innovative Anchorage Concepts, and Self-ligating Brackets
in Orthodontics: Current Concepts and Techniques.

School of Dental Medicine, and his orthodontic master’s degree from the
West Virginia University School of Dental Medicine. He has been very active
in organic chemistry research and invented/developed a line of dental cements, one of which is still on the market today. He and his brother, Dr. Tom
Wilcko, a periodontist, invented/developed the PAOO System to help accelerate the rate of orthodontic treatment but more importantly increase and
enhance the periodontal support and stability of teeth following treatment.
They have been lecturing and conducting certification courses on this procedure since 1998. Dr. Wilcko has co-authored many peer-reviewed articles,

Localized Controlled Bone Injury to Enhance the Rate of Tooth

The physiological response of the
Periodontally Accelerated Osteogenic
Orthodontics (PAOO) procedure will be
discussed along with the many benefits
of this treatment modality. These include
greater range of movement, greater
periodontal support for the teeth, and
of course much shorter treatment times.
Fewer extractions are required while
achieving improved facial aesthetics and
increased long-term stability. The greatest benefit with the PAOO procedure is
the creation of new bone surrounding the
roots of teeth in the direction of movement. It can also cover preexisting bone loss in certain situations. A thorough understanding
of various types of orthodontically induced bone response is important in choosing the correct treatment plan for each patient. A
range of treatments will be discussed including difficult situations
and exposure cases.

have been shown to modulate the speed
of tooth movement both in clinical and

One of the challenges facing orthodontists is the need to move teeth in areas
presenting with little gingiva or bone.
The possibility of creating or aggravating
an existing Mucogingival defect is always
present when dealing with a thin biotype.
Another concern, from the patient's
perspective, is the time spent wearing
brackets. Over the years, several surgical
techniques have been developed to address these issues and reduce overall treatment time. Piezocision™ is a minimally invasive
surgical procedure, combining micro incisions and localized piezoelectric decortication aimed at achieving rapid orthodontic tooth
movement, with minimal trauma and patients discomfort, while
strengthening the existing periodontium. The possibility of hard
and/or soft tissue grafting via tunneling during piezocision™
prevents or treats Mucogingival defects and allows for successful
esthetic and functional outcomes.

with an inverse relationship between
tooth size and alveolar width, or with a
labial orientation of the tooth relative to
the facial or lingual alveolar bony housing, or both. Periodontally Accelerated
Osteogenic Orthodontics (PAOO) is an
emerging therapy which has as a major
component, alveolar augmentation prior to the initiation of orthodontic therapy. By incorporating PAOO into orthodontic therapy,
where indicated, we minimize the risk of potential iatrogenic gingival
recession. This program will address the role of PAOO relative to
enhancing gingival biotype, as well as appropriate strategies for
incorporating PAOO therapy into routine orthodontic/periodontal
practice.

Options to replace missing, ankylosed or
non-restorable teeth in the anterior maxillary region are limited for the growing
patient. Treatment is usually aimed at
developing the alveolar ridge for future
dental implant placement. Autotransplantation of a premolar to the incisor
site offers permanent tooth replacement
using a natural tooth to achieve an optimal esthetic and functional outcome. This presentation will review
the surgical, orthodontic and restorative considerations for autotransplantation.
A minor amount of root resorption
accompanies all orthodontic tooth
movement. Fortunately, most patients
have the capacity to “heal” the resorptive
lacunae with the deposition of reparative,
cellular cementum. However, a few
patients apparently do not have the
capability to repair the root resorption
and suffer significant root shortening
during orthodontic treatment. Fortunately, previous research has
shown that the root resorption process stops when the orthodontic
appliances are removed. But what if an adult with previously
resorbed roots requires orthodontic retreatment at a later age?
Are these roots susceptible to further resorption? This presentation
will report the results of a study that evaluated the effects of orthodontic retreatment in a sample of subjects with significant previous
root resorption.

Diplomate of the American Board of Periodontology, serves as an affiliate
assistant professor in the Department of Periodontology at the University
of Washington and maintains a private practice in Tacoma, Washington.
Dr. Vincent Kokich Sr. is an affiliate professor in the Department of Orthodontics at the University of Washington in Seattle. He has published 21
book chapters, 90 scientific articles, and 48 review articles. He has lectured
to orthodontic societies in 50 countries, and has been awarded the Sicher
Research Award by the American Association of Orthodontists, the 2005
Distinguished Alumnus Award from the University of Washington School of
Dentistry, and the 2011 Albert H. Ketcham Award from the American Board
of Orthodontics. Dr. Kokich is currently the editor-in-chief of the American
Journal of Orthodontics and Dentofacial Orthopedics and has served as
president of both the American Academy of Esthetic Dentistry and the
American Board of Orthodontics.

is a fellow of the prestigious American Academy of Esthetic Dentistry, is a

patients with excess gingival display or
“gummy smiles,” it is often necessary to
also incorporate periodontal surgery
and/or restorative dentistry to achieve
ideal smile esthetics. In this presentation, Dr. Kokich will identify the threshold
levels at which gingival discrepancies become noticeable as well
as the various diagnoses associated with excess gingival display.
He will also discuss the parameters necessary to create age-appropriate treatment alternatives for these patients while emphasizing

Modern orthodontic and periodontal
therapies require a long-term stable
