Account Set-Up and Qualifications
To open an account, please complete the enclosed information sheet and return it to us via mail or fax. Minimum opening
order is $1,500.00. There is a $50.00 minimum on all orders. In order to qualify for the dealer discount, 12 items, mix and
match must be ordered. Annual minimum of $6,000.00 is required to maintain dealer status.
