A, B: Lateral and AP views of the left elbow demonstrate a very large joint effusion. Notice the enormous “sail sign” anteriorly (arrows) and the prominent
posterior fat pad (arrow head). The position of the capitelum is abnormal. A line drawn along the anterior cortex of the humerus should intersect the middle
third of the capitelum. In this case, such a line would pass along the anterior cortex of the capitelum. This indicates that the capitelum is displaced
posteriorly. Therefore, the patient has a mildly displaced supracondylar fracture. A very subtle fracture line is visible on the AP view (B) through the medial
epicondyle (arrow).
