MAPPING SCHEMA FOR
ICD-O-3 TO ICD-10-CA
DRAFT

OBJECTIVE:
The purpose of Mapping ICD-0-3 TO ICD-10-CA is done so that the core terminologies work to
achieve the vision of a comprehensive and useful vocabulary system to serve clinical care. It is
essential that conversion/mappings tables be available to CIHI approved vendors so that all data
submitted to Cancer Care Ontario (CCO) from the Integrated Cancer Program centres are
comparably coded and abstracted.
This document contains the following sections:

Appendix B: Process Flow – ICD-0-3 Mapping for Morphology
1. Reference Files:
a) ICD-0-3 (International Classification of Disease for Oncology – 3rd edition)
b) ICD-10-CA (International Statistical Classification of Diseases and Related Health Problems
10th revision, Canada-2007).
c) Surveillance, Epidemiology and End Results Program (SEER) program ICD 03-2 manual:

2. Mapping Methodology:
a) In the ICD-0-3, Topography, histology and behaviour codes have been mapped to
ICD-10-CA.
b) Additional histology and behaviour codes indicated by CCO have also been mapped to
ICD-10-CA.
Note: When one classification is mapped to another it is difficult to conduct a precise code
mapping due to differences in granularity, management of context and version control; inevitably
some specificity may be lost.
4. TOPOGRAPHY:

The IARC/IACR (International Agency for Research on Cancer) Web Conversion Tools for Cancer
Registries was used as a reference for the mapping of ICD-0-3 Topography to ICD-10-CA codes.
1) The mapping indicates a one to many relationship.
2) Specified behaviour types as well as morphologies are indicated for the correct mapping to
ICD-10-CA (as histology and behaviour are included in the ICD-10-CA code and description)
/0
for benign neoplasm
/1
for uncertain whether benign or malignant neoplasm
/2
for carcinoma in situ
/3
for malignant neoplasm, primary site
/6
for malignant neoplasm, metastatic site
3) Default ICD-10-CA codes have been assigned when mapping could not take place for
instances where laterality in ICD-10-CA was involved and in other specific mappings dealing
with behaviour types.

5. MORPHOLOGY:
It is important to note that the base document that is used for the conversion/or mapping the
morphology codes from ICD-O-3 to ICD-O-2 is the Surveillance, Epidemiology and End Results
Program (SEER) program.
1) The mapping indicates a one to many relationship
2) All of the ICD-0-2 morphology codes were validated with ICD-10-CA morphology codes.
3) For those ICD-10-CA morphology codes that were not available, default codes were used
with their corresponding behaviour types to reflect ICD-0-3 behaviour.
The default codes used are:
80000 Neoplasm, benign
80001 Neoplasm, uncertain whether benign or malignant
80003 Neoplasm, malignant
80006 Neoplasm, metastatic
80102 Carcinoma in-situ NOS
80103 Carcinoma NOS
Note:
 For the ICD-O-3 code with the behavior type “9” the default code was mapped to 80003
Neoplasm, Malignant.
 For the CCO defined morphology codes the above ICD-10-CA default morphology codes
were assigned.
