Knowledge required related to skill (should

 Familiarity with the Trust’s discharge policy
and procedures
 Familiarity with current legislation relevant to
discharge of patients from hospital i.e.
Continuing health Care Assessment,
reimbursements, Single Patient Assessment,
Delayed Transfer of Care.
 An understanding of assessment processes
 Policy/ availability of hospital transport
 How and when to organise medication, TTO
drugs, dressings and equipment

 Using discharge planner to commence
discharge planning on admission
 Uses appropriate skills and tools to assess
patients needs in relation to self care
