The Barton Home Health team includes: • Skilled nursing care • Physical Therapy • Occupational Therapy • Speech Therapy • Medical Social Services • Home Health aide What types of services are provided by the Home Health team? Our services include but are not limited to: • Monitoring medical problems and vital signs • Medication Management • Wound care • Blood draws • Physical rehabilitation • Patient and caregiver education Home health care is a wide range of health care services that can be given in your home and is usually less expensive, more convenient, and just as effective as care you get in the hospital or skilled nursing facility. The goal of home health care is to treat illness or injury and help you recover, regain your independence, and become as self-sufficient as possible. • Assistance with activities of daily living (e.g., bathing, light housekeeping) How do I get Home Health services? You need a doctor’s referral for Home Health services. Your doctor can refer you directly or we can assist in obtaining a referral. If you or your doctor have questions regarding starting home health services, please call 530.543.5581 or 775.782.1510. Will my insurance cover Home Health Care? Medicare covers Home Health Care at 100%. We accept Medicaid/Medi-Cal and private insurances as well - all of which may have some shared cost. Financial Aid is available to those who qualify. What should I expect? Once your doctor refers you for Barton Home Health services, we will schedule an appointment and come to your home to talk about your health history and your current needs. With that information we will establish a plan of care and begin to coordinate the Home Health services you will need. Communication with your physician is also imperative so he/she is an active part of your recovery process. NOTE: In order to cover home health care, most health insurance plans require that you are “homebound”. Generally speaking, “homebound” means that leaving the home is a severe and taxing effort, assistance is required to leave the home safely or significant shortness of breath occurs with minimal effort.
